Thank you in advance for your help with this.

Our Winchester was diagnosed with diabetes about a year ago and since then he has gone into remission, become diabetic again, and now appears to be transient. Winchester had one unit of insulin Friday morning. His blood sugar has been hovering around 150 since then and he’s hasn’t had any further insulin. When he was first diagnosed, he had a very scary incident of DKA.

Winchester also has heart disease, asthma, and kidney issues related to his diabetes. He takes furosemide, benazepril, pimobedan, and inhaled fluticasone to manage those.

Winchester has had an upset stomach since last week and has vomited several times. We have been to the ER vet twice. Two sets of X-rays came back clear. All blood work also appears normal (no kidney concerns, no pancreas concerns, mild dehydration).

He also had some yellow eye goop discharge yesterday. That appears to mostly have cleared up. We moved in December and adopted a new kitty about 6 weeks ago. Winchester is a pretty chill cat and has handled it all well. New kitty and him are getting along well. Maybe related?

Winchester hasn’t had a full meal since Friday morning and it is now Sunday morning. He’s had a few nibbles and water, but his appetite is not normal at all. The vet gave him an appetite stimulate and Pepcid on Saturday morning.

The ER vets have been helpful, but I am concerned that they are missing something. My last experience with them give me a bad gut feeling. His main vet is now 2 hours away and has consulted via phone and email. I will probably bring him there tomorrow. In the meantime, what should I do?!

I would check him for ketones as often as you can (twice a day if you can) while he is not feeling well.
Do you have any all meat (no onions or garlic) baby food you can try to entice him with?
If he seems interested in food and then licks his lips or walks away or takes a bite or two and then walks away or licks his lips, he may be nauseated.

Why check for ketones if his blood sugar is on the low end? We are checking blood sugar every 1-2 days.

He doesn’t seem interested in any food. I’ve tried offering him variety of things.

Cats don't always have high blood glucose when they get ketones. I would just check to be sure, for peace of mind on that.

Only checking blood sugar every couple of days? Since he isn't feeling well how about checking a few times today and see what is going on. He could be going higher or much to low.

    Hi,
it is not common to throw ketones in the Blue BG range but it is not unheard of.
There is someone else on the Board right now dealing with the same situation - good BG, no dose and possibly ketones. @Joanj .

There was great post on this subject by Jill & Alex (GA) but my PC was reformed and I lost all of my bookmarks and I do not remember how the situation was dealt with...
I tag @Jill & Alex (GA) in case she has the link to that post or if she can give any advise.

PS. Being dehydrated might interfere with eating. Is he still on furosemide?

Thanks, I’ll keep an eye out for the link.

Have checked ketones Saturday afternoon and it was negative. Haven’t been able to check since then.

We have skipped a dose of furosemide and will probably skip this evening as well.

He’s acting a bit more like himself and continues to nibble if I put food in front him.

Any chance this eye thing is related? Or just a seperate infection that needs medication?

I'm sorry. Just seeing this now. I *think* the post you're referring to is "Suggestions on How to Stimulate Kitty's Appetite" written by Cheryl and Winnie.
It's possible it's eye related, but that's a determination a vet would have to make after an examination and probably blood work.
Did you decide to take him in?

    Edited to add:
The INDEX: Health Links/FAQs contains links to a lot of good informational type posts.

    Hi Jill, Thanks!
No, it is not the post I meant...

The one I was talking about (read early in 2017 so forgive me for not remembering)
concerned your Alex when she was either well regulated or gone remission and you taking a test in mid/ low Blue range whilst Alex threw a ketones at you unexpectedly.

I believe the thing happened on either a long weekend or a Holiday or perhaps you were snowed in?..) - but I remember reading that you had trouble getting LRS for Alex on that day.. Does it ring any bell at all?

I might be mistaken but I think the Title might read something like "Alex the veteran had ketones in Blue..." I do not remember the exact wording, sorry.

I swear I do not imagine this post - my Ducia was having ketones as well at the time (early 2017) but she was in Pinks mostly and I thought that I simply should bookmark the post for future ref in case my Ducia ever goes Blue + ketones...

This may sound weird. But have you tried putting a low/no-salt low-fat chicken broth on his food? I'm new to this community, so I don't know for sure, but it may be safe for diabetic cats (check with the vet). If it is, it's a good way to "trick" your cat into thinking he's getting a a special treat, and it could bring his appetite back.
