Need opinion - Sophie is 6 weeks out from surgery - do I try to go the R route and get her BG down

    So, in addition to being Acro, Sophie also is IAA, which is probably why I can't get her regulated for SHT. She is 6 weeks out from her surgery (scheduled for 9/25, preop on 9/24. Is it worth it for me to take the time to gather up all the stuff and attempt the R route to break her resistance. Dr. McCue says not to bother, but my concern is that the insulin resistance will still be there even if her pituitary gland and tumor aren't.

    What do you guys think?

    Neko had both acromegaly and IAA. I didn't use R until after her antibodies broke just under 9 units. Basically at the time there wasn't anyone on board here familiar with using it in a high dose cat and willing/able to teach. So yes, you can break the resistance without R. However, I have also seen IAA/acro cats need to get north of 50 units of insulin to break through, even more if you consider the R dose on top. ECID - and high dose condition cats are even more different. I have also seen acromegaly only cats need north of 30 units to break the acro resistance. It may be the tumour, not the IAA that is your enemy at this point.

    We do recommend that people get an R teaching buddy to "sit" with them the first few times they try R. There are a few key things to learn about when, and more importantly, when not to dose R with the basal insulin.

    Remind me again, are you using Lantus? It's helpful to put a few things in your signature, such as the link to your spreadsheet, date of diagnosis, insulin and dosing method being used, type of diet, any other medical conditions. Some people see an antibody response if they switch to Levemir. Though with 6 weeks left, I wouldn't bother with the change at this point.

    One more thing, Neko's IAA basically broke around SRT time. I'm guessing the radiation therapy on her tumour started it. If Sophie doesn't need exogenous insulin after surgery, she won't have IAA.

    I probably wouldn't worry about it at this point. You have enough to worry about - travel, packing, etc. It is only 6 weeks away.

    I also wouldn’t bother. If anything just continue to titrate your long acting insulin, but most importantly just enjoy the time with your cat and the rest of the summer! :bighug:

    This is really exciting for you and Sophie. And now we have both you and Amanda going to NYC within a month of each other. I really hope it goes well for both of your kittehs.

    TY Jeff. It is rather overwhelming, but, since I am from the area (Long Island), probably not as much as it will be for Amanda. And I only have a 6 hour drive. My major concern is Sophie's anxiety while in the car, so I am going to call our vet this week and discuss the upcoming surgery, and get a prescription of something to help her chill out. I have enough time that I can see how she reacts and take some 2 hr drives with her to accustom her to a longer trip than the usual 25 minutes to the vet.
