    HUMAN METER - NOVOLIN N INSULIN - Regular Assist Feeding via Syringe

    DIABETIC STATUS:
    Dx - 19 Dec 2016; Insulin started PM 23 Dec (extremely poor vet support caused delay)

    URINE KETONE STATUS:

    23 Dec - c. 12:00 EST - Positive SMALL (at vets)
    25 Dec - c. 06:00 EST - SMALL to TRACE (closer to Trace) - +7.5 post-insulin
    25 Dec - c. 12:30 EST - SMALL / +4.75 post-insulin / +6.5 post-subq

    Guidelines for Ketone Management from Meya14 (DON'T copy dosing! It's for a specific on Vetsulin.)

    MEDS:

    Potassium Deficiency - Supplement Administraton (1 tablet q12 for 2 weeks)
    - Treatment Start Date: 23 Dec 2016 (PM)
    - Most Recent Dose Given: 25 Dec 07:00 EST

    Sub-q Fluids - Vet Rx: 100ml every 24-48 hours (depending on oral fluid intake)

    If I'm right it's 16:07 EST - you're slightly running behind schedule. Are you ready to give the insulin?

    I've not heard from Linda or Meya so in the absence of other input please stick to the 0.25IU dose.

    confirmation that it's the pates? I'm not sure I understand, it's the fancy feast classics. sorry, it's Heather again, I just woke up and am now trying to catch back up on things.

    Was the 189 on the first or second cycle of 0.25 u? If on the second cycle I think it's safe to increase to 0.5u considering there's still small ketones. You can mix a few drops of honey with the syringe feeding to keep sugars in the right range. Range 150-220mg/DL on human meter.

    Just saw your post @Critter Mom re: dose. Need to catch up a bit here but with that 189 earlier, I'd go with the 0.25u again too!
    ETA - oh I see Meya weighed in and was thinking 0.5U. Is someone going to be able to monitor tonight? I can't as I have to go out tomorrow and it entails a hefty drive for which I can't risk being tired unfortunately.

    Thanks for responding, Linda. Meya recommended 0.5 (ETA - with drop of honey in food to bump up the carbs a bit) but Dan & Heather didn't see the message in time.

    Just increase at next dose time. Adding at this point will make it harder to track numbers and see what the insulin dose is doing.

    How much of the FF Classics did Mittens eat before her current insulin dose?

    (FYI re your earlier question, Heather, on the board we often refer to Fancy Feast Classic varieties as Fancy Feast paté style food.)

    Also can you confirm the exact EST dose time, please?

    We gave her .25U insulin at 4:15pm, one hour before she ate 7.5ml of fancy feast and im about to feed her again in 30 minutes

    Is she eating anything on her own yet? Keep offering, eventually she will start you don't want to miss any opportunity.

    Not yet she isnt, i keep some of the Fancy Feast out by the water(raised up a few inches so she dont have to bend over as much to eat). She looks at it like she wants to but then just turns around and walks away.

    Thank you! Needed to check for dose onset. As Meya commented earlier, it can take a few cycles to see how effective a dose is going to be.

    I've been doing a fair bit of that myself today. :oops:

    Will keep an eye out for the +2.5 test. Also please can you keep posting the feeding info as you go (also in '+' notation if possible, please! :) ). Also a note of the feed containing the potassium supplement.

    Just checked her BG, it is 223

    4:15pm MDPS -220

    Im not exactly sure when she pooped last, it might have been a few days ago. The last time i know for sure was a few days ago. If im not there to see her go in, im not sure who left it her or my other cat. All ive seen her do latley is pee.

    To post her food would i just put "+10ml of Fancy Feast" alongside the BG results?

    Meya advised that the low potassium can slow the gut. If Mittens is a bit constipated that could contribute to the nausea.

    Keep a note of all bowel motions. Even with two kitties in the mix we may be able to deduce who pooped what. Also monitor the litter box for any signs of liquid/runny poop.

    Format example:

    +2.50 - 201 - 7.5ml Fancy Feast Classic at +3

    Ok i will start keeping track of their bowel movements, im guessing hers should be easy to distinguish since she hasnt really eaten any solid food for a while.

    Thanks for clearing up how to post the food, i will start posting that as well(im keeping track of it here on a notepad on the computer too). Got another 7.5ml warming in a cup of water now, will post after her next BG Test in just over a hour from now.

    Another thing re the stools, watch Mittens in the litter box to see if she strains to go - a very hunched position.

    Position of cat when urinating:

    When having a poop the position is more 'hunched', the bum is more in the air, and the hind legs tend to be more tucked under the body (if you get what I mean - can't find a pic).

    Watch out for any 'marbles' or very hard stools. ETA - also watch for liquid/runny poop.

