Diabetic cat with pancreatitis

    I hope I am posting in the right forum 12 year old cat been diabetic for over a year almost going in remission I dropped off last Thursday for glucose curve check when I picked her up she would not eat and she loves to eat I took her to the vet friday morning she said her abdomen and back was tender and her bloodwork said showed abnormal pancreas reading she had me wait while she gave her fluids and nausea medication when I took her home she was her perky self and ate well later on it was obvious she was not feeling well stayed in meatloaf position in front of the water Bowl not drinking water I took her back Saturday she stayed for the day fluids pain and nausea medication vet did not feed her lunch and said to only feed her half of what she normally gets and to reduce the insulin to one units twice a day she also said her bloodwork showed pancreas reading of 50 that was high it should be 25.This morning was not as perky but she started eating she ate very little drank very little a water and the same thing happened at her lunch feeding she ate very little she stayed in the meatloaf position all day by the water bowl. At dinner time she seemed interesting in eating did the same thing not eating much so I put low sodium chicken broth her food and she ate a lot more she is receiving her nausea and pain medication the vet said to feed her 3 times a day but is it better to give small meals throughout the day please give me any advice that you can thank u

    Call an Animal Hospital that also besides the Texas fPLI can do an UCG onthe pancrea and have her on tube feed and IVfluids.

    I answered you on the intro page.

    I am concerned the vet said to halve the food intake and reduce the insulin. Doing those two things with the inflammation of the pancreatitis is a recipe for inviting ketones.
    As I said on the other page I would not reduce the food intake, I would offer smaller more frequent meals.

    I would also go out to Walmart or a pharmacy and buy a bottle of Ketostix so you can test for ketones. All you need to do is collect a urine sample from your kitty, dip the test strip in the urine and read the result against the colours on the side of the bottle. Anything above a trace of ketones needs vet attention. Ketones can lead to the dangerous DKA ( ketoacidosis) and it can be prevented by testing for ketones.

    Are you home testing?. I would strongly advise you to think about doing it. It is far easier to manage the diabetes when home testing.

    Just answering your questions from the intro page.
    With feeding I would offer her small amounts of food frequently during both cycles.
    If she will tolerate the broth in the food I would offer that as well or on its own.

    You mention she is lethargic. That could be from being dehydrated.
    Do you think she is dehydrated?
    Are her gums slippery or tacky?
    If you pull up her scruff does it fall back quickly or slowly?

    I think it is important you get some Ketostix as I mentioned above if she is lethargic to test for ketones.

    You said you leave the testing of the blood glucose to the vet. Would you consider thinking about home testing? It is probably one of the biggest things you can do now to help your kitty.

    I am also concerned that the dose of insulin has been halved but without testing the BG levels I can’t advise what to do.

    The vet cut the insulin because she said she was not eating as much food she wanted to eat again so I gave her some more food I have the s ketostix strips I will test her urine the next times she urinates she had fluids yesterday and drank a lot os water last night just not today she looks much better since she ate the food with the broth should I just feed her every time she wants to eat is that okay she seems hungry she will walk in the kitchen to let me know I have fed her twice because she wanted to eat thank u for helping me

    I am going to learn to test her I have found a petsitter that is a vet tech and I am going to have her show me how is there any thing else I should do thx

    That is great you have the Ketostix. Testing for the ketones and hopefullly getting a negative result will eliminate one problem.
    I would test daily while she has the pancreatitis and is not eating normally and has a reduced dose of insulin.

    I am so glad you are going to test the blood slucose levels. It will be so much better for your kitty than having to go to the vets for a curve. Most cats don't like vet visits....and it will be cheaper.
    I would buy a human meter. The test strips for the human meters are much cheaper than the strips for the pet meters. If you are in the US Walmart has a ReliOn meter which is suitable.
    While you are waiting to get started with the testing, pick a spot where you think you might think is suitable to do the testing.
    Always use the same spot.
    Take your kitty there and rub his ears then give him a treat. This will get him used to it and he will learn to associate it with a treat..

    I will my concern now is even though she ate well earlier went I got up to give her pain medication she ate very little is just sitting on the rug in that meatloaf position she has not slept for days she urinated twice very well I can tell because she did this on a puppy pad and seems a little constipated she seems to be trying to sleep now but at what point does she need go back to the vet or is it better just to try to treat this at home thank u for your help

    Have you managed to get a ketone test yet?

