BigMac is in surgery. Update - I'm home & calm.

    With a heavy heart I am sitting here in the hospital waiting room as BigMac is in surgery.

    Yesterday after another ultrasound and exam by a specialist, BigMac was diagnosed with adenocarcinoma. It is on his bile duct. While this is early and the mass seems to be tiny, the doctor told me it is a very aggressive type of cancer.

    The surgery is to re-route the blocked bile duct to help regain the flow of bile from the liver. The bile has been backing up into his liver causing it to be enlarged and angry. Our aim is to make BigMac be more comfortable and feel better for as long as possible. They will resect whatever mass they can but I was told that the cancer will probably still be in his body and will spread as time goes on. :cry:

    I have been concerned about his weight loss for several months and we've been to the vets often for lots of tests. His liver enzymes kept getting worse even after trying several medications. Finally my vet said they needed to find the answer since clearly they were missing something. BigMac's appetite and demeanor have been normal, no outward symptoms except the weight loss which was getting alarming.

    My vet said that the only thing left was exploratory surgery. Before we did that we were referred to this specialist and hospital. We got an appointment quickly and after the evaluation yesterday they wanted to do surgery right away to give BigMac as much time as possible and help him feel better. And avoid the sickness that was coming when his bile duct became 100% blocked.

    Poor BigMac thought he was doing ok. He gets so horribly stressed at the vet that his heart rate skyrockets. (I used to have a vet that did house calls for this very reason.) They let me keep him at home last night instead of their usual pre-op requirement of an overnight stay.

    He won't be able to come home for several days and will have a stomach(?) feeding tube, but I'm going to stay here until he is resting comfortably.

    So now I wait.

    (((((((((((((Carolyn and BigMac)))))))))))))))) My heart is in my throat and I'm wishing with my heart and soul he comes through this okay. I would be sick with worry and anxiety if this were Maverick. I always stressed about him stressing about the vets too. You caught it early. Every day you have with him is a gift to treasure.

    Is he getting a neck ETUBE or a stomach PEG Tube? If he's getting either - please join the Yahoo Feline Assisted Feeding group to read up. If its a PEG tube you really need to join and get help and give the group updates. Maverick had a PEG tube with complications. He needed a second major surgery at day 10 because he pulled it out before the stoma formed. We'll give you lots of tips and hand holding. He won't be able to be fed via a PEG tube for at least a day.

    I really wish I could be there for you and give you a real hug. Big Mac is in great hands and he's going to pull through this. When Maverick had his surgery in 2008, I just wanted even a day or two more with him. It would have made it worth it. I got 2.5 wonderful years but would have been grateful for just a short time.

    You are doing all you can, and you have been right along. You are a good bean and BigMac knows that you love him.

    You know I'm not eloquent, I just wish I could do something....you do so much for all of us.

    oh carolyn, yesterday in Loose Lips you said something about needing your spirits raised and i wondered then . . . i'm so glad you opened a condo so we can keep you company while you wait. we're with you in spirit, sitting right there. can i get a cup of coffee or tea, or my personal favorite, a can of diet dr pepper for you?

    sending many "be strong" vibes to Big Mac. he's a special guy - of course you want as much time with him as possible. on the bright side, several people here have dealt with cats with cancer and it seems that they tolerate chemo much better than other species.

    Thank you everyone. Oh my, I start to tear up every time I read your kind thoughts.

    BigMac has been in surgery for an hour and a half so far. No news yet.

    Thank you Julie, I am drinking that Dr. Pepper you sent. :lol:

    I do feel all the hugs and snowflakes... That explains why my toes are freezing. :eek:

    Binks chivvied me out of bed early this morning during a snowstorm- and he and the girls were immediately begging to go outside.
    When I questioned them, they told me that all of LL was gathering bouquets and all the snowflakes available to sit vigil with you and your wonderful boy.
    I know you can feel us all there in spirit, as a great outpouring of love surrounds you during this difficult time.

