i know how frustrating that is when your posts aren't coming thru. If you are using a phone or similar device, sometimes they don't cooperate.

I need a recap....
What was his last dose on prozinc and how long ago was it....?

If you just switched. You can start lantus close to the dose of prozinc.

You can also start it according to his weight.

Lantus is a depot type insulin and it needs to accumulate in his body.
There are many sticky's at the top of the forum that you will want to start reading.

I don't see any of our more experienced members on at the moment....but I am wondering about insulin resistance. And that is not something I am well versed in myself.
There are tests.
But meanwhile you do need to move forward to help Baxter now.

Everyone will love for you to create a spreadsheet.....
and if you need help doing so, we have members who are good at helping with that.

We need to see the bg numbers in order to best help you and advise you on dosing.

It sounds like you could best you someone to really guide you on the day to day details and that is what we are so good at here.
We want to help.
keep asking questions

you will want to post a daily condo ( like this one) and give us details of what's going on....
bg numbers

what you shot....

all the things that we will be seeing in your spreadsheet.....

Most of all, it sounds like Baxter could use some consistency and methodical dose increases ...
we have a protocol we follow for doing just that.

so Welcome Melissa and Baxter.....

Wellcome , I'm not an expert hopefully one of the most experience members will soon take a look but in the mean time you may want to take a look at the protocols, TR (tight regulation) or SLGS (Start low go slow) see which one suits you better, they both establish a starting dose that varies a little with each protocol, but both consider adjusting the starting dose according with the dose he was on the other insulin.

Are you shooting every 12 hours?

Also the general recomendation is to increase the dose in 0.25u increments that way you don't miss the right dose, some times the right dose is just a little bit more than what you are giving and that little makes all the difference