    If her pain meds were due she may feel better once they kick in and might eat then.

    I think as long as she doesn't have ketones in the urine, is eating some food and is not dehydrated she should be ok at home.
    If she gets more lethargic or goes backwards, is not eating, gets ketones in the urine or gets dehydrated I would go back to the vet.

    Can you tell us her name please? And a photo of her in the avatar would be grea.

    I am sorry her name is Shelby I will try to figure how to post a photo tomorrow I am not good at that sort of thing I was so tired when I woke up to give her the pain meds I forgot to test and I would rather use a fresh urine sample I am exhausted because I am worried about her I just wish she would fall asleep she has not been drinking water but she is getting the liquids in the broth but her nose is very dry she is back to her meatloaf position on the rug she just seems uncomfortable when she try’s to lay down thank u again

    Shelby is a lovely name. I'm looking forward to seeing a photo of her.
    I'm sure you are exhausted. It is such a worry when they are sick and can't tell us how they feel.
    Sometimes they need more than one lot of sub Q fluids and pain meds at the vet to get over the pancreatitis.
    I hope she is feeling a bit better in the morning.

    so Do u think it is better to to try to treat this at home or take her to the vet for treatment she is sleeping now the sitter I have is a vet tech maybe she can come over and give her some fluids I am worried about the stress she will have at the vet thank u

    I can't see Shelby so I can't really tell you what you should do.... you know her best.
    But I agree a lot of cats get very stressed going to the vet. But every cat is different

    I always managed the pancreatitis at home after the initial visit to the vet for fluids, pain killers and antinausea meds. A lot of us also keep those meds at home in case of a flare up.
    Certainly a lot of us give the SubQ fluids at home ourselves but you would need to get a clearance from the vet first time, just to check that there is no reason why she shouldn't have SubQ fluids. Some cats with heart conditions can't tolerate extra fluids.

    I am glad she is sleeping now.
    The things you need to look out for is dehydration and I mentioned above how to look for that;
    Also to make sure she is eating
    And testing for the ketones
    And giving the insulin.
    She may take several days to get over this.
    As long as she is not getting worse and you are monitoring all these things, she should be able to stay home.
    But if she seems worse I would go back to the vet or at least ring him.
    Try and get some sleep if you can

    I'm picking up FA from the hospital in a couple hours after 3 days of hospitalization. He was DKA for the second time in just 3 months due to a pancreatitis flare up. Talked to the internal med vet over the phone who he has seen before and asked about how to try to manage the pancreatitis better in the future and he didn't mention anything about administering SubQ fluids at home. He really didn't say much other than remain consistent with the insulin and adjust dose as necessary as pancreatitis flare ups can be caused by many different things. Not only do we want FA to remain healthy but we cannot be spending thousands of dollars every few months for hospitalization. This past incident seemed to occur very quickly, he was fine in the morning when we left for work and when we got home he was clearly sick - vomiting and only eating a very little. Any advice on how we can better manage in the future?

    I'm so sorry to hear that you're going through this, we went through something like this on Thursday last week and previously went through this in August as well. Shelby's symptoms sound very similar to FA's - we originally thought he ate something funny but it really was a pancreatitis flare up that caused DKA. It happened very quickly and we ended up hospitalizing him for 3 days both times. The mistake we made the first time was that we brought him to our regular vet 3 times before we ended up hospitalizing, this time we took him straight to the ER so it saved a considerable amount of time and money. I have never managed pancreatitis at home myself yet but hope to be able to do so in the future. Please keep up updated on how she is doing!

    I am at the ER vet right now she was worse this morning I think my vet should have sent her there right off I am so scared I do not know if she is going to make it

    Thinking of you! It is definitely a learning experience but you are doing the right thing - she will be in the best hands at the ER as they have all the resources that she needs right now.

    I'm just catching up. So sorry you are going through this. Shelby is where she should be right now. Give it a day or two to see consistent improvement. It will be up and down and touch and go for awhile. It's a roller coaster. YOU need to take care of yourself also. Keep resting when you can. When Shelby comes home it will be stressful for you. A lot of us have and/or continue to battle p'titis. When it gets closer to coming hone, check-in here, we can offer tips of what you might need at home so you can ask for supplies from vet. Good luck, keep us posted.

