QUESTION 11/13/2020 Blitzey

    Good Afternoon,
    I am switching from prozinc to Lantus in the am.. He is currently on 5 units of prozinc.. So i would start with 5 units of Lantus?

    Hi! Welcome to you and Blitzey!

    I don't know about the dosing from prozinc to lantus but I'll tag @Wendy&Neko to see if she can help give some guidance.

    You may know this already, but I believe you need to make sure you have u-100 syringes for lantus versus the u-40 for prozinc.

    Also, when you have time, take a peak at the sticky's at the top of the board - they are all helpful but you may want to read through the dosing method one first. I've linked it here.

    And, here's your last post in the prozinc forum in case others want to read through.

    welcome to the l&l board and good luck w the switch.
    we did the switch for kit in june and she seems to like it much better.
    we also took the timing as an opportunity to switch to a human meter. looks like you're using pet meter?

    in addition to susanne's tips and tag above, you might want to make your post title text more explicit and add the "?" prefix.
    so maybe something like

    [? prefix] 11/13/2020 Blitzey tomorrow starting dose for switch prozinc to lantus?

    Hello and welcome. Yes you would start on 5 units of Lantus - though honestly, with that size of dose, Levemir would be a better choice. Lantus has an acid base, which can sting at higher doses for many cats, and I rather suspect Blitzey will end up needing more than 5 units. However, let's see how it goes and hope Blitzey is fine with the Lantus.

    Can you tell me a little bit about Blitzey's history, how he was diagnosed, any other medical issues? Good to see you are on top of his ketone testing with him in those high numbers.

    Yes you can do a straight swap from 5U prozinc to 5U lantus, there’s nothing in your spreadsheet to indicate a possibility that the dose of prozinc is too high and with trace ketones recently I wouldn’t want to be conservative in the change. As Susanne mentioned it’s very important that you also change your syringes. If there is no dry food being fed I would strongly recommend you use TR for dosing so you can get Blitzey into better numbers asap.

    Would you please add some extra information to your signature for us - the date of diagnosis, the type of insulin (you can put prozinc x date-x date then lantus) the type of meter used, the type of food being fed (e.g. wet LC, dry or combo) any other meds or medical issues and trace ketones with the date. We will also want you to add the method of dosing (SLGS or TR) but if you haven’t decided yet it’s fine to leave that out for now. It’s nice if you start with your and Blitzeys names too.

    This is information that will help us help you so it’s better if it’s all there handy and people don’t have to ask you the same questions over and over, especially if you need advice in a hurry.

    He lost a lot of weight within about 3 weeks, took to the vet, diabetes, fatty liver, pancreatitis and IBD on ultrasound. I tried to get Levemir but the vet wouldn't write a script, just said I should try Lantus.. His bloodwork shows his liver has greatly improved and he is no longer jaundice and has also gained 2 lbs.

    Thanks for the update on the signature. Is he on any medications for any of those conditions? Some medications can contribute to needing higher doses.

    Is the Dr. Elsey's dry or wet?

    Dr Elsey's dry, He eats very little, he is on cerenia and sub q fluids as needed, just came off antibiotics(orbax)

    Should I just stay with prozinc until I could get levemir?

    You can always try Lantus and see how it goes. If he flinches or walks away from the Lantus, then he's one of those who feel the sting. We do have a couple kitties here on higher doses of Lantus. Check out Oberon and Moe's posts for two examples.

    Thanks for the food update. Ideally with a kitty on high numbers, we can fast track increases to get him into better numbers faster. However, that requires using Tight Regulation as a dosing method, and that means all low carb wet or raw.

    I called the vet today as I had a IGF test and pancreatitis test done and he was positive for pancreatitis which i knew he would be and his IGF was 336, so possibly acromegaly, she is going to get with the internist on Monday to discuss it and I am hoping she will give me Levmeir as if he is ACRO his insulin needs will go up? I am afraid if it stings him it will be hard to give shots, being in the military i am away and I don't want to make it hard for my mom to give insulin, He hides during the day when it is time for her to get a blood sample..

    Anything over 92 is positive for acromegaly. Sorry for the results. :bighug: It's hard to predict what acros will do regarding dose. We've seen them on as little as 4 units (I saw one study that said they saw 1 unit as the low) and as high as "you don't want to know". Well into double digits for sure. Levemir would definitely be a better choice and I'd look to buy in from Marks in Canada for price savings. There is someone on the Supply Closet forum who sells it regularly too.

    With a diagnosis of acromegaly, you might want to think about treatments. Most of the treatments (surgery and radiation therapy and some medications) are quite expensive, but there is a drug called cabergoline that is a reasonable monthly cost and has helped a number of kitties. With them it has both lowered the dose, and abated some of the acro symptoms.
