? Sprocket bloodwork results- advise pls

    Here are Sprockets test results from this weeks vet appt.
    Test results attached.

    His teeth have tarter already, he had dental 9/16. She said 2.5 out of 4 stage.
    Glucose 14day on meter was 246. And his Alt is elevated. Wondering if amitryptiline could cause it? Reducing amitryp to half dose now. Still on ursodiol for liver. Wants to add denamarin but he wont take-too big. Company told her ok to crush if need be, I am concerned on that. Looking for alternatives.
    Ok thoughts on all this?

    Someone else mentioned high leukocytes yet WBC is normal.
    Any clues on this?

    Previous ALT on 7/31/17 was 32
    ALT 2/20/18 is 204

    Any chance you could enter the data on the Labs tab of his SS. Makes it much easier to read and do comparisons with previous b/w.

    I had the same thought as Wendy. Remember to put the date of the labs at the top of the column.

    I dont have all his labs in the SS.
    So nothing to compare to.

    I put one test in awhile ago but i never did any others because the ranges were different. Had had alot of bloodwork last year.
    I have it all somewhere. It would take me awhile to put all that in his ss.
    Plus all other tests were when he was sick. This was his first check up when he wasnt sick.

    You don't need to put all of the old bloodwork results in. Maybe this one and the previous one. I see there is some data already, but the date is on the bottom which is harder to see. Neko's IM vet used a different lab, so I just added a new column of reference values. Or you can change the one column of reference vales to the current one and put a note on the old Labs that some reference values are different.

    Each lab has different value ranges.
    This is hard to match up because its all abbreviations. I dont know them all. Now i remember why i gave up inputting the lab info. Its because i dont know any of abbreviations. Someone whos knows them needs to put a column with abbreviations. Or give a copy on a sticky.
    This is becoming complicated.
    I am not a computer person

    I just put in what I could but your better off just looking at the pics of the results because its not the same as the lab list in the SS.

    You can take a look at Gizmo's lab tab for how I have done it. It does get complicated especially with different ranges.

    FWIW my Max often had an ALT slightly over 200 and my vet was not at all concerned. It seemed to be elevated whenever he had a pancreatitis episode. Does Soricket have pancreatitis or is there another reason he’s on ursodiol? I can’t make out the labs. Were they done in house orcsent out ? That can make a difference too as I think outside labs are more precise and machines calibrated more often.

    How is Sprocket acting?

    Sprocket has not had pancreatitis but had elevated liver levels at dx in 3/2015 and last year 3/2017 when he had cholangiohepititis. (Misspelled)
    Since 2nd time they left him on ursodiol and said he may need it for life.

    Results were in house.
    He is acting ok. He has food issues as in picky like crazy.
    I posted about that a few times recently.
    When his bgs are in lower 200s he doesnt want to eat at normal times.
    Or he doesnt want that flavor so give another and no.
    Repeat. Drives me nuts.
    Otherwise not bad acting.
    He is also on amitriptyline.
    I was thinking that might be raising his number.
    Ever hear of that?

    I tried to fill in ss/labs but dont know abbreviations to match it all.
    I will try type up some tomorrow if I can.

    Question...saw you give digestive enzymes. I am starting it when i figure out dosage but I just go DrMercolas Digestive enzymes and smelled it....to me is smells like cat urine....is that normal??
    Will cats eat it with that smell? Do they like the smell?

    I too found the pictures hard to read, but from what I could see, most looks in range. Did your vet say anything about the ALT?

    Wanted to add denamarin but he wont take it. Pill only...to big. Tried pill pockets...he spits out pill. Cant pill him at all.
    Looking for alternative. Found milk thistle liquid drops so far.
    Think it could be elevated from taking amitriptyline.

    Amitriptyline is metabolized via the liver. Given that Sprocket has as history of liver issues, I would discuss this with your vet. You might want to look at this link from Mar Vista Vet. In addition to noting that liver enzymes should be carefully monitored in cats with liver issues, the site also notes it can have an effect on BG. There are other links that state that the drug should be used with caution in animals with diabetes, abnormal thyroid function, liver disease, kidney disease, eye problems and abnormal heart rhythm.

    One point on the BG levels -- this drug is used to relieve pain from diabetic neuropathy in humans.Also of note, that with humans, all tricycle antidepressants, which is the class of medication amitriptyline falls in, require getting an EKG. The drug can cause or exacerbate a cardiac arrhythmia.

    Also, do NOT just stop the drug. If Sprocket has been on the medication for a while, you will need to taper him off.

    I had a feeling that was the issue. We did cut his dose in half a few days ago. So he will be in half dose awhile then half that and off. I will have to ask about how long for each level. I think he was on the half dose 2.5mg for 3-4weeks when he started the medicine. He started it because of the idiopathic cystitis. So he is on for anxiety.
    So any suggestions for anxiety that is better for him?

    I also posted the pic in fb group too and you can see them clearer there for some reason.
    Could you look there for now please as i dont have time right now to type all those pages in SS with ranges and all.??

    I dont know any other way to upload the pics. I dont know why they arent clear here but clear on fd groups. Dont have time to type all that up. Not fast at typing.
    It took me awhile to figure out how to upload pics on here already.

    I will keep that info for DE but i got alot already to use here so have to use that stuff first.

    Is it really important that we see the b/w results? You've told us his ALT is high. Sienne gave a great response you should discuss with your vet.

    The urinalysis is in question too.
    This is his first bloodwork where he hasnt been sick. Thought it would be better.

    I did a post back from the picture on FB of his urinalysis. Leukocytes are not reliable on the multistick with kitties. The trace of blood could be from the cystocentesis is the urine was drawn with a needle.

    I just copied your urine pic from FB and posted it over here. If there is a trace of blood and they did cystocentesis then the visual would tell if there is any possible infection, which is negative in their comments. Other than a trace of protein, which is common in FD kitties the rest of the urinalysis is good.

    Other than the ALT and glucose being high they have stated that everything else looked good. The link below explains some reasons for higher ALT, but as said before if you have any concerns you can discuss them with your vet.

    the plant digestive enzymes that most use do not have a smell so you have to mention that dr.mercolas' digestive enzyme is animal based to get proper feedback ;)
    i looked it up after you mention it smelled like urine the other day just in case the three were from a bad batch and if you search for product reviews, on different sites, it shows others mentioning the strong smell too, some even smell yucky pee too like you do..think it is just the nature of animal digestive enzymes but, not 100% sure since the was the first brand i ever bought.

    What is the ALT? If other liver enzymes are fine my vet says a high ALT alone unless well over two times the high range is not a concern. I’d retest in a month if my cat. What does your vet say? I believe one of the meds you use is metabolized through the liver. Could be the cause. What does your vet say about that ?

    Wow. Thanks. I didnt know it was animal based vs plant based. As long as its normal. Will try it and hope he eats it.

    Ya. I spoke to her last night. Says the urinary levels was not accurate as said before.
    And the ALT with the amitryptiline causing the rise could be. Not alot of evidence of it with studies with pets but possible. Have reduced his dose and am starting to take him off the medicine.
    She is gonna help me choose a milk thistle & samE product since he cant get the denamarin down.
    Then we will retest in a month or so.

    Thanks everyone. We still struggle with food but happy if he feels better.
