    Phoebe opinion/question #2.
Should I be able to keep her in blue and green constantly? Can I ever get her to the point of no insulin?

She's looking better...getting some yellows and blues. If you could grab a nadir number soon (maybe run a curve this weekend?) that might help. My guess is she needs more insulin, but could be wrong.

You DO want to get her to blues and greens. Those are the healing numbers where the pancreas can hopefully heal over time. You know cats can achieve remission...the longer they've been on insulin, the less chance there is, but hey...I've seen it happen after years. However, you're going to have to get more aggressive with dosing for that I think. You'll want to get as many mid cycle tests as you can (I know it's hard when you work and whatnot...you do the best you can) and be willing to raise the dose faster than you have been. I do think Phoebe could come down into pretty blues and greens and achieve remission, but you can't really stick at a dose that isn't working for such long periods of time to do it. I'd LOVE to see her get remission!

Eric, in an earlier post in this thread you were asking about getting Phoebe into some better numbers. Would you be open to some suggestions on doing that? I don't want to impose if you'd rather not, but also don't want you to think we don't care. I just haven't been sure if you wanted advice....? :)

Okay, so let me talk through this for a minute. If I'm remembering correctly (and please correct me if I'm wrong here) you are gone for work at least 12 hours Monday-Friday, sometimes a little longer. So you can do most of your testing on the weekends. So I'm thinking you probably usually shoot right before you leave the house, and then again right as you walk in the door. Is that right? And how many hours are there between your usual PMPS and your bedtime?

Based on Phoebe's numbers, she needs more insulin. Normally we don't recommend increasing unless it's on a cycle you can monitor, but she's been stuck in high numbers for a long time, so I think you can safely move her to 2u on your next cycle, and then hold her there until the weekend when you can test again. I'd like someone else who follows you to confirm if they agree with that or not though since it's a little unconventional. :)

And just to confirm on the food, are you still feeding the NV Instinct? Are those the Limited Ingredient ones? And is that the canned or the raw?

I like Djamila's plan. I agree that Phoebe could use a little dose boost. I'm also a fan of consistent AM/PM doses while you try to sort things out. :)

Currently I'm starting work at 8am and I'm off by 4pm. I dose her at 630am and 630pm. I'm usually out the door at 730am and I'm home by 415pm. I feed and dose her when I wake and feed her upon my return. I'm usually in bed between +3 or +4 after Phoebe's PMPS. But I do get up early in the a.m. by Phoebe. I've only been feading her Fancy Feast Classic chicken and beef pate because of money restraints but she loves it. I've wanted to bump her up to 2u but I've been very afraid to do so. Thanks for all your help!

The Fancy Feast is fine, Eric. You have a nice little window of opportunity between the time you get home from work and her evening shot time to get a few numbers that can be very useful in guiding your dosing. Those, along with an evening test, will be great data to have. :)

Group question? What number or higher would you shoot 2u? What if Phoebe is 201 or 299 at PMPS?

If you can get a test in after, I'd give her the 2 as long as she's over 200. She's been sitting in those higher numbers for so long, that you don't want to lose momentum if a dose starts bringing her down. Looks like you could grab a +1 on the way out the door in the morning and get a few tests in the evening, if needed, right?

Yes. Getting tests is never a problem. I usually test her before I go to bed and sometimes in the middle of the night when I or she wakes me.

Sounds like a good plan for the next step, and your schedule will allow you to keep an eye on her if you do your dose changes in the evening. Do you leave food out for her when you're at work? Is she able to graze during the day?

No I don't leave food out for her. She used to graze when she was on Young Again dry food before she switch to wet food. Phoebe was at one time a dry food junkie. If I was to leave wet food out she would eat it all. So in the morning I gave her a 3 oz can. And before I leave for work I might give her .75 to 1 oz. Then when I come home 3 oz. Before I go to bed Ill give her 75 to 1 oz.

Sam used to always wake me up at 4:30 for a snack. Since he was waking me up anyway, I moved his shot time to 4:30am/pm. Now he wakes me up at 3:00 :stop::rolleyes::smuggrin:

Since she doesn't have food during the day, that just helps to give a little guidance to how we manage the increases to be sure she stays safe during the day. Any chance you have a birthday coming up and can ask for a timed feeder? ;)

No birthday till October. Yes I'm all aware of the sliding a.m. treat meow...hahaha. I hope this doesn't take her dangerously low when I'm at work? That's one thing that worries me.

She should be fine at 2u right now. And we'll wait to get enough data to be sure she's good before making the next move. Just make you sure you remind her no funny business while you're at work. :cat:

She will likely need a few dose adjustments to start to bring her numbers into healthier ranges. First we'll need to find a dose that will bring her nadir down, and then as she gets more time in "healing numbers" hopefully her PS numbers will start to come down too. Ideally you want her PS numbers to be in the yellows at the highest, and her mid-cycle numbers to be consistently blues and dark greens. "Ideal" doesn't always happen, but that's the direction to aim.

I think Phoebe may of had too many treats yesterday. Lol. I don't mind getting up at 1:30a.m. to get that because it's better than her waking me up at 3a.m. or 4a.m. for a treat lol!
