Darkside was on Lantus 3ml. twice a day for a month. He was taken off dry food and stayed with his wet food. He was reduced to 2ml 2x for 2 days then 1ml 2x for 3 days. With his change of diet his BG went down. He was devouring the wet food. When he was put on 1ml he went downhill fast. I had him overnight at the vet but no urine sample she could get. She gave him vitamin shot and an appetite stimulant. He's barely eating or drinking. He has gone both ways in the litter box. He's back to 2ml 2x but no difference. What can I do?

Have you taken a ketone test or has the vet? If not, I would do so. Has the vet considered pancreatitis?

Brought home a bowl for collection. She gave him iv fluids, appetite stimulant and a painkiller. She thinks his pancreas is inflamed. Hopefully this regiment will get him back to normal. Thanks for your suggestions.

Welcome to the FDMB, the best place you never wanted to be.
Insulin doses are measured in units. A U-100 insulin such as Lantus has 100 units per milliliter (mL). If you were giving 1 mL, you'd be giving 100 units which would be an extremely high dose. I suspect you are now giving 1 unit, right?

If he has pancreatitis he probably doesn't feel like eating. My cat goes off her food when she has a flare up of pancreatitis. IV fluids, painkillers, anti nausea meds more than appetite stimulants because cats won't eat if they are nauseated. Try and encourage small amounts of food and fluids. Ask for anti nausea meds.

Has the vet prescribed anything for the pain? I have had cats go through something similar and the vet prescribed budesonide. It worked for both of them.

Update: IV fluids and Cerenia injection. No painkiller yet. Darkside is eating little more food. Still not drinking water. More alert. Now there is another problem. Two times now that he ran out of the litter box dropping his poop outside the box. Both times bloody butt. You can tell it hurts. I wiped. He finally calms down. I looked at the stool and it's pale to light brown no blood in it. It's not real hard but thick. What can I do to soften stool if he won't drink water to help with softening?

Hey there. Try adding some water to his wet food so that he is getting some water since he isn't drinking for you. Also, to help with the constipation add 1 tbs. of pumpkin (pure pumpkin, not the one with spices and sugar in it) Actually the pumpkin helps for constipation and diarrhea. I put it in my cat's food every meal to keep him regular. Hope this helps. Keep us posted.

I was thinking watering down the wet food too. I forgot about pumpkin. Thanks.

Well he turned up his nose to the pumpkin mixture. But he finally came around because he was starving. Added pumpkin and water for third feeding and now he licked the bowl clean. The Cerenia has wore off and no sign of being nauseous. The appetite stimulant, Mirtazapine is in Day 2 of 3. It only last 72 hours. Still not drinking water. Has urinated twice Friday. Been 22 hours since last bowel movement. Otherwise he's alert, purring and giving paw when asked.

Glad he is eating. Keep putting water in his food. Maybe the IV fluids are keeping him from being thirsty, good thing he is urinating. Hopefully the pumpkin will help with the bowel movement. Hopefully, he is on the mend! Fingers and Paws crossed.

UPDATE: Darkside is eating like a piggy. Still not drinking. Bowel movement at 4am. Still running out of the box. No bloody butt this time. Stool wasn't as thick. Hopefully the pumpkin and extra water mixture will work. Thanks everybody!

If the food switch was recent, it might have upset his system. Some cats have a hard time switching from dry to wet.
It sounds like it was recent.
if the cerenia wears off and the vet didn't give you more to give later..... you might ask for ondansetron for nausea. 1/4 of a 4 mg tablet twice a day.

He was on dry and wet food equally. In the pass, I ran out of dry food before and had him on all wet food. No problems at that time. He likes wet food better. As of now, eating is not the problem. He is not nauseous. Keeping food down. He won't drink. Plus his stools need to be softened. Hence the pumpkin mixture.

When my Squallie doesn't seem to be drinking enough I mix a lot of warm water into his food, basically creating a cat food soup. He eats (drinks?) it just fine and, if they are anything at all like us, probably like having a warm soup when they feel under the weather! :)

Update: Darkside is doing quite well. I add pumpkin and warm water to his wet food. Only once he ran out of the litter box in the last 3 weeks. Eating great but hardly touches his water. Going both ways in the litter box, even though he's not drinking the water. I guess he has enough from his "kitty soup". BG numbers when down. He's been holding steady in the 80s the last 2 weeks. The vet took him off of Lantus on November 17th for a trial period. Wish us luck. Here's a picture of Darkside. Update next week. 20150802_010049.jpg

There are a couple of dehydration checks in my signature link Secondary Monitoring Tools. If he isn't dehydrated, you can relax a bit.

That all sounds like great news to me.

Our Whisper *LOVES* his wet food as a soup, but the boy just don't care if it is warm, or straight out of the refrigerator. I haven't ever seen him drink from any of the bowls or fountains around the house. But despite my concern, our vet repeatedly confirms that he is very well hydrated (confirming what I see at home with the "tent "test).

Best wishes for continued luck from me and our kitty six-pack!

Thanks Sandi and Whisper. I feel better know your kitty is like mine. Before the diabetes he drank an appropriate amount of water.
Then he drank water till the bowl was dry. I had a feeling it was diabetes and I was right. Since the bout with pancreatitis he has drank very little but the "kitty soup" must be making up for the water.
I did the tent test and it seems fine plus the urine output is very sufficient.

Darkside is such a handsome boy!

Congratulations on getting him low enough for a trial period, fingers and paws crossed for you that it all works out and he will be OTJ! :):):)

Update: Darkside's urine is negative across the board. BG is 120. Off of Lantus for 9 days. Still has weakness in his legs. I'm starting him on Zobaline today. He is 16 pounds. How much should I give him? Is there any human B12 methylcobalamin that has no sweeteners or rice that I can give him? $30.00 a month is a little pricey.
    Thanks.
