Need advice

    Thank you for accepting me. I’m a new member. I have a sugar baby-Jagger. Doing a curve on him today. Came out of remission 10 days ago. Using Vetsulin 2U 2x daily. Having a hard time regulating. Numbers so far today for curve is
    5:30 am. 267. 2u given
    7:30 am. 236
    9:30 am 119
    11:30 am 115
    1:30 pm. 223

    I have 2 more sticks to go before testing and giving shot. BG on 1/12 1st stick was 558 then back down to 310 in the pm. Having a hard time regulating.

    I'm very sorry to hear that Jagger's fallen out of remission. Sucks, doesn't it. :(

    The BG curve today looks very typical of Vetsulin's action profile. Indeed, for safety you wouldn't want to see the nadir go much lower if you're using a pet meter to measure BG.

    Couple of questions:

    1. What are you currently feeding Jagger?

    2. Was Jagger treated with insulin before he went into remission? If yes, which one?

    3. Are you using a human or pet BG meter?

    4. Has the vet given Jagger a check to see if there are any dental/infection/inflammation/other health issues going on that might have knocked him out of remission?

    4. Vet has performed blood panel (things seemed normal). Didn’t Check his teeth.

    Might be worth getting vet to check teeth and gums; it's a really common cause of kitties falling out of remission. UTIs are another common offender. If Jagger has any history of pancreatitis it might be advisable to ask the vet to run a Spec fPL test to check for any pancreatic inflammation (the test isn't normally included on a standard blood panel).

    What sort of duration did Jagger get on Vetsulin first time around? Looking at today's curve data, the dose seems to be wearing off around the +8 mark.

    Overall today's curve is quite smooth for a cat on Vetsulin and Jagger has spent some time under the renal threshold. (There are some members here who would be quite chuffed to see a Vetsulin curve like that! :) ) If you are considering adjusting the dose, I'd suggest doing so in very small increments (e.g. 0.25IU Vetsulin). Because of the way that Vetsulin can really drop BG from preshot BG to nadir, for safety it's wise not to target too low a nadir BG. Jagger's already dipping into the low 100s after only 10 days of treatment. If it were my kitty getting those numbers I'd be very cautious about tweaking the dose, especially as there's a question mark over whether constipation might be raising Jagger's numbers. Assuming for a minute that constipation is a contributing factor then Jagger's numbers might improve significantly and quickly once the constipation is resolved and the dose might need to be adjusted downwards. (Other members may have different suggestions.)

    The spreadsheet provides a really helpful visual representation of a kitty's response to insulin treatment and it's very helpful as a guide to improving regulation and making dosing decisions.

    You may find that by holding the current dose Jagger may start to trend down as the Vetsulin allows the pancreas to get some rest. Also, if you can identify any other issue(s) that bumped him out of remission then treating the underlying problem could lead to a big improvement in Jagger's BG levels.

    Thank you for the info. When he was 1st DX IN Mar of 2016, I managed to get him into remission about 4 was later with taking the dry food away. I test often so I feel like I caught it pretty quick after his numbers started going up.
    I will get a spreadsheet started so I can post when needed. Thank you for the help and advice. I’ll be following him around tomorrow to make sure he is pooping.
