More help needed for Mum cat -foods for diabetes AND kidney failure...

    As some you who followed the drama unfolding these past few days will know that Mum was hospitalised for ketones hight temperature etc. Back home for a few days seemingly fine.. This morning vomited and refused food... off she went again.. back on fluids.. apparently no ketones. Vet thinks she is in stage 3-4 of chronic kidney failure.

    My dilemma is all being well - if Mum makes it - I feed her raw diet of 3 types of mince (beef,pork and turkey) all low fat and mixed with fish oil and Felini complete supplements. She also has the lowest carbohydrate handful of dry food... all I believe opposite to kidney diet food?

    Has anyone been in a similar position and how did they resolve this apparent conflict?
    She is not a fan of canned cat food of any kind. Many thanks.

    The important part is getting Mum to eat. Whatever she will eat. To prevent ketones from forming.

    Since you need a lower carb food for the diabetes, and need a food for the late stage CKD, you may want to talk to your vet about phosphorus binders to add to your cats raw food diet. The raw is what she will eat, so keep feeding the raw.

    No idea if the phosphorus binders. taste bad, pills or liquid or powder. But think mixing them with the food is the best way to get them into your cat.

    "Controlling Phosphorus Levels with Phosphorus Binders

    If your cat will not eat a therapeutic kidney diet, or if his/her phosphorus levels are still too high (over6 mg/dl (US) or 1.9 mmol/L (international)), despite feeding such a diet for a month, products called phosphorus binders are used. Even if your cat is eating a low phosphorus diet, at some point it is quite possible that this will become insufficient to control your cat's phosphorus levels. The role of phosphorus in feline chronic renal disease(2010) Chew D & Kidder A CVC in San Diego Proceedings states "Diet alone is not successful in adequate phosphorus control as chronic renal disease becomes more advanced. In these instances serum phosphorus concentration increases above the normal range or stays in the upper half of the normal range." In this situation, you will probably have to consider the addition of phosphorus binders.

    Phosphorus binders work by binding with some of the phosphorus in the cat's food in the intestine, which is then passed out in the stool. This prevents the phosphorus in the food from being absorbed into the bloodstream, and therefore over time this reduces the levels of phosphorus in the cat's body.

    To be effective, phosphorus binders must be given shortly before or with food. You should start to see a difference in your cat's blood phosphorus levels around 7-10 days after starting binders. There are instructions on how to give binders below.

    Using binders is not as effective as feeding a therapeutic kidney diet, because binders cannot bind all the phosphorus in the food, so your cat will still be absorbing some of the phosphorus in the food. Therefore, if you do have to feed a non-therapeutic kidney food (because the most important thing is that your cat eats), you should still aim to feed the lowest phosphorus food that your cat will eat."

    How is Mum doing?
    Is this the first time the vet has mentioned kidney disease?
    It is possible that the kidney values may improve with IV fluids and time following the recent illness.
    Are you able to post any recent and the last few months path results to compare?
    If you don’t have them, ask the vet to email the results. They are yours. You paid for them.
    Any food at this point is more important than CRD food. It is very important following ketones that Mum eats plenty of food.
    Are you testing daily at home for ketones? It is vital you do test daily for ketones following the appearance of them. You can do this by collecting a urine sample and dipping a Ketostix strip into the urine and reading the result against the colours on the side of the bottle exactly 15 seconds later. Or you can use a blood ketone meter which tests like a BG test. You can buy Ketostix from a pharmacy.

    Thank you all so much. Early stage kidney failure was indicated but seems to have escalated now to possibly stage 3-4... I’ll of course do my very best for my beloved girl. Collecting her later on so will know more...

    Glad to know she's coming home.

    There have been some of us in the same dilema that you have, it is manageable and they can be quite ok as mentioned it would be good to have a copy of her blood work and tests that got made to really have an idea of what you will be dealing with and some previous ones if you have them

    Was the escalation sudden, just after the ketone crisis or is it something that has been going on for a while? is important to know because sometimes after a crisis or infection kidney values raise but they may lower after the crisis pases as opposed when is a chronic situation

    Handling diabetes and CKD is more about the diet and supplements, you mentioned you are giving raw , do you prepare the food or you buy it?

    Before jumping into phosphorus binders did the vet told you what her phosphorus levels are? What is her creatinine level?

    • @Zee, I use a Phosphorus Binder by RX Vitamins...https://www.amazon.com/Rx-Vitamins-Phos-Bind-Powder-200g/dp/B00PT4ORD8. It is a very fine powder like confectionery sugar and can easily be mixed into her raw food. Did the Vet mention anything about supporting her at home with sub-q fluids for her CKD? My Renal foster kitties were always on sub-q fluids, Phos Binder and Potassium supplement and it helped tremendously.

