02-07 Small Kitty amps 225 pmps 263 +3 538 - DKA preventive treatment started

Morning all,

As you might have seen yesterday, blood test on Small Kitty showed ketone built-up.
Freestyle Neo Ketone meter gave results of 2.7 (meaning is positive). Test strips accuracy
was confirmed by testing myself, 0.2 ???

Got some urine today and tested with Keto-Diastix AND Chemstrip 7 urine test
stix and both shows negative results. Have pictures to analyse the other element
of chemstrip 7 but think it's alright.

Now, what's confusing me is the fact that there is possible glucose toxicity
with numbers that are not that bad. We even got blue and green passes.. doesn't
make sens to me. Sorry for forgetting his name, but one of our guru posted something
interesting, will jump into it if it's quiet today.

Ha, another thing, Small Kitty is eating like crazy, not a sign of DKA, for what I know
of the phenomena.

Anyways, don't worry about me, I'm not freaking out at all, a counter offencive
will be put together and the enemy will be eliminated, just a question of weapons.

Lucky me I have some support from you and now have someone in my life 24/7.
She's not next door but some feels have no frontiers.

I don't know anything about ketones, but I hope you figure out if he really needs treatment and can take care of things at home. Sending prayers.
 
I am relieved you got a negative ketone result this morning.

I am loving the yellow start today. :cat: Be safe today Small Kitty and don't let any ketones near you....they are enemy number one.

When Osha had DKA she was barely eating, so I believe eating well is a good sign.

When my guy had ketones, he wouldn't eat and barely moved. He was limp like a rag doll - a very sick kitty. It doesn't sound like SK is acting like a Ketone Kitty.

There's no mystery here. Trace ketones can show up and then they could be gone (flushed out) at the next test. This is why I said a single trace ketone test doesn't concern me yesterday.

Again, not a mystery. A kitty can be throwing ketones without being in DKA.

A kitty doesn't have to show signs of being extremely sick when throwing ketones. @Bobbie And Bubba can certainly attest to that. IIRC, Bubba was throwing moderate ketones, not acting sickly, and eating well. However, the treatment is the same: get more insulin into the cat, make sure they're eating well, increasing water consumption/administering squids, and treating any health issues that may be present.

If I'm understanding your thoughts, you're confused about the possibility of SK throwing ketones when his numbers have been pretty good (blues & greens). Here's the thing, when a kitty's numbers are high, they practically scream "test for ketones", but ketones can and do develop in lower numbers (blues). It's rare, but it can happen. Recently, I mentioned the two worst cases of DKA I've seen on this board were when kitty had been pretty well regulated in the 100s. It was touch and go with one of them for a couple weeks. My point is, contrary to popular belief, a kitty can develop ketones without being in high numbers.

Done with my reading and conclusion are not very reassuring... It confirm Jill's theory on possibility
of DKA even with low number.

Found :
  • A cut-off value of 2.4 mmol/l revealed 100% sensitivity and 87% specificity to diagnose DKA.
  • False-negative readings have been reported when the urine test strips have been exposed to air for an extended time (Keto Stix and Chem Stick Expiration date not reach, 2018)
  • In cats with a β-HB concentration < 2.4 mmol/l, DKA is very unlikely.
This was not part of the plan, hope flooding the system + extra food will clear this up. That's
the only thing I did last time, give 80cc of water every 8h. or something like that + force feeding.

Yep Bubba was one of those kitties that was throwing ketones in the 200 range which reminds me that it's been a while since I tested him for ketones. ;)
 
Sebastian, I think you might be pushing the panic button. SK has had one positive ketone test followed by a negative ketone test. A cat throwing a little bit of ketones is not the same as a cat being in full-blown diabetic ketoacidosis (DKA).

Up 'til now, all this says to me is:

  • to make sure my kitty is eating well,
  • to make sure my kitty is getting more fluids than usual (via mouth or sub-q fluids),
  • to make sure full insulin doses are given on a timely basis,
  • to carefully observe my cat (look for signs of any health issues & deal with them if need be), and
  • to continue testing for ketones often.

A single positive test for trace ketones is is a warning... a red flag. It simply means I need to be alert and cognizant that kitty is on the edge of what could be a very slippery slope. It means I have to take the precautions I listed above.

Now if he continues to throw ketones, you'll have to be more aggressive, but let's cross that bridge if we come to it.

One day my best friend told me : ''We would ask you to built a bike cycle and you'll come up with a tank''. :)

But, I've had a bad experience that I do not want to relive. Will take all available means to stop progression of possible DKA.

Urine level is VERY high and I don't see see Small Kitty drink that much.

Anyhow, out of Ketone blood strip test, will buy more this week-end after sub-q will have clear our potential treat.
I've added water to food as well. Vet recommended Sub-Q every 8h but finally said 12h will be fine as I'm working.

Hope it's just false alert...

Sebastian , I so get why the ketones make you nervous. But, as Jill said , at this point it's a red flag to observe all those things Jill listed. Bubba got as high as 5.0 ketones on the ketone meter and never went into DKA because :

He was eating

I was giving lots of water in his food to flush the ketones as the Sub Q fluids will do .

And , because he was not sick or stressed.

As Jill said just be vigilant with all the things that Jill outlined.

I totally understand your fear of DKA. My guy had an episode in March last year. Very scary and very $$$$.
