Riley's sick again!

    I took Riley to the vet when I came home and checked his sugar was around 500 he has been in remission for some time now. He does have kidney failure and has had radiation therapy on his thyroid. The vet has me giving him 3 milliliters 2 times a day and I'm trying to remember exactly how it got him in remission to begin with. Of course through soft food. He's been getting into dog bowl latley :(

    I need to start filling out a spreadsheet but his numbers seem to be consistently High but for some reason in the mornings after a 12-hour cycle they're the lowest at 315. I hesitated on giving him the three milliliters today but I went ahead and did it because I can monitor him at home.
    He has not seemed to be dropping too low at all actually the numbers go up higher than that during the mid cycle but then they are at the lowest point at 7 a.m. pre shot.?
    We started him out one and a half MLS and his sugar stayed at a steady 400 so she went up to 3 ml's and it's got a little better but not much. The vet says that she wants to wait before we raise it anymore I'm just concerned he's getting too much but if his numbers are not dropping too low then I guess he's not?

    Sorry you have to be back and Riley is out of remission :bighug:
    I'm wondering why she made the jump from 1.5U (units) to 3U. I haven't been around terribly long but long enough to know that's a HUGE jump. What dose was he on before and how long was he on insulin? Was he given a steroid? That can throw a kitty out of remission too.

    Hi Dana, and welcome back -- although I'm sorry you have to be back here! Getting your spreadsheet back up and running will be really helpful. So the jump from 1.5u go 3u was all at once? That is a pretty big increase. I think the best thing right now is to get back into your testing routine - getting a test before each shot, and mid-cycle test whenever you can. Once the spreadsheet is up and we can look at the data, it will be easier to help with dosing.

    I agree with what's been said about the big jump in dose. The spreadsheet will help to sort things out.

    I don't remember how to get my spreadsheet up I have it downloaded on my phone but I don't know how to do that period anyways I'm freaking out now because his sugar last night was 190 and I did not give him a shot and this morning it was 400 so I gave him one unit and then I came home and his sugar so high it's not even giving me a reading so I just did with the vet said and gave him three units I freaked out.

    I'm going to be keeping an eye on him I just don't know what to do I forget everything.

    I think he bounced because he was really high when I gave him a shot and then the next cycle he was low?

    I'm thinking 3.0U is too high a re-starting dose. He was never on that much the first time around. Wondering if you should just hold 1.0U for a few cycles (2-3 days, numbers permitting (so no Preshot under 200) and then if needed, slowly increasing by 0.25U. Hopefully the more experienced members will chime in :). I think tonight's HI reading is a bounce due to what Janet said above.

    If it's under 200 definitely I will not but if it's over I should start with one unit?

    What time are your PS tests and shot time? Are you able to get mid cycle tests? I would still say if he is over 200, start with the 1.0U

    6:00 a.m. okay. Yes, Ive been checking him and he's still too high to read. :( I've never had that happen before!!

    He's bouncing very high from too much insulin. I agree with the suggestion to try 1.0 unit AM and PM for a couple of days to get him settled down.

    Let's see what it is in the morning. She was really holding her own for a few days there with almost no insulin. But since it's a human meter, anything over 120 us above normal. I think of you get
    150-200 you should do 0.5
    Anything else 1 unit for a couple of days is a good idea.

    That's from her remission back in 2015 :). Dana, maybe add a row of solid cells to separate the new data :)

    Unfortunately, this is the old SS which we had to quit using because it had some significant glitches in it. It would be best if we could get him over to the new SS.

    I can help you (actually, I can do it all for you if you like). Please PM me but we only have tomorrow as I leave for France on Wed for two weeks.

    I agree with what others have said -- hold steady at 1u for a few cycles and let Riley's body re-adjust to getting insulin, and give you a few days to settle into all of this again.

    If you do get a number below 200, I would not recommend giving a reduced dose as your first step. The general recommendation is not to shoot a preshot under 200, but to wait 20 minutes (without feeding as food raises blood glucose levels) and retest. If the number is rising and above 200, then a shot can be given.

    I agree. I think holding 1 unit for a few days and getting whatever midcycles you can would help. If under 200, try the stall and retest method to see if he comes up.

    I really think he needs a lower dose. We've suggested 1 u twice a day for a couple of days. What do you think?

    Hi Dana, I saw your post over on the main forum, but thought I'd respond here in case other Prozinc folks are around to give an opinion as well. I would say yes, go ahead and give the 1u, and then get a couple of test or two this evening before bed to make sure Riley is staying up in safe numbers.