    She is at a good pleace the girl that used to work,at my vets office is there a and she bonded with her she is helping out with her there I just do not understand why my vet did not send me there to begin with They said she has fluid in her abdomen because of the inflammation she is in ICU and they will know more later she has had no vomiting and was trying to eat the pain meds the vet gave me wasnnot working do u think she has a chance of making it I did everything I can

    I am so sorry to hear Shelby is in the ER. It is the best place for her if her was getting worse.
    Poor little thing must have been in pain if the pain meds were not working.

    I hope so but I don’t really know her condition as I am not there. You need to be guided by the vets.
    We had a kitty here a few months ago who was extremely ill from pancreatitis and he pulled through and is fine now.

    If she is showing interest in food that is good. What do the vets say?
    Are you allowed to visit?

    Hopefully they will do blood tests.
    Vets are often pessimistic with prognosis I have found, reading posts on the forum.
    I would just keep her getting treatment and hopefully she will be ok.
    If they suggest something you are not comfortable with, post here to get some feedback.
    Really positive she wants to eat.

    I suppose they are thinking that because there is fluid in the abdominal cavity.
    Did they do an ultrasound?

    Blood tests should tell if it is an abscess because the white cells would be high.
    Do they have her on a drip and giving her pain meds and antinausea meds still?

    I think all you can do at this point is wait for the results of the fluid drawn from her abdomen.
    I know you must be very worried. :bighug:

    The vet called there is a area on her abdomen they have to draw out fluid it could be cancer a abscess or a cyst they gave to draw fluids from her abdomen they will let me know please share any thoughts it’s going to be a long night for me this

    Me to I just think it may be cancer And I am going to lose her I have worked so hard to keep her and she was headed for remission how can she be fine on Thursday morning and get this sick starting Thursday evening has anyone experienced this please let me know I a trying to prepare myself any information would help thank u

    I just want to thank everyone for praying for shelby I got good news the fluid in her abdomen showed no cancer cells they r going to do periodic scans to keep a eye on the area of her pancreas she is eating well getting perky and the fluid they feel is from the inflammation from her pancreas I really appreciate all your help god bless u I will keep u updated on her progress I will pick her up tomorrow night

    We are all glad to hear she’s doing better and they see no sign of cancer.

    A someone said be prepared for ups and downs last night there were setbacks she developed a fever of 106 now down to103.6 her belly is still swollen and painful they changed pain meds because that could be causing the fever her blood sugar when last checked was 70 they think she has neurological problems because she was wobbly and her head was to one side or it could be low blood sugar they are testing her for a bladder infection because of the fever on the positive side eating well no vomiting or diarrhea give me your thoughts please

    So glad there was no cancer.!!
    I’m sorry she has had a temperature.
    If her BGs were low that could cause wobbling.
    I’m pleased they are checking the urine for infection.
    Eating and no vomiting or diarrhoea... great.
    I still have no internet coverage but am out of my area at the moment so have it temporarily.
    I’ll look back in when I can.

    Thx they tried to reduce and give her a different pain medication and had to go back to the stronger type they said the fever could be caused by the inflammation I just wish they could stabilized so I could take her home thx for checking it really helps I am Going to see her tomorrow it’s just another long night

    Ya why do they suggest that? Something needs to be addressed because the vets at the ER here told me to reduce by a whole unit that time she was shooting low and having just been released from having DKA told me to feed her only twice a day- and said it in a manner like I shouldn’t feed her anymore than that... what is going on with diabetes in the vet community? Why are they so ill informed. My client who is a human doctor told me that sounded odd based on their size and low dosing it seems common sense you wouldn’t reduce the whole units. (Seems more like a math problem they need to figure out) and human diabetics are instructed to eat every few hours, to help further prevent DKA happening in them! It’s making me sick at the thought of not being able to fully trust my vet on anything anymore.

    my vet thought my cat had a stroke the second day she was incarcerated (probably what it feels like to them) for DKA and it turns out she was just starving, and her potassium was really low... why are they so ill informed on this disease!?

