2/1 Callao PMPS = 110 Low (good) numbers with low doses - maybe try OTJ?

    Hi everyone,

    After dealing with some wild fluctuations for 3 weeks, I finally found a vet who agreed to support a change in diet from a high carb CKD dry food diet to a low-ish carb (10% DM) wet food diet (still reasonably low in phosphorous, and we will be monitoring her kidney values over the next several months). Prior to switching food, her original vet recommended 1 u lantus twice a day and not to shoot if she's below 250. On Thursday evening, we switched to the low carb wet food, and I've been monitoring her fairly closely since then to make sure we don't OD her now that she isn't being pumped full of carbs at every meal. There's been quite a dramatic difference! And a good one, I think!

    Throughout the weekend, we've been pretty consistently in the greens and blues. Kitty seems very happy with her new food and newfound playful bursts of energy. If you'll notice, I've been giving ~0.1 u doses -- basically touching the bottom of the 0 line on the syringe (we have syringes with half marks, but it's still tricky to get that small of a dose).

    I'm considering not shooting tonight, since she has had pretty good numbers with last night's dose of 0.1 u and this morning's dose of 0.05 u. I would like to dose her very small again for consistency. Obviously something is working! But since her numbers have been good, I worry about her BG dropping over night while I'm asleep. At the same time, the recent doses have been so small that I wonder if it's even having an effect. Wondering if there's any advice out there on this issue? Perhaps it's such a small does that it's silly to worry one way or another? I just want to do what's best for this little furball. :)

    Hello and welcome. You've been on quite a ride. :bighug: You still don't need to have veterinary foods for a diabetes/CKD combo. My Neko has both. I just feed high quality, low phosphorus raw or canned food.

    Our experience here is that the longer you can safely keep kitty on insulin to give the pancreas some support, the stronger the remission. It was only a couple of days ago you saw reds. If you could keep her on the 0.1 dose for a week, then you could try the next dose (OTJ trial). :D When you decrease a dose, the depot from the higher dose can influence up to 4-6 cycles after the reduction. It's likely that the 0.4 and 0.2 doses could be influencing today's cycle and helping to keep Callao in nice numbers.

    I would probably keep to the 0.1 dose for a little while longer. If you look at the days where you gave 0u PM, the next morning she is quite high (black, red or pink).

    Thanks! I suspected that might be the better choice, but wanted to hear what others thought. It's easier to worry less if I know what I'm doing has worked for others. (Though I will still certainly worry... it's in my nature!) Thank you both!