    Still waiting. Three and a half hours in surgery now.
    (I've been here 6.5 hours.)

    Love the Dr. Pepper, Julie! Hahaha!
    I do appreciate all the hugs, it makes me feel less alone.
    I just heard a kitty meowing and it is breaking my heart. I know it wasn't BigMac but it still got to me.

    Here he is just before the tech took him away. He clearly wanted out of that box. But I'm sure once they got him in the back - he wouldn't get out for anything!

    Oh ((Carolyn)), so sorry to read this news. :sad: Bailey and I are sending prayers, vines and snowflakes for BigMac's recovery. The waiting is terrible. :sad: I hope they are able to get the cancer. Oh, those big eyes in the pic you posted!! We are waiting with you.

    While we wait, a carrier story for you.

    Once took Binks and Homie(GA) to the vet in the same carrier.

    In the exam room, neither was willing to make an exit.
    To my utter shock, the vet tech picked up the carrier, aimed the open door toward the floor, and shook it vigorously :shock:
    Binks tumbled out immediately, but not Homie.

    When I grabbed the carrier away from this idiot, I looked in, and there was the Homester,
    claws spread and braced against the side of the carrier like a hairy little Bruce Willis! :lol:

    BTW...that was that vet tech's last day at work :twisted:

    I used to have to dismantle the carrier to get BigMac out. But I've learned to keep a cushion in the carrier and drag it, and him, out at the same time.

    Still here... 4.5 hours in surgery.
    Starting to worry now.

    Here is my view for the past 7.5 hours. I do get up and walk outside every so often.

    :sad: not yet.
    About 10 minutes ago the Tech did say they were getting close. But this is extremely upsetting- 5 hours? It just seems like too much, way too long. I'm so worried about recovery, as in, will he? [insert nasty words here]

    look at it this way.. if he wasn't going to make it, or things were too far gone.. they wouldn't work on him for so long.. I think it's a good sign.. his vitals are obviously good enough that they can continue, and they obviously think it's worth it!! hoping the vet tells you amazingly good news shortly!

    Big Big Big huge smothering hugs for you. You caught this very early. He is going to get more time with Mom and thats what counts. More time for hugs and kisses, purrs, meows, dances, songs, brushing, treats and lots and lots of love. He is having a big surgery. Maverick was opened up from chest to groin and was in liver failure - so yellow he was almost orange - he had this surgery twice. But once he starts recovering it will refill your heart. He is a strong and healthy big boy. He will recover quicker than Maverick I bet. You stay strong. Remember, they can feel your anxiety and worry. Try and be strong for him. Let him feel your love and strength. He's in great hands with his doctors and you looking out for him. More hugs.

    OH, so sorry to see this post. I will be thinking of BigMac all days till we hear again. Its true, what someone said, about being in surgery so long. If it wasn't good, they stop and don't finish. So it looks like they are probably making sure they get it all. I know how hard this is, I lived in the waiting room of a hospital for 2 days! Hang in there Carolyn, sending lots of healing vines and white light.

    They are finished with most of the surgery. The feeding tube is in and now they are putting in a central line. Tech said "she" is doing fine, even after 6 hours of surgery. I guess BigMac is so cute with his long flowing hair that the tech thought he was a she. ;-)

    I hope he will be in recovery soon. I'm going to continue to wait as he is coming out of the anesthesia. So my toes will keep on being frosty from all the FDMB snowflakes.

    The tech seemed at ease when talking about BigMac so I do believe her when she said he is ok.
    Still have a little way to go, but at least I know he is hanging in there. Way to go BigMac!!! cat_pet_icon

    mocha is so happy to hear this, she said she is willing to share her brand new bag of chicken freeze dried treats with big mac and all his friends .. and trust me, THAT is not something she would normally do, so she is super stoked about him getting better!

I know how impossible that six hours must have been but we were all with you in thought and prayers and will continue to be throughout BigMac's recuperation. It is great news to hear that
    the surgery is over, his feeding tube is in, and you'll get to see him soon.

