You would need to use syringes to dose fractional units. A pen can only dose in whole units. And yes, 4.25 would be midway between 4 and 4.5u. I don't think so. See what the others say.
I dint think the oil is raising the bg. It would be easier to dose if you used digital calipers and your syringes.
Some people give olive oil for hair balls. It's pretty much pure fat, no carbs. Time for that increase unless you see greens today. This will be six cycles after the reduced dose for his ultrasound.

Hopefully, yes. And he started to eat smaller meal (while eating @meal, he looked like even though he was eating a lot, didn’t feel full enough. But from day before yesterday, he started to leave some foods.) He’s been eating almost 380 kcal. Which is bit too much for 15lb cat
Have you been weighing him periodically to see how is weight is trending? Neko was 14 - 14.9 lbs - I told her she couldn't go up to 15. About 220 calories was a good amount for her.
I don’t have any pet weight scale yet. But he was 13.9 to 14.2 on the first day seeing new vet 4/9 and recent when he had ultrasound, his weight was 14.9. When he gained weight from 13.9 to 14.2 he still had that excessive hunger (from my guess is that insulin wasn’t working right)