    Not sure how long he escalation has gone on... I prefer Mum’s food - 3 types of low fat mince - beef,turkey,pork with Fellini complete supplement and fish oil. Mixed with water. She also has a handful of low carbohydrate dry food (thrive). Will upload Mum’s latest bloods...

    I understand that Phosphorus Binder by RX Vitamins is Aluminium Hydroxide and yes that is one of the phosphours binders you can use, but it would be good to know some of her values before starting her on phosphorus binders and potassium can be really good if they need it but very dangerous to give if not so you need to know what her potassium level is before giving a supplement with potassium

    I'm trying to take a look at the blood work but even enlarging them I'm having a hard time so I want to make sure I'm seein correct she's got:

    Creatinine 362 mumol/L
    Phosphorus 1.4 mmol/L
    Calcium 2.44 mmol/L
    Potassium (K) 4.3 mmol/L
    BUN 14

    Veronica you seem to know your stuff! I believe her phosphorus levels are on her blood work? I’m not sure what these figures mean? Would you mind taking a look?

    From her blood tests, they have marked both Bun, which is protein tied, and Creatinine the Kidneys as very high. That's the only 2. So she needs a low-protein diet higher in fat energy instead. Lower potein, higher fat.

    When it comes to fat for kitties one don't have to worry, it's not like with humans.

    Yes, that's the white blood cell Lymphocytes, she is just a tad bit low on them. Lycopenia it's called when they are low to non-existent. She doesn't have an infection though.

    Well Mum was considerably overweight which may have contributed to her diabetes? I think a through consultation is required here - although every vet gives you different advise..

    These bloods were done after night and a day at hospital on fluids.. when she went in she had high temperature and was given antibiotics...

    It would be good to have some previous blood results before jumping into conclusions because she has very recently had a ketone problem and that can cause her BUN and creatinine to raise, specially because her potassium and phosphorus levels are normal so her kidney problem may not be so bad

    BUN can be high also because she's on a high protein diet.

    That being said her numbers are not so bad even for a CKD (chronic kidney desease) she would be classified as stage 3 but she doesn't need a phosphorus binder yet and certainly no potassium supplements but she will benefit from some subcutaneous fluids to help her lower her creatinine.

    To help her overall you need to give a low carb diet because keeping her diabetes under control is really important uncontrolled diabetes and ketones will harm her kidneys more than what any CKD diet could help but you need her diet to also be low phosphorus ( that doesn't mean low protein) because even though she doesn't have a phosphorus problem just yet ( the value is a bit on the high side ) keeping this value low will help her kidneys

    You need to ask the manufacturer of the food you are giving the phosphorus content it has to see if you need to do any changes

    Also it will benefit her to get some B complex vitamin supplement and Omegas

    Thank you for taking the time! As I said earlier on I MAKE HER FOOD - she eats a mix of 3 raw mince - beef at 5% pork at 5% and turkey breast at %2. These are mixed with Fellini complete supplement and fish oil and water. She also has a handful, of low carb dry food (thrive)

    Sometimes, items like that can be easy to miss. Sorry for that slip up on our part, asking about the canned food.
    You don't feed canned food, so checking the canned food for phosphorus content is not possible.

    Veronica lives in Mexico and makes her own cat food also. She helps out a chronic kidney disease cat group in Mexico. (in Spanish I believe)

    That is how she has so much knowledge about kidney disease to help you out here. Plus, she has a diabetic cat.

    We are doing the best we can to help. We make mistakes sometimes too.

    If you have decided to provide your cat with a BARF diet, it is not always easy to ensure that all their nutritional needs are met. Thanks to Felini Renal, it is now very simple: in order to provide your cat with all essential vitamins, minerals and trace elements, simple add one level measuring spoon of powder to 80g fresh meat. The measuring spoon is included and can be found inside the tub, making it even easier!

    Felini Renal is hypoallergenic and free from fillers. It is also ideal for cats with allergies and intolerances. The powder comes in a tub that can be opened and closed quickly and safely, keeping the supplement dry and avoiding clumping.

    Use and dosage:

    An adult cat (of around 4.5kg) needs around 130g of meat per day, divided into 2-3 meals. For one meal mix 80g of meat with one level Felini measuring spoon (0.8g) of Felini Renal and 2 table spoons of water.

    When preparing larger quantities, add 10g of Felini Renal to 1kg of meat. To make a kidney diet, use only muscle meat; Heart can make up to 20% of the meal. We recommend to abstaining from skin, innards and connective tissue-containing meat parts, as these form biogenic amines or ammonia, which can have an unfavorable effect on the course of the disease.