    Shelby is doing much better her tempature is normal eating good and the swelling in her belly is going down and is active I have not posted for a awhile because she was not improving I should be able to take her home tomorrow I will keep u updated I still exhausted by this when I saw her on thanksgiving she did not look great but yesterday was better thx u for asking about her

    I am so pleased she is improving. That is wonderful news. I’m sure you have been exhausted from all the stress and worry.

    Thank I for caring I am well aware she could still have cancer it could always show up in the future scans but I am hopeful I can take her home and she will get better and I am exhausted

    I am picking up Shelby today I know I will need your help,they have put her on the expensive cat diabetic food and I want to get her back to fancy feast she was doing better on that and the cost was affordable I had her on another prescription cat food for months and her numbers were terrible in the past few months fancy feast made a huge difference I will up date u and when I get her home thx for helping me

    Hi again, they Do that the vets, prescribes the m/d with lower carbs to diabetics. It's because they are 'under the spell from Hill's food company into to all their things'.
    Here in Sweden I couldn't find anything so low carb first and Simba got to eat the wet food m/d, but oboy Simba hates liver and what was that m/d wet food only made of .... liver. So Simba refused to eat it anyway at first.

    Of course you can switch back back at home. But perhaps gently transition with what she has gotten to eat at hospital mixed into her normal at home food, so not to upset the stomach too fast.

    Thank u but today has been a nightmare they released here to early she is in no better shape than when I brought in legathric in meatloaf position not wanting to to eat I have been watching this all day and they prescribed her antibiotics that I told them makes her sick and they all gave me medications in pill form that is hard to give I panicked and took her back tonight and was told I would have to wait three hours and the nurse was nasty acting like I do not know my own cat so I left screw them so here I am watching her look miserable not knowing what to do and they also are trying to push that terrible prescription food to her and cutting her portions so small I just put chicken broth on it and she ate more please help I do not know what to do thank u for your help

    Oh I am sorry. I am across the Atlantic so to far to see Shelby for myself.

    What Shelby is suffering from is a very painful thing in her body.
    They are our babies so your panic and reactions is normal.

    I have a few questions.


    1. What is the antibiotics name and what is it prescribed for?

    2. What other medication pills is it you mention, what is each name and what is each prescribed for?


    3. What frozen or fresh meats for yourself do you have at home? Do you have fresh eggs at home?
    Try as much as possible different food things until she seems to find something she eats.


    4. But it's all the different medications I want their name on and prescribed for I want to know first.

    They never gave me the results of their urine culture though I Asked A million times and I told the vet the antibiotic she prescribed made her sick in the past and she she still prescribed it I will give the names in a few seconds they cut back her food dramatically and changed her feeding schedule when I came back tonight I gave her some wet and put not fat low sodium chicken broth on it she really it like and ate a lot more I let her eat as much as she wanted then I put her on the couch she crawled on my soft sweater jacket and fell asleep I hate to separate my boy cat Morty from her because he keeps bothering . The medications are clavamox antibiotic she said it brought her fever down 5 mg tablets 1.5 per day and buprenorphine 0.6 liquid 1ml every 8 to 12 hours for pain and maropitant citrate 16mg 1/2 tablet by mouth that is for nausea let me know what u think thank u

    I know you are but she is so happy just to be home. Trust me. A week is a long time in hospital for our cats, they don't like to be there. So they say at home is the best healing place.

    1. Antibiotic Clavamox 5 mg tablets 1.5per day

    Clavamox is a broad spectrum Amoxillin and there is actually an Oral liquid suspension available the vet could have prescribed if she doesn't want the tablets, https://www.chewy.com/clavamox-amoxicillin-clavulanate/dp/173058

    But my cats have had problems with that too and here it is known as Vetrimoxin.

    So we have used the better antibiotic Flagyl which is Metronidazol and comes in both pills and as Oral suspension liquid.

    2. Pain buprenorphine 0.6 liquid 1ml every 8 to 12 hours

    It's an narcotic opiodic more widely used in U.S than in much more conservative non-use in Sweden, and while it might give pain relief, you either fall asleep and or becomes lethargic.

