While I haven't been recording on the spreadsheet, I always test Spitzer pre-shot. Lately, I have been testing him throughout the day, though I've not been home 12 hours straight to do a full curve.
I had to restart his Lantus after his hospitalization (in early August) for repeated vomiting; the ER vet wanted me to start at 0.5 units of Lantus. I started there and after a week, was getting several mid-cycle tests in the 200s (+4, +6 +8), so followed protocol and upped him to 0.75 units. Another week, and he's still riding high; upped him to 1.0 units. Then to 1.25, and now to 1.5 units, each time with a week or more at the dose.
Basically, his glucose control sucks. At 1.5 units of Lantus, he's giving me middle to high 300s and 400s for pre-shots and mid-cycle values that never go below 200 if they drop at all. If I go over 1.5 units of Lantus, he starts vomiting again and his mid-cycle glucose crashes down to under 150. The under 150 would be nice ... if he didn't start up with the vomiting, which can put him at risk for a hypo incident and was what sent him to the ER before.
I am feeding Sophisticat Turkey and Giblets, Fancy Feast Turkey and Giblets (usually mixed in), and testing out the new Special Kitty Turkey and Giblets in the 3 oz cans which has a different formula from the 6 oz cans (the latter contains soy, which some vets believe may have hormone-like effects). Sometimes, I add in some of the Fancy Feast Kitten Turkey and Giblets for the extra calories; Spitzer is about a 1.5 out of 5 for body condition (poor), just skin and bones.
I called the vet's office today (new vet, as practice was sold - which is OK as new vet supports home testing and is willing to do research) and left a message asking to switch Spitzer to Levemir. I also sent some of the Levemir links and a couple articles - Gilor and Chen's manuscript and Rand's summary on dosing ranges. She called back and said she wanted to do some research first, so I let her know about the e-mail and message board - I wasn't sure if she'd be able to view the message board without a login, though.
Note: when Spitzer was hospitalized, they did an ultrasound and found hepatopathy. They did a needle aspirate and found nothing conclusive. It is possible the aspirate just missed an area with disease. A fPLI had come back with a middle normal value. His mother was seized in a humane case and the photo I saw suggested inbreeding for a couple generations (lots of buff tabbies). Thus, he may have some genetic defects or occult illness going on, too.
If I can't get him controlled, is it fair to him to keep trying? Or should I just let him go?
