? Dosing advice w/AMPS BG at 150 -time sensitive

Yesterday I did a curve. Jester's AMPS was about 190 and I gave him 1u.

After his +4 he was already down to 83.

This morning he's at 150.

I'm thinking either no insulin at all or a "token" shot of .5u.

Thoughts?

How much can you monitor today? Could you get at least a couple of mid cycle tests to see how he's doing? I'd hate to see him lose momentum with a skip, but you want to be able to keep an eye on him when you start shooting lower pre-shots.

Hope a ProZinc person can weigh in on this too.

 

He's an hour late now.

I probably would have given the full dose if you are home to monitor. 153 is not much different than 195. You can give Prozinc up to an hour late, but if you do, adjust your next few shot times to gradually get it back on schedule.

Updating to add... At least with Mia, I find that it is best to keep consistent dosing to the extent possible, so I try not to change up dosing based on each pre-shot. (I'm not saying I do not reduce when it is dangerously low, but I try not to 'get scared' just because it is a little lower than normal and change my dose.) Also, keep in mind that Jester got to 85 which is not too low at all, especially with a human meter. And, as I mentioned before, 153 is not much different than 195.

A lot of this is up to you and your comfort level. I am finding that I have to take on some risk to get the results I want, BUT I would not be doing that if I was not going to be able to monitor. So, take how much you can monitor (and what carbs you have on hand that you can manage to get down him) into consideration when deciding what to do at times like this.

 

The last time I gave him a full unit at 141, he took a nosedive down to 53 by mid-cycle.

 

I am just telling you what I would do. I see value in consistent dosing when possible. I think changing the dose every time and skipping when not absolutely necessary slows progress. You are using a human meter, so 53 is still not in the hypo zone. It is up to you though. We each have to decide for ourselves what to do for our babies since we are the ones who have to deal with the outcome. All anyone here can do is share thoughts and experience.

You do have some flexibility here. I don't think there is a wrong answer here, other than shooting 1.0U and walking out the door for 12 hours. If you can monitor today, it might be a good opportunity to gather data on what happens when you shoot lower numbers, even if it's a half-dose. Eventually, you'll be doing more of this to try to get him to spend more time in the good (dark green) healing numbers.

That doesn't mean he'll do that today-- and 53 is still safe, even if it's close to the line.

Ultimately, "you hold the syringe"-- do what you are comfortable doing, always. If that means a skip today, do that, but if you do decide to shoot, we're here!

 

Decided to go for middle of the road and gave him .5u.
Also, with him being so fussy about food again, I didn't want a full unit on such an empty stomach.
I can usually get Jester to eat about half a can in about an hour. But by 10:00, he'd only eaten less than 1/3 a can.

Now, of course, after his half unit, he's decided he's hungry.

And I haven't been able to sit down yet this morning.....

Oh, and it snowed last night (still is, in fact).
