Noah / approaching OTJ or something else?

A quick history first.
Noah is a neutered male about 14 years old. He has cardiomyopathy and an enlarged heart that has broken through his cardiac sac. This has been confirmed by X-ray and two ultrasounds, one performed by a cardiac specialist. He also needs all his teeth pulled but only has a 10% chance of surviving the surgery so we control his pain with transdermal BUPE. He is also more than just nervous. He's never been crated, afraid of windows and any trip to the vet is emergency only. He is on U40 Caninsulin and is/was a high-dose bouncer.
For the past month his numbers and dose have been going down about 5% / day to the point where I've reduced his dose to as little as 1 unit both AM and PM. I have not checked his weight but he is as chunky as ever and although he is a secret eater his appetite appears normal. The only drawback is that I have to pull him from his basket and show him his dish. His vision is fine, he just doesn't like eating when the other 5 cats are in the room. I also stimulate his appetite with Temptation treats, something I know is not good for his blood sugar but it gets him to eat every time. He never vomits and his stool is always normal.
So the question is this. I know cats don't just suddenly go OTJ but is this typical of what happens? A slow and steady reduction in numbers? For us OTJ is something that happens to other cats and not here. It's almost like watching a miracle unfold before our eyes. Is it also more unlikely to happen given his age?
He has gone as high as 6 units but that's usually when I know he's bouncing.

 

Baby would have a 300 bg then drop to 70 in the same day. The next day up & down (down meaning normal bg) .. this went on for about 5 days with more downs progressively, then eventually all normal bg. I’m sure each cat is different! I just remember panicking & thinking I’ve given less insulin but she’s really dropping & it scared me. That was our experience. I hope he’s going on an insulin break for you! I’m sure it would be better for him.

 

Wouldn't that be nice that Noah decided he was done with all this poking business!

No answer for the OTJ question as I have never been and probably never will be there.

 

I think it can start that way. If he continues this way and you end up giving ever smaller fractions of 1 u he might be heading there. That would be fantastic!

 

Life would be so much easier if Noah could have his scheduled blood work, weigh-ins etc. His mental state being out of the house has to be seen to be believed. It's not an exaggeration to say he may have a cardiac event or a stroke being in the car. He really is a little miracle cat, way past his best-before date. We 're both quite happy to see him live a life of tranquility in his little basket. Thank you Kris and Tracey.

 

Diamond doesn’t have heart problems but I know exactly what you go though with Noah’s fear. Diamond is the same way, even in the house. He has gotten a bit better in the house because he has neuropathy and can’t get around well at all. I have no experience with OTJ, and never will with my acro cat, so I have nothing to add except, Fingers and paws crossed that Noah is heading towards remission!

 

While my buddy Lou didn't make it all the way to OTJ, he was soo close. He ended up failing on the last day, but I can at least speak to how his numbers decreased. It was a lot like what you're describing. We had just got him regulated, and then he just started throwing up blue preshots. We had to dance with his dose amounts as his preshot BGs kept going down. One day he just had a normal preshot BG and it stayed normal from there (until it wasn't.... but that's a depressing story we don't need to talk about on this post full of hope ). I know Lou's dance down to his OTJ trial was only 2-3 weeks, but he was very newly diagnosed, so I don't know if that affected the timeframe. I very sincerely hope Noah's pancreas is starting to wake up and wanting to get back to work. We'll keep our fingers and paws crossed for y'all!!!

 

Ohh that sounds like a dream, how wonderful it would be if he was going OTJ. There's hope for everyone!! I understand the spastic, gonna have a heart attack trips to the vet, me and kitty both. Everything is crossed for ya.