    Decreasing the protein content relieves the kidneys and results in less urea, which reduces the symptom of nausea. At the same time cats require a high protein content in the feed, as a shortage leads to muscle loss. In order to reduce the relative protein and phosphorus content of the feed while maintaining the caloric intake, the fat content can be increased up to 20%; e.g. by the addition of lard or salmon oil, which has an anti-inflammatory effect due to its high proportion of unsaturated fatty acids. Furthermore, fat is a good flavor carrier that increases the acceptance of the kidney diet. If necessary, 5% fiber can be added (carrot, rice).

    Felini Renal contains a higher amount of water-soluble B vitamins to counteract their loss through increased urine output. In the advanced stage of the disease, the diet can be supplemented with a prescription phosphate binder. A kidney diet must be fed for life and should be implemented immediately with the diagnosis of CKD (feline chronic kidney disease).

    Please do not feed raw pork, nor raw wild boar (danger of Aujeszky virus) cooked pork/boar can be fed.

    Fish is a nice change for cats in the diet. However, cats are primarily carnivores so this treat should not end up in the bowl more than once a week. Please make sure that if you are feeding raw fish, it is a salmonid. This is e.g. genuine salmon or trout. Whitefish are not suitable for raw feeding because they contain thiaminase, an enzyme that destroys vitamin B1 in the diet and can lead to deficiency symptoms. Cooking destroys thiaminase.

    Notes: Felini Renal is solely designed for the preparation of meals from raw or cooked meat and may not be added as a "supplement" to already "complete foods" (like "complete cat food" cans or pouches)

    When feeding cooked meat you should also feed the cooking broth/juices. To avoid loss of vitamins do not cook with Felini Renal, nor add it to hot food. Only add Felini Renal after the meat and broth have cooled down. You can freeze meat with added Felini Renal. But we recommend to keep frozen storage time to as short as possible to avoid loss of vitamins.

    Please store in a cool, dry and dark place."

    Excellent! I didn’t know - originally she didn’t have the kidney issue... as you know Vets are very suspicious of non commercial foods... see how she goes.. she is refusing food at the moment...

    Ok for what I see they have a Renal version that has no phosphorus so you may want to change to that version

    Eventually you will need to change the proteins you are giving to lower the phosphorus content, not now right now, because now the most important thing is for her to eat, because pork, beef and chicken breast are all on the high end of phosphorus content, right now probably you could start by changing the chicken breast for chicken thighs those have a lot less phosphorus and eventually also substitute the pork and the beef at least partially, you could substitute a part of those with egg whites which is a high quality protein which means is high in protein with almost no phosphorus but it has to be cooked

    Yes I cook my own food and I had to learn about kidney disease the rough way actually since I had 3 CKD cats (two of which had PKD and PLD so the kidney issues started pretty much when they were borned) and there is not much knowledge down here about it so I couldn't actually relay on vets, I currently have 2 because one passed away last year of totally non CKD related issues and helped one of my best friends with his two CKD cats ( not surprisingly they were my cats parents), I do help some people here in Mexico but we are not so much a formal group yet

    And my diabetic cat is one of my the CKD cats so I get the problem about dealing with both issues

    She needs to eat specially with ketones history so right now anything she would accept is a good option, you may need to consider assist feeding if she is not eating on her own

    @Veronica & Babu-chiri, Phosphorus Binder and Potassium supplementation was all based on Lab work for my CKD fosters. Giving sub-q fluids at home was standard.

    I'm new to looking at labs but isn't the phosphorus number in the average range when looking at the IDEXX range on the sheet?

    I agee with all that @Veronica & Babu-chiri is saying... the kidney values could be worse at the moment following the recent illness, but may improve once she gets better. Seeing some previous results would be very helpful. We have seen this happen before here on the forum.
    I would also not rush into a phosphorus binder until you know if the kidneys have actually got worse, or are temporarily worse following the illness. Her phosphorus levels are within normal limits at the moment and should not need the binder.
    I would ask for a repeat blood tests in a few weeks to see if they have improved.
    As Veronica says, SubQ fluids may help bring the kidney levels down.

    With the recent ketones it is extremely important that Mum eats plenty, so at this point give her whatever she will eat. Food helps stop ketones from forming. Also really important that the insulin is given and plenty of fluids.
    Are you testing the urine for ketones daily? It is really important you do this with a recent ketones illness.

    Thank you again. Mum refusing food again... her reading was horrendous at 9 London time 32.4! I gave her her insulin regardless of food hoping it’ll bring her levels down and she’ll want some food.. what the hell is going on? Offered 3 different foods... ps. Now eating a few biscuits.. all incredibly puzzling.. totally refusing her normal food..

    Sorry to hear Mum isn't eating, Zee, it's so important... have you tried the usual tricks, sprinkling some Parmesan on top of her food, or a little tuna (in water) - something strong-smelling to tempt her to eat? If it comes to it you can also ask the vet for an appetite stimulant.