    It sounds like a positive update to me, I agree that they wouldn't keep him under so long if he wasn't doing ok. :smile:

    You know that LL vines are magical....positive thoughts, snowflakes and vines are still on the way.....and of course :YMHUG: for you!

    Saw this post & the tears came, and went & Now the very strongest Prayers will be said for Big Mac's Recovery! I have been in one of those rooms many times last few years, It's so difficult, but you have us & we are all Here For you--For Strength, for Support, For Love , for help & HOPE!

    Wishing you and BigMac the best possible outcome and lots more time together. Know this diagnosis as it was very similar to Yoshi's but we didn't find it early.

    What an awful wait you had to endure! I'm glad to see you had so many people offering support online. Please know there were many of us silently on the sidelines- watching, waiting, and sending you massive amounts of healing vines. I am relieved to hear he is out of surgery and recovering. It's going to be ok, Carolyn. I just know it! You have a heart of gold, and it is wrapped around Big Mac right now...protecting and helping him heal.

    BigMac's surgery was finished at 5:30pm. That was a very long 6.5 hours of surgery. Whew!

    He had 3 surgeons working on him. Of these, two came in to talk to me afterwards. The first thing I heard was that BigMac was already waking up as they barely finished. They had to keep giving him extra sedation because he was not staying asleep as deeply as most animals. Not sure what to think about that.

    I know the doctors told me a lot of stuff and drew all sorts of diagrams and showed me X-rays they had on their iPhones... I will need to go over it again tomorrow because my main thing concern was that BigMac was going to be ok. The doctors seemed very animated so I asked if this case was something they saw a lot of ... and they both said, Oh no! BigMac was pretty unusual in the location of the mass and what was involved to get it removed and to repair the stomach, intestines, bile duct and even a bit of pancreas. Without really going into detail it had to do with fixing one thing and then another because they had to move an opening and close another. That was a bad description but for now it will do.

    He has 5 tubes snaking into his body. He has a J-tube into his abdomen, E- tube that reaches down to his stomach which they are not using for feeding at this time. I forgot what they said but will find out more tomorrow. He has a weird long perforated tube snaking around in his stomach that they use to monitor what is going on in the stomach... I think stuff comes out of that and they test it. He has a central line in his thigh since they couldn't get the central line in his jugular vein. (Too many recent blood tests there apparently.) And there is another tube but I forgot where/what.

    BigMac was kind of lively when I was with him and they gave him more pain meds and he immediately calmed down. He did not fall asleep though. He put his nose to the shirt that I brought and he started kneading it. After a while he rested his head on my hand. I stayed there for almost 2 hours, standing at his cage. I tried to remove my hands and see how BigMac did and I hoped he would be calm and sleep, but he kept picking up his head and moving around. We didn't want him to do that. The surgeon came over and gave him more meds and said that he would rest now so I could leave. I told them I'd be back tomorrow. They smiled and said it would be good for BigMac if I came back. :D

    The surgeons seemed glad to be able to use their skills on my kitty and from their demeanor they felt like they did a great job. One of the surgeons drove up from Atlanta (2 hours) to be a part of this.

    My head is still spinning and I haven't eaten anything today... I only had that Dr. Pepper that Julie sent. :lol: So forgive me if I seem wonky.

    I want to thank everyone so much for keeping me distracted and encouraged today. It was so very hard to sit there and not know what was happening. At the 4 or 5 hour mark I was inwardly frantic about how long it was taking but you guys brought me back from that despair by pointing out that they wouldn't keep going if it was not going well. That is pretty much what they said to me afterwards.

    You guys are SO SMART!!!

    Wow. What a day. We are sooooo glad to hear your great report. What an amazing team of surgeons! He is doing fine and will continue to do fine. Now for recovery. Big hugs to you, CD. We hope you can sleep tonight, knowing Big Mac is in good hands.
