Lantus to Levemir - Sunshine

    Sunshine's vet has authorized a prescription for Levemir.

    What is the typical protocol for switching? The Levemir should arrive around 10:30am. I generally shoot Sunny at 6-ish am and pm. Should I skip tomorrow morning and start with the evening shot or shoot both am & pm?
    Should I start with 0.25u or 0.50u of Lev?

    You can shoot one in the morning and start the other on the p.m. shot. They form different styles of depots. Lantus forms crystals under the skin. Levemir binds and unbinds to the albudin (sp?)

    General rule of thumb is to start at 75% of the current Lantus does. However where Sunny girl has never been stable on Lantus I would start her at.5 and adjust from there.

    When I switched Mikey from Lantus to Levemir, we did a morning dose of 2.5u Lantus and then an evening dose of 1.5u Levemir. The change in him has been remarkable! I hope Sunshine does as well as Mikey has on the new insulin. Good luck, you two!