    Did the vet send you home with antinausea medications such as cerenia and ondansetron? If not I would call first thing tomorrow and ask for both. They work on different pathways and can be given together. If Mum is nauseated she won’t eat and eating is very very important following ketones.

    Have you thought about a blood ketone meter?. They are more expensive than Ketostix but you test for the ketones the same way you test for BG.

    If Mum is nauseated she won’t want to eat. Im glad you were able to get a little food and fluid into Mum. Keep trying and ask for the antinausea medications

    @Zee, I had the same problem trying to catch her pee plus this is a multi-kitty houshold which makes it even harder. I ended up buying a Blood Ketone Kit...https://smile.amazon.com/gp/product/B07MPCX4DZ/ref=ppx_yo_dt_b_asin_title_o06_s00?ie=UTF8&psc=1. As @Chris & China (GA) explained to me, Ketones show up in the blood first before they spill over into the urine. The test strips are more expensive then the urine KetoStix. For this particular meter you get 30 Test strips and 30 Lancet for $27.99. I didn't care about the Lancets but they only sell the strips as a kit. To me it was an investment well worth it. No more pee-hunting. Poke and test.

    Ahhh yes... I know you'll do everything you can for her Zee to keep her comfortable... all paws very tightly crossed here that she will rally and come through this...

    On a lighter note she’s relishing being allowed forbidden dry food... the only thing she’ll eat..
    As far as I’m concerned she can have anything she wants as long as she eats something...

    Diana her readings this morning were 32.4!! She refused any food so injected anyway than gradually had fed a bit and she ate a little... she looks better than she is alas... we’ll obviously see how we go but the feeding is a real challenge... her readings are all over the place.
    Skipped the nadir will test again later..

    @Deb & Wink, that's exactly what I was thinking. A Smorgasbord deluxe! @Zee, Mum looks great in that picture. We call it BAR in the Rescue World...(Bright - Alert - Responsive) Keep on doing what you are doing, to get her over the hump, positive thoughts ...:bighug:

    Bless you Deb - but she’s totally uncontrolled 37.7 this morning. She most definitely is very very sick. She goes in and out of being alert to drifting off..

    I adore madam. I knew when I took her on at age 11+ that this day might come.. she’s one of the sweetest cats I’ve ever known and her trust in me is unbelievable... which is why I’ll do right by her..

    Whilst on Mum - does anyone who followed her story have a view on chicken bone broth? I sourced pure broth with no additives etc and she loves it. However in view of her kidneys - it appears to have some phosphorous... thoughts?

    More fluids could make her feel better. Phosphorus content is not important right now.

    Food and fluids are more important than carb or phosphorus content right now.

    Is Mum dehydrated? Have you tented up the skin? Does it go right back down, or stay in that tent?

    Make Mum as comfortable as you can. Keeping our paws crossed for her well being. And yours.:bighug::bighug::bighug:

    Thank you so much for your ongoing kindness! Mum is drinking - whether enough I can’t tell... don’t think she’s dehydrated.. not keen on eating... had a tiny bit. Also had Cerenia to make sure she isn’t feeling sick... she’s not regulated at all today... keep trying to tempt her.. poor puss...

    Her phosphorus levels ain't that high so don't worry about phosphorus content for now, once she's eating normally you can deal with that, dehydration can also cause nausea and lack of appetite, have you given some tough about giving her subcutaneous fluids, that can help with dehydtration, getting her kiddney values to lower which will make her feel better and I think that it could help avoid ketones (not so sure about this last one though)

    Also keep in mind assist feeding her specially because of her ketone history you need her to eat

    Really trying folks... made some scrambled eggs and had to eat it myself... she ate a minute amount of her wet food.. conversation with vet tomorrow...

    Are you able to get Young Again dry food? Fits both the diabetes and kidney diet and is low in phosphorus. Hoping your sweet kitty starts feeling better soon.

    Oh Zee...I am so very very sorry. I've been thinking of you this morning and been worried at your silence. Bless your heart, you did everything possible for your beautiful old lady and she knew it. She is free now and can eat all the dry food she could ever want...

    Do take care of yourself, this is so tough. Post on the Grief forum if it would help - you will get understanding and support there.

    How kind you all are. Saw my vet with her and her kidneys were really gone.. didn’t want to bring her to her knees. I had her for 3 wonderful difficult years, learnt so much... thank you again. I’m ok with it.

    I’m so sorry. It is 4 weeks today I lost my little guy. It is so hard. I know everyone says they are better off and not suffering, etc, but that doesn’t replace the sadness and emptiness. You have many people here that understand your grief. Hopefully knowing you did all you could helps. Try to remember the happy times. You were a good mom. :bighug:

    Thank you. I feel it was the right thing now. Sorry for your loss.. they do leave an empty space but I still have my boy Eric who was pushed aside recently... I’ll make it up to him...
