Dosing advice

    Okay, all, I need your knowledge. Squallie has had high-ish BG numbers for a couple of days; I just tested him at +7 hrs and he's down to 67! Is this good or should I be worried? I'm going to monitor him to see if it drops any lower. He seems perfectly okay, maybe a little hungrier than usual.

    at 67 mg/dL, he is having a lovely number for +7.
    He can go as low as 50 mg/dL on a human glucometer for his nadir (lowest glucose between shots), safely.

    And check out the signature link Secondary Monitoring Tools for other assessments you can make to evaluate Squallie's health. Glucose is just 1 aspect - behavior, food & water intake & output, etc. tell us a great deal about our cats.

    It could be that the 6 IU BID dose may be settling in. Remind me what you're feeding Squallie (PTSD - I have major memory problems).

    He eats Purina ProPlan DM and either Fancy Feast or Friskies pate'. Getting him to eat the canned food is a challenge, I usually try to give it to him pre-shot but sometimes he just won't eat it.

    I just tested him again (+9 hrs) and his BG is 132.

    I am so frustrated! I don't understand why Squallie's numbers are all over the place! Is this normal?

    Hi, Lucy - How long a needle is on your syringe - 8mm? For I while I used the shorter 6mm ones on Bat-Bat (BD makes them, marked on the 1/2 unit, too). And that cut down on the problem considerably. Might be worth a try (unless your kitty is particularly thick-skinned).

    I'm going to guess there may be some bouncing going on. With 6 units, Squallie gets as low as 67 mg/dL (just yesterday). Cats often go lower at night, so it is possible she may have gone lower than that 67 overnight. Either of those situations sets her up for bouncing.
    I'm blind, discard the AlphaTrak notes. Saw Robin's thought it was yours.
    Plus, the 67 earns her a 0.25 units reduction on the AlphaTrak, too.

    Maybe put AlphaTrak at the top of your spreadsheet.
    Also, you've typed into the spreadsheet cell which is supposed to read > 500.

    Radar purrs when I test BG but the shot has been problematic for us after he started feeling better. When he shot me that look that says why did you do that?? I started distracting him with something yummy, even a bite of his favorite food is enough, and it's gone much better. I noticed after I was able to get him to stop walking away mid shot because he was distracted, my tenting technique got much better. As I improved he didn't give me that look. Don't know if that's helpful or not??

    No, just a bit OCD. And that 67 was so close to the 68 we use as a cutoff, that I checked what meter she was using.

    Lucy, I'm a bit confused. Your signature says Relion meter but BJ mentions Alphatrak.

    As I've been following your progress, I'd appreciate it if you could do a quick post to confirm which meter you're using. I'd feel better contributing to your threads being 'belt 'n' braces' sure which meter you use. :)

    Okay. What do I do about bouncing? I saw that on the spreadsheet, I don't know how that happened except my bluetooth keyboard goes crazy and types things on its own, I have to boot the tablet to make it stop. Can I just delete it?

    Yes, I use the Relion on a regular basis, I use the Alphatrak every other day for numbers for my vet (I do NOT put those numbers on his spreadsheet, I keep them in a separate notebook).

    Hi Lucy - can't help you with the loopy keyboard or any advice, but you can just type >500 back in the black square and it should overwrite it. If you have any issues (I noticed a few formatting issues) I can PM you my email and if you share the spreadsheet with me I'm happy to fix it for you!

    Squallie is a cutie (but I'm partial to tabbies).

    Yes, 8mm. He doesn't have thick skin but he knows how to tense it up so I have a hard time getting a "tent." I have bent needles on him before.

    I think Squallie is a cutie too, but I may be a teensie bit biased!

    Hi all, back with more questions *sigh*. Spoke with my vet today, she advised putting Squallie back on 7 units instead of 6. His PMPS was 397. Should I start the 7 units tonight (when it is more difficult to monitor him) or tomorrow AM? Any advice? Thanks!

    Can you set an alarm so that you can wake up to test somewhere in the +3 to +5 zone?
    (You only have one overnight # so far on the SS, so I'm "guestimating" @ nighttime-cycle nadir here ...) I've done the alarm thing so many times :rolleyes: myself.

    So then you should be ok starting the dosage increase tonight, as long as you can monitor. And you know how to steer with food to keep him out of the <50 hypo-zone?

    I do in theory, yes, I've never had to do it before. I do sometimes have a problem getting him to eat.

    Well, if while you're monitoring his BG and you see him dropping close to the hypo-zone, give him a little bit of higher-carb canned. (Ex: if it were Bat-Bat & she was dropping nicely at +2 and then hit, say, a 60 on a human glucometer @ +4 overnight, I'd steer w/food at that point, then retest in about 20 min. or so to make sure she's heading back up.)

    You say sometimes you have a problem getting him to eat. What are you feeding these days? How much per day & how often?

    In the event that you're having problems getting Squallie to eat and he goes too low, rub honey/karo on his gums then post here ASAP for help and also call your vet's out of hours service.

    Where are you with the food transition, Lucy? What's the split of higher to lower carbs?