    It only used as pain relief but has no anti-inflammatory aspects. Cats are in general highly sensitive to all anti-inflammatoric. My Animal Hospital did use the only anti-inflammatoric cats can have, they get bleeding stomach ulcer from it, only once on Simba's first real severe severe Pancreatitis, but then only in one small dose after two weeks to see if the inflammation would start budge, and that one is called Tolfedine https://www.drugs.com/vet/tolfedine-6-mg-tablets-can.html


    3. Anti nausea maropitantcitrate 16mg 1/2 tablet by mouth for nausea

    We have to start somewhere with these 3 medications her vets there have sent her home with.
    And let's other get the chance of input first, but I understand you strongly want to get her antibiotic changed to one way better for her.

    She did sleep thr the night and ate a little this morning I am switching her back to the regular vets compounded medication now the prolbem is what do I feed her the vets want the prescription food hills m/d glucose support or the pro plan dm can I go back to the classic chicken fancy feast pate when I put her on that her numbers were good and heading toward a possible remission could the fancy feast caused the pancreatitis when I had her on the royal canine glycol balance her numbers were a mess and just kept getting higher any advice would help thank u

    Try to let her eat her classic chicken fancy feast today.

    It wasn't that food. It is just simply the fact that many of our diabetics are more pancreatitis prone than others.

    Thank u I am putting her back on that she liked it and her numbers are good she is back on her compounded medication and sleeping the pain killer in the compounded medication is stronger I am trying to but her back on her regular feeding schedule and will contact my regular vet about finding a new specialist for her scans she is actually laying down not sitting up she is comfortable on my sweater jacket on the couch for first time I have seen her stretched out and looking peacefulI have not seen her like that in a long time she is a little wobbly when she walks but that could be from the pain killers her appetite is not great but I hope it builds up thank u and please any more info is greatly appreciated

    well it has been up and down the fluid from abdomen showed no cancer cells but there is a area that could be a cyst, abscess or cancer that will do another scan in a few weeks. She was admitted to the hospital on Monday and was released Saturday and came home not in good shape I think when they took her off the pain meds they gave her at the hospital and what they gave her to take home was not enough and she was is in terrible shape. They told me that while she was there she was doing great and only needed pain medication so I took her back and it was awful the way they treated me and I left. I contacted her regular vet and she said to put her back on the stronger, pain medication and seemed to help today she is not to hungry but she sleeping and even jumped on the couch she likes my soft jacket and she sleeps on that on the couch. She was not drinking so I gave her the fat free low sodium broth and she is drinking a lot of that she urinated some and even had a bowel movement she is also on nausea meds the emergency vet put her a antibiotic that I told her made her sick but gave to her it to her anyway they said it brought her fever down because the second night it went up to 106 it was normal when they released her. They tested her for a bladder infection but the results have not came back yet her belly is still swollen and painful and she is having trouble walking she can walk some and then her feet they slide out and she has to lay down. They said to only give her the prescription food but I will give her fancy feast if that is she wants is there anything else I should be doing I really appreciate u checking have in support helps I am exhausted by all this please let me know if there is anything I can do thank u

    u guys r helping me so much now I am worried about her legs she is very wobbly when she walks and she has to lay down her feet just seems to slide out from under her I texted my vet but she ate better tonight and insisted on jumping on the couch it was a struggle but she loves to sleep on that sweater and she is sleeping peacefully maybe it is the pain medication or just the pain in her belly I just hope she gets better thank u

    The sliding out with her feet sounds like diabetic neuropathy which can be treated with Zobaline which is methyl b12 tablets you can buy from lifelink.
    A lot of diabetic cats get it. It improves when the blood sugar levels come down to normal range and with the help of methyl B12 tablets. Vets do not carry methyl B12 tablets.
    How much insulin are you giving now?