    Let us know about 'developments'; they can be a very valuable diagnostic aid.

    I can be around for a couple hours at least-- I might be able to stay up until the next shot time in four hours (I think), but I won't last much past then :(.     Im still kicking lol, Im actually not working atm(1st time ive been happy for that) and Heather is at work right now for 6 more hours and off tomorrow so we'll be able to catch up on some sleep tomorrow. Well probably sleep in shifts so someone can keep an eye on Mittens, pretty much what weve been doing.

    Just tested her BG again +4 -226

    So far today:
    4:15pm MDPS -220
    5:45pm +1.5 -227
    6:45pm +2.5 -223
    8:15pm +4 -226 7.5ml Fancy Feast at +3

    I think i did the Food portion right? I'll start adding the food everytime, i just did the most recent this time. I got it all written down tho so if i need to i can.

    Next dose is due at 00:15 EST (05:15 UK time). Meya suggested earlier to increase the dose to 0.5IU Novolin N and up the carbs in the food. (A larger carb load can enable a support larger dose of insulin so as to reduce ketone levels. Here are her guidelines:

    We need eyes of experienced members confident in steering curves on Mittens because it's the first cycle on that dose. All is made trickier, of course, by the nausea issue plus we're still in the process of gathering data to help us learn how Mittens' BG levels respond to the Novolin N and to food.

    We need to keep an eye out for who's around. I'll be here for a little while yet (still trying to get stuff into the spreadsheet! :rolleyes: )

    One question: what's going on with the potassium dose-- was that included in the +3 feeding?

    If you can keep tabs on who's around that would be really helpful. Just on the off chance I fade unexpectedly, if I go quiet but you haven't seen a 'formal sign-off' on this thread or if I don't respond to tags then work on the principle that I've passed out and go whoever you can find. (If FH is quiet try going over to L&L to round up a posse.)

    If ketones weren't in the mix I'd very much agree with the above but we're a bit caught between Scylla and Charybdis at the moment: not enough insulin and the ketones aren't getting dealt with as well as possible; an active cycle without experienced eyes on Mittens and numbers could go low.

    This cycle seems to be quite flat at the moment. With so little data it's tough trying to make a call this early.

    Perhaps holding off on a dose increase is a call for Dan and Heather to make. Not only will there be more people around to help in the morning, but Dan and Heather may need a little bit more rest which they might be able to get if the dose remains at 0.25u for tonight. Things have been rather intense and no one will be of any help (especially Dan & Heather) if they can't get "some" rest.

    Just throwing this out there for consideration...

    Jill: would that suggestion apply if Mittens is still throwing "small" ketones at the next test (whenever she decides to cooperate :rolleyes:), or only if she's back down to trace or negative? She's doing great on the 0.25IU so far, keeping numbers in the low 200's, so I'm hopeful things will keep under control but I just don't know enough about how these things go to guess.

    I really appreciate your experienced input on this, Jill. I would be very, very much in support of keeping to the 0.25IU dose for the next cycle. (My only worry was what to do about the ketone levels.)

    I desperately need rest myself. I'm likely to be the only person around on the UK side in the morning.

    Just wanted to pop in, I can't add anything because I'm at work. Just wanted to send out some thank you's to everyone for all the help. I'm halfway thru my shift and counting down till I can get home and relieve Dan so he can sleep. I keep checking each break to try to keep myself updated. I'm going to try to learn about the spread sheet tonight so I can help enter stuff into it. I'll check back later, keep up the good work Dan.

    Unless ketones are higher than small, I would stick with 0.25u tonight along with getting as much food and water into Mittens as possible. If she were mine, my goal would be to syringe 20 mL of food and 10mL of water (more if possible) before sleeping. I would plan on increasing the dose to 0.5u in the morning.

    My reasoning: All of you have been going above and beyond and have done one hell of a job doing it, BUT none of you can keep up this pace. We're talking a break of a few hours... not days. There are a lot of us, but if Dan or Heather break... who will care for Mittens?

    Just my opinion.

    I would be ok on waiting to up the insulin dosage til tomorrow if thats what you think? I havent gave Mittens any insulin yet(Heather has been giving it)

    OK, still pretty flat, that's great!

    We seem to be nearing a kind of consensus, but let's not make the final decision until we get closer, to see if Mittens does something new or if someone else shows up with some advice/information about the situation.

    So, Dan, this will be the first time for you to give insulin by yourself tonight, correct? Will you be on your own for a while then or will Heather get back relatively soon afterwards?

