7/23 Kozmo, AMPS 88,

    Kozmo update. We’ve struggled with pain management over the few days, and thanks to the FDMB spreadsheet (and my notes on it) we realised that the Gabapentin wears off after 8 hours and by +9 he’d be climbing the walls in pain and completely lost his appetite before pre-shot. He would gain it again at +1 (after the gaba kicked in).

    We tried increasing his dose to 50mg in the hope it had a longer tail but it too wore off at +8. So we’ve experimented with 25mg every 8 hours (3/day) starting two hours before AMPS and this is working (touch wood). It ties me to the house at AM+6 every day but we can’t see any way around this. Neither of us want to see Kozzie in such pain again.

    He was like this for two years before our previous vet prescribed Gabapentin for neuropathy due to OA, and I’m furious that (a) I was ignored for so long, and (b) that Kozmo had to endure such excruciating pain for so long.

    On a happier note, oh what a changed boy we have! It’s six weeks today that he was diagnosed (first of two DKA’s) and look how far he’s come! I’ve been living hour to hour feeling sick to my stomach that he’s either going to DKA or hypo any moment, and now with his lovely green numbers, he’s flourishing and I at long last have started to enjoy it.

    We’ve got a long way to go, but we live in hope that he’s one of the lucky kitties that slip into remission.

    Does Kozmo have to give me a heart attack by throwing a <50 before the dose can be decreased? :nailbiting:

    How long shall I leave Kozzie’s post DKA in the subject line above?

    He’s doing really well with his self feeding. I’ve just now worked out a way to leave his food out during the day without our little dog getting to it - he loves eating from his auto-feeder on a kitchen chair! But, being a cat he loves it today but not necessarily tomorrow.

    Again, thanks to the FDMB and the indefatigable dedication, kindness and generosity of our GA’s, we’ve been able to come so far and will undoubtably be in constant need, but for today (hopefully) we’ll leave you in peace to help others!

    We’ll keep testing and updating our SS.

    Isn't it wonderful the progress they make once getting the right amount of insulin? I've read of these transformations time and time again...enjoy your perky Kozmo!! Have a wonderful day!

    Oh, poor Kozzie! It just breaks my heart hearing about what he's been dealing with :(. I'm glad you've found a pain management solution, but wow, that is a rough schedule. I wonder if there are any time-release formulations of the gaba?

    At least on the FD front, he is looking a-maaaaaazing!!!!!!

    Oh, and I think you can take your post-DKA note out of the title. He's a month out now and doing great!

    I'm sorry Kozzie has had so much pain but it's so good you have worked out a routine to manage that pain.
    I think you can take down the DKA in the subject line now. We just need it in the signature.

    And yes you do need to drop under 50 to get a reduction.:D
    The dear boy has come such a long way in the last month. I'm very proud of him.

    Kozmo hasn’t had a good night at all. I was cradling him in my arms and felt short bursts of buzzing in his pelvis 1.5 hours after his gaba (he was asleep). Fairly sure that was his neuropathy firing off. PM+4 to +8 It’s like he didn’t have his tablet but I saw it go down. Meowing, pacing, trying to get outside, frequent visits to litter, twitching, chewing his legs. It’s like the pain gets worse as his BG numbers get better.

    That is so tough to see... I hope you can find something that manages the pain consistently.