    The vet said it may be the pain meds her blood sugar was good at the hospital she was given two units twice a day until she got sick then they reduced it to one unit twice a day because of reduced food intake they tried to go back to two units twice a day while she was at the hospital but her numbers dropped and now since she has been eating less she has been staying on one unit twice a day I appreciate any info what u posted sounds like a good idea do u have any idea how long the pancreatitis will last thank u for all your help

    I have to go to a doctor tomorrow and have an ECG of my heart, and I need to recouperate a bit after that, but then I can give as much info as possible from my Simba's first very severe severe pancreatitis in August 2007 I think it was, he was in ER intensive care hospital for two entire long weeks, then we continued with following it with fPLI tests and Ultrasounds all in to November 27th 2007. His pancrea who should only be millimeter thick was so severely inflammaed to entire 1,5 cm thick, and what the inflammation did after it had become better was leaving a lot of scaring in the pancrea which was clearly seen on the Ultrasounds.

    Thank u for all your help I hope u have a speedy recover and feel well soon Today was a better day she was awake when I got up and actually was walking much better I am stretching out the time between pain meds She actually walked in the kitchen and meowed for food she would have to sit down periodically and she wanted to eat it was 7:00am and insulin was not to be be given until 8:00am I still fed her and she ate well and she drank a lot of broth she had urinated twice during the night and had a bowel movement I did what my regular vet said and took her off the antibotics that was making her sick she sat on the rug for awhile and then she jumped on the couch to sleep on her sweater she also is still on her nausea meds I hope this means she is improving i am going to see if she will eat lunch at 12:00pm again thx for caring I am exhausted and hopefully I can get some rest

    Areyou testing the blood sugars at all? Or have you thought about doing it?

    Sorry I have not went to the link yet I am exhausted and having medical issues she seemed to be doing better but yesterday I tried to push her pain meds form six hours to eight hours now she seems worse her right belly is extremely tender and she will not eat much it will be two weeks tomorrow I am thinking I might have to take her back the emergency vet today she seems miserable any advice will help thank u

    I am sorry Shelby seems worse.
    Maybe taking her off the antibiotics was not a good idea.
    If she does not improve I would take her back to the vet.

    I just came home she is much better up and moving around from now on I will keep the pain killers every six hours now I have to deal with a accessed tooth it never ends they do want me to get back to feeding her three times a day to regulate her blood sugar which I will do she actually seems hungry and again I will go to that link I just need to get everything together thx

    Feeding just three times a day will not regulate the blood sugars. I don’t know where vets get these ideas. Feed her whenever she wants except for the 2 hours before Preshot when we want a Preshot BG that isn’t food influenced. She needs to eat.

    I am in the waiting room at the emergency vet she was not doing better she was growling if I touched her it’s the middle of the night I just can not have her in this much pain it has been going on for two weeks today thank u for responding

    Well now she has something under her tail that is not a abscess but looks like a wound I am okay I do not want her to suffer but I had to bring her back she has lot two bounds in two weeks that’s a lot her blood sugar was normal I am waiting on the blood work now i beginning to wonder if I am making her suffer because I do not want to let her go I appreciate u responding it is best to talk to someone who has been thru it I am well aware that when I bring her here she may not go home I will keep u posted any response is appreciated makes it easier to cope thx

    Her blood work was good white cell count elevated but that could be because of the wound under her tail and everything else was okay she has lost tompounds in two weeks now I am waiting for them to do the ultrasound scan thx

    There r these lesions breaking out all over body they were not there when she was there the last time so also weird showing on ultrasound of the fibers that hold your abdomen together they r going to do a needle biopsy and clean the lesions and biopsy those and put a feeding tube in her neck it could be cancer or some strange infection this is agonizing I appreciate u checking in thank u

    A high white cell count does seem to indicate an infection.
    Oh dear, I am not surprised you are exhausted. It is such a worry when they are sick.
    I hope they can find the cause of the problem and it is fixable.

    Just catching up here. You are certainly in a roller coaster. Feeding tube sounds like a good idea. I was going to say what you saw under the tail could be anal glands that ruptured. That can cause infection. But then you mentioned lesions, is it possible reaction to antibiotic? Keep up the pain med for p'titis, it can be long term, if every 8 hrs doesn't work try every 6 hrs. But keep it up. If vet insists on clavamox get the suspension and take the total amount they want you to give in 24 hrs and divide by 3 and give every 8 hrs (you don't have to tell them). That worked for Ollie, she didn't foam up or vomit it back that way.

    I'm sure everything will change, but just some ideas to consider. Prayers for Shelby and you.

    It is the hardest decision to make to help them cross, but it is the greatest gift you can give her.