    If you need to get out the higher carb food, be sure to just give a tsp or two. You do not want Squallie to get full the first time you have to give the high carb food because you might have to give him more. After you keep testing during those low numbers, you may find that you have to give more high carb food so you want for Squallie to be hungry enough to eat it as long as you need him to. In other words, do not give him the full meal course. :)

    He eats Fancy Feast or Friskie's pate' and ProPlan DM. I can only get him to eat about 1/2 can per day, 1/4 for breakfast and 1/4 for dinner. He much prefers the dry, and he gets that throughout the day, about 3/4 of a cup all told.

    He really doesn't want to transition to canned food, it's an uphill battle all the way. His canned food is all well under 10% carbs, not sure what the carbs with the ProPlan DM are.

    Robin, if you're still up, I just tested him at +3 hrs and he's at 186. I've got an alarm set to test him at +5 and +7 hrs. I'll keep his spreadsheet updated. Thanks for the support, much appreciated! :)

    Good morning to you, too! We are pretty good this AM. We made it safely thru the night, and before breakfast Squall's BG was 346. He ate well and got his morning shot, so I will monitor him thru the day. Poor guy's getting tired of having his ears poked, I may research how to do a test using a pad, although to me that seems like it would be even more painful.

    You can apply Neosporin on his ears. I use regular Neosporin on his ears when I am testing a lot but I would use the Neosporin that has a little pain med in it (I can't remember the name). I just put a little on my finger and apply along the edges of his ear. When drawing blood, Neosporin will also bead up the blood nicely.

    Am so glad you're around to monitor through the day; I always feel for people who can't be home to do that. (Btw, when you get a minute could you add your morning dose to the spreadsheet?)

    Re: Ear pokes - Yeah, at the beginning of Bat-Bat's treatment, I felt horrible about having to poke her, as she made it so obvious she hated it.
    (Trying to squirm away, a little bit of squawking at the pin-prick.) But she got used to it fairly quickly - I alternate ears from test to test - and now she just crouches on the chair ("assumes the position") & acts like it's a normal part of life. There really aren't that many nerve endings at the edges of their ears; I would think that getting pricked on the paw-pads would hurt more ... but then, I have no experience with that option because Bat-Bat BITES if I try to touch her feet!:eek:

    Thank you for the advice, I will try that! Does it have to be Neosporin or will any triple antibiotic suffice? I know Squallie will try to wash his ears, it won't hurt if he ingests what he gets on his paws?

    Oops,sorry, thought I already had entered it, my bad!

    Poor Squallie is such a good patient, but I know he hates it. I do everything I can to be quick and try not to hurt him, but he's been tested SO much over the last week that his ears are starting to bruise. He used to enjoy ear-rubs tremendously, now I'm afraid to rub his ears for fear I'll make them hurt. He also never purrs anymore, or pats me on the face, stuff like that he used to do to show me he was happy or feeling affection. It makes me so terribly sad :-(. I love him with all my heart.

    Aww, Lucy, I'm sorry you're feeling so low about that. It's hard on us when we're already worried because of the diabetes, and then we must do all these things to help that our kitties don't like & there's no way we can explain to them that it's for their own good. But trust me, he WILL become accustomed to all the things you must do to treat him effectively. His more affectionate behaviors will return as he starts feeling better.

    Discomfort at ear-poke time is far better than what can result from a hypo episode - and thinking of that is what helped me stay strong, even though I was a "tower of Jello" at times, inside. You're doing just fine!:bighug:

    Thanks for the words of encouragement. I know I'm doing all the right things, I just wish he understood. Adding to the stress is the fact that we got a new Bengal kitty just before Squall was diagnosed, and no one has really accepted her yet, especially my other Bengal. So there's lots of growling (Bengals make the most AWFUL noises!) and running and carrying-on. Squallie stays out of it but it affects him just the same.

    Hi Lucy,
    Someone may have already mentioned this, but in case not - after the ear test, folding some tissue over the ear and holding for several seconds until you're sure it's stopped bleeding will help prevent bruises.

    Thanks , Joan, I do this every time but for some reason his left ear seems prone to bruising, more than his right. I'm thinking of trying a finer lancet to see if that helps.

    I know this isn't about dosing issues, but I'm hoping someone here can help me with this. I need to find a low/no carb dry food that I can feed all 4 of my cats; i.e., affordable! Right now I'm feeding three different types of dry, each with it's own "specialty", and it's costing me a fortune. I just read something about Young Again No Carb and Innova Evo; if anyone has any knowledge of these, or any others, I'd really appreciate hearing it! Thanks!

    Bat-Bat's ear have "toughened up" nicely since we restarted insulin in at end of Feb. But I must admit that after being in remission for nearly a year and a half, it was like starting all over again, ear-wise. (Should have seen the look I got when she saw that meter come out of the case the first time ...)

    I'd say ... about a month. Or maybe two weeks. She tolerated it better & better as the weeks went by.

    Melanie, I think I've done something to screw up the spreadsheet again :-( I don't know why but today's date-box turned black when I entered the info, and somehow the info line for the 6th has moved back up the list again! I swear I don't do this stuff on purpose, I don't know what I keep doing that causes all this havoc! BTW, I just last night noticed the second spreadsheet for the Alphatrak, thank you so much, it's lovely to have all the info in one place (I just need to fill it in, now)!

