Anyone know if we should be giving this with food or on an empty stomach? He takes it just fine in a portion of pill pocket so we can do it whenever.
We don't want to overwhelm him all at once so right now he gets:

Breakfast - 1/8 tsp miralax and proviable probiotic, then insulin
Dinner - cerenia in pill pocket, then 1/4 tsp miralax and 0.75ml omega3 fish oil, then insulin

our thinking for this is: 1. if he's hungry he'll eat it - and he always seems pretty hungry at dinnertime (7pm) and 2. his weird vomits (not hairballs)
were always in the very early am, and cerenia works 24 hours but we'd like it working strongest over night - not ending its effectiveness around the time he gets sick (6am).

Should we do it earlier, like an hour before or 2-3 hours AFTER dinner?

The vet told me to give cerenia 4 hours before dinner time, so I guess it's on an empty stomach?
Also I think that mostly you want them to be hungry for dinner.

Your vet most likely asked you to do that because it takes about 2 hours for cerenia to work.

I did some googling and Zoetis recommended to take cerenia with a light meal and if taken with an empty stomach, can cause more vomiting (sources here and here). It makes sense to me since you don't want to feed too much if your cat is vomiting but you also don't want to cause it to irritate the stomach. But in many cases, when cerenia is used in hospital, the pet isn't eating already and/or is vomiting. So taking it with or without food is fine.

I think in your case, it doesn't really matter as 6 am is well within the 24 hours and not at the tail end of it. You could definitely experiment with giving cerenia later before bed.

I'm curious though. Is Tigger vomiting even with cerenia on board? And what is he vomiting? I know some cats and other pets vomit when their stomach is empty and 6 am is awfully close to breakfast time!

I mostly need him hungry for breakfast as we can not play games in the am when I'm getting ready to leave for work. I need to be sure he eats so I can be confident leaving him home alone the next 10 hrs or so. At dinner we have wiggle time and can test when needed!

he has vomited a few times but not nearly as much as before! I would say maybe 75% less vomits now . . . but it's only been 3 weeks since we started the daily cerenia.
i think he has thrown up 3 times in that period? MAYBE one watery am - but i cant be sure. Definitely one yellow bile looking puddle i remembered to document and i think a food one. the food ones always go the same. he does it right after eating then is hungry again right away. those have been happening for years and its thought to be apart of his chronic constipation too.