    Post when you get the pre-shot BG! And try to convince Mittens that now would be an excellent time to head to the litterbox....;)

    Yes this will be my first insulin injection for Mittens(Heather has done the rest). Im also a little concerned because it took a couple lancets last time to get her BG and now i got one left in the original lancet set we got, but i think its used already. Am i ok to re-use it? I know its not the right way, the soonest i can have new ones is about 3 1/2 hours.

    She just used the litterbox but she didnt look like she was even settled in there yet when she was already done and on her way out, so i missed the chance to get another ketone test. I apologize, i think my lack of sleep is starting to catch up to me, tomorrow i will be refreshed i promise. I will keep an eye on her and the litterbox still.

    Re-using a lancet isn't ideal (they get dull), but needs must. I think in a pinch you could even use a syringe needle-- you'll probably have to later because I think we'll want at least one test before +3 to see where the cycle is going. Mittens has been behaving beautifully on insulin so far, but we don't want her surprising us tonight.

    The plastic wrap in the litter box is a good trick-- just a little bit has to splash on it for you to dip the stick in, so that way you don't have to actually catch her in the act (much easier).

    Probably for a couple hours, but I think considering how tired Dan is (and that neither of them have gotten any meaningful sleep for the past several days) it's going to be safest and sanest to stick with the .25 tonight

    Mittens seems to be getting a lot more calories and water in and her numbers aren't horribly high so for the sake of everyone's sanity, I think that's the best thing to do for tonight

    OK, if other folks are going to be around for a bit I think I'm going to sign off now because I'm kind of losing it. I will check back in the morning to see how things are going-- crossing fingers and paws that everyone will be able to get some rest tonight!

    Dan: How much did Mittens eat this time? Assuming she's still getting a decent amount of food, I think the general opinion is yes, 0.25U. Are you OK measuring that dose? It's really tiny-- there was a picture in the previous thread, I'll try to repost it here:

    She just ate another 10ml of Fancy feast about 40 minutes ago. Ok thank you for the picture, yes i know where the .25 is and yes it is real tiny but i think i can get it.

    OK.....looks like it may be another cycle like the other ones (which in a way is good because maybe you and Heather will be able to get some sleep tonight!)

    I think you could easily take a break and get another test at +3 and if she's still doing OK then, think about getting some rest tonight

    Looks like she's eating better too and that's good news

    I tested her at 3:30 and it was 273 then I fed her 7.5mL of fancy feast. Dan went to bed literally as soon as I got in the door, I haven't had a chance to really look yet, I'm guessing this is the +2.5 reading your looking for. I'm sorry I didn't write it right I'm just trying to get caught back up, I couldn't follow as closely at work as I would've liked, but wanted to get you this number right away.

    Thanks Heather. No worries about how you wrote that Heather. I've got the details. +2.5 reading @ 03:30am EST is 273
    So she has only dropped 10 points since the last test and the previous test was actually elevated from PMPS slightly. It's looking like the cycle is going to be pretty inactive but she is holding. You usually test again at +4 but I am going to suggest you push the next tests out to +5 and +7 tonight. It will give us info for another time in the cycle without leaving too long between tests. Keep up the feeding schedule and get fluids into her as you can.

    I am heading back off to bed and hope you can take a little nap at some point too. If there is any drop of 100 points or more at either the +5 put the 911 on your title immediately to get someone's attention to follow along with you just in case. Mittens will still be safe at that point but will need more frequent checking.
    Sorry it took so long to send this. I still can't see straight yet :woot: and I'm not the fastest typist either. Will check in again when I get up hopefully around 6:30 or 7am your time.

    I did not see your message until right now, so I did test at +4 (about 45 minutes ago). Sorry I was eating and didn't check the board before I did it. So it's +4 -280 and I fed her 7mL of fancy feast immediately after I tested her. I will test again in a bit and post the results.

    Good morning. You can put the 280 in the +4 column of your spreadsheet, and add the food you gave to the Remarks section.
    I think getting a +6 and then the +7.5 before insulin will give a good feel for where she might be headed, for the possible increase this morning.
    Sorry, I won't be here to see how it goes. I have to work all day. Hope you have a good day, and Mittens eats more food and has less ketones.

    Ok, I will try to add those things to the spreadsheet. I just tested her a little bit ago +6 -296. Also, 7.5mL fancy feast mixed with potassium tablet right after testing. I'll try to add all of this to the spreadsheet.

    I do feel like we got more into her yesterday and the night before than we had previously.

    She needs her next dose of insulin, should I give her 0.25 again? What times should I do testing this time?

    I think you need to follow Jill's and Meya's recommendation to increase the dose to 0.50IU Novolin N. BG has drifted up.

    Have you done preshot BG and fed yet? Insulin is due.

    If not, please do BG test and feed straight away.

    Post result for BG first and post result immediately so we can check it's safe to give the increased dose.