    Hi Lucy,

    Try entering your dates in the format 07/06/2015 instead of 7/6/2015. If the spreadsheet is glitching, the date format might be a possible reason why it's going doolally on you.

    'fraid I'm a born-again techno- numpty. Can't help you with using sheets on a tablet (only use mine to view the spreadsheet, not to edit it). Maybe ask the person helping you (Melanie?) if they can do a date order sort on a copy of your spreadsheet to see if that might fix the problem.

    Sorry, couldn't help myself - had to go take a peek real quick make sure it wasn't something I did to mess you up!

    I can't tell you what happened but it's fixed now. I also put in a formula that added all the dates in so you don't have to type it every day. If you don't like it or didn't want it that way just let me know and I can change it back!

    Lucy can you tell me what this means? I didn't see any line for the 6th at all, not on either sheet, so I'm guessing it was never there. I've added a line for July 6 but wonder if you have the data? I'll try to track the history of changes to see if I can backtrack to your original sheet.

    Never mind, I found it! July 6 went missing on or about July 22 (thank you Google Sheets for letting me track it down!). I'm not a big fan of revisionist history, but in this case I'll take it!

    Lucy, have you checked your credit card statements to make sure Squallie hasn't ordered anything off Amazon when you weren't looking? ;)

    PS Can I just say how lovely Bat-Bat is? I am partial to black cats and have one at home - but Genghis was recently inducted into the BCC (Black Cat Club).

    Actually my boy Jet (aka French Fry, which is what my daughter calls him) is also a feral rescue and prone to …. toothy persuasion….

    Two of my cats chew everything they can get their mouths on, including fingers, toes and so forth. One loves to chew my fingernails, and the metal bristles on the slicker brush! Ouch! If there is anything - and I mean, ANYTHING - on the floor, no matter how small, they will eat it. They worry me too death!

    I love black kitties, there's something about them that always says to me that they have information we all want and they're not sharing!

    Thanks for checking on him, Robin :) He seems good tonight. His PMPS BG was only 219 and I was a little hesitant about giving him 7 units but I took a deep breath and gave it, figure it'll just be another long sleepless night of testing. But that's okay, if that's what it takes that's what we'll do!

    I just discovered I am out of test strips! I won't be able to get any more until at least tomorrow evening, so I will have to rely on the Alphatrak in the meantime. I will make sure to post Squall's #'s on his Alphatrak spreadsheet.

    Was reading through the above posts re: food ... Lucy, if you can't transition him out of eating kibble, please at least transition him from the Purina D/M dry to either of the two dry foods others have recommended (Evo or Young Again, I believe.) Because the D/M dry is @ 31% carbs, & you say he's eating about 3/4 cup of the D/M per day, right? (One of your earlier posts: He eats Fancy Feast or Friskie's pate' and ProPlan DM. I can only get him to eat about 1/2 can per day, 1/4 for breakfast and 1/4 for dinner. He much prefers the dry, and he gets that throughout the day, about 3/4 of a cup all told.)

    Lucy, that amount of high-carb dry every day is really shooting you & Squallie in the foot. I'm feeling fairly certain that this is playing a significant role in his numbers not coming down as much as you'd like: Squallie's getting a dry food that's 3 times the max. amount of carb content that a diabetic cat should be consuming. (Not beating you up here, just encouraging you. It took me a while to wrap my head around all of this the first time around with Bat-Bat, too.o_O)

    So if you must feed him dry at those times he refuses to eat any brand of low-carb canned, please transition him from the D/M kibble to a much lower-carb kibble. (Any extra cost will be cancelled out by less need for insulin over time.) Make that transition bit-by-bit, though - Ex: 75% DM/25% Young Again or Evo; then 50/50, then 25/75, etc., making sure you keep monitoring because his BG numbers will likely drop considerably (and his dosage of insulin will likely have to be reduced then, too).

    Keep on keepin' on, Lucy ... Squallie will get there, I know: because I can see (even from this far away) how much you adore your kitty! :bighug:

    A HUGE thank you for the heads-up! I was pretty sure I had heard that the DM was high in carbs but couldn't pin it down, so when my vet said that's what Squallie should be eating I just figured she knew better than me about good food/bad food for diabetic kitties! I have already done research on both Evo and Young Again and will be starting Squall, and all my babiies, on Evo as soon as it's delivered (the only thing I don't like about Evo is that it has a LOT of fruits/vegetables - I like the YA better but it's more difficult to get, where I live). I don't feel at all "beat up", lol, I really appreciate the advice and encouragement, and always want to do what's best for Squall and all my guys. I'm trying Tiki Cat canned because it says it has no carbs/no grains in most of it's formulas, but my goofy guys don't seem to care for it, either.

    And yes, I do adore him, so very much. He's been my dearest friend and confidant for 16 years :)

    Did you get the black formatting in the units cell fixed? All you need to do is right click on that cell, then clear all formatting.
