? 1/30 Bugs, AMPS 191, New to Group

    Hi,

    Our girl Bugs is currently on Prozinc and we are ready to make the change to Semglee. We have been part of the Prozinc forum since last Fall and Bugs has improved by leaps and bounds. Below are the links to our prior threads.

    Bugs has been on a drop dose as needed since 1/23/2023 with a NS BG of 170. Prior to that we were doing a OTJ trial. (Her spreadsheet is current)

    1. Based off Bugs BG levels, what would you recommend for the dose when we start Semglee?

    We are anxious to get started on the Semglee and hoping this will be the last push to get Bugs into remission.

    Any recommendations you have are welcome. Thank!!

    Hi Sharon & Bugs, welcome to the LBL forum :)

    I think you can start Semglee at a dose of 0.25U but you want to wait for confirmation about the starting dose from more experienced members I'm tagging:

    Would you be okay to switch to a human meter? Our dosing methods are written for a human meter. Not that it's mandatory though.
    Dosing Methods: Start Low, Go Slow (SLGS) & Tight Regulation (TR)

    And if you don't mind, could you please change the "D"s in your spreadsheet to "drop"? It's probably my eyes, but it's hard to tell the 0 and D apart in the Unit column!

    Thank you for the welcome. I changed the D to drop.

    We've thought about switching to a human meter but have been using the pet meter since we started this journey. What human meter would you recommend? How would that impact our current spreadsheet as we are using the pet meter version?

    I don't think I'd start as high as 0.25 units, maybe 0.1 unit. We consider how kitty is doing on the current insulin and dose when considering the switch. Our goal is to see more upper greens, lower blues, even on the pet meter. Side note, it is easier to figure out how he's doing with our dosing methods if you use the human meter, but we can figure it out. Back to the current numbers, even a drop dose does not take him into the range we are looking for, hence the suggestion to start with 0.1 units. He may need to go up in dose again, but time will tell. You do have to wait for the Lantus depot to build.

    Do you have the U-100 syringes already? If you want to use a human meter, a lot of people in the US use the Relion brands from Walmart. If you want to use a CGM, the Libre is what is commonly used for kitties here.

    We can help you fix the spreadsheet quite easily if you do decide to switch. Basically there will be a line where above shows the pet meter and below the human meter.

    Do you have a Walmart by you if not you can order the meter and test strips on line
    You can pick everything up from Walmart The Relion Premier Classic meter and test strips

    At least if you are running low on test strips you can run into Walmart and pick them up

    Like Wendy said the SS is easy to fix , and Bhooma can help you with that if you decide to switch to a human , she would also fix the the last 2 color codes up top and ADC whets you have your current meter listed in your signature and on your spreadsheet that you have switched to a human meter .

    We'll make the switch to a human meter and I'll let you know as soon as we have one. Thanks for the suggestions on brands. Having easier access to test strips will be a huge plus.
    Once we have the new meter I'll definitely take you up on your offer to assist with Bug's spreadsheet.

    I did purchase the U100 syringes.

    1. Do we still do the Semglee 2x daily at the 0.1 unit dosage?
    2. Is there a NS number we should use with the Semglee?
    3. What is an estimated time frame for the new insulin to build? (I understand all cats are different)

    Yes, Semglee is dosed 2 times a day.

    The NS number depends on which dosing method you'd like to follow, the Tight Regulation Protocol, or the Start Low Go Slow dosing method.

    New insulin takes 5-7 days for the depot to build. Probably on the shorter end for smaller doses.

    When you switch to Lantus then, just change the insulin name and add SLGS to your signature. That'll help us help you faster. People with data and experience following SLGS do not shoot below 90. In the beginning, you might choose a higher NS number. You aren't new to feline diabetes, and looks like you have no problem getting a good number of tests in, so it doesn't need to be as higher a NS as someone new to testing.

    Yes. I read through the thread. But I didn't see a line on your spreadsheet indicating the change to Lantus. I'm talking about a horizontal line going across that says something like "Started Semglee" If you look at my spreadsheet, you will see when we switched from Lantus to Levemir as an example. It just makes it really obvious that you've done the switch and exactly when it occurred.
