? Dosing advice please - don't know if we are too high or too low

    Hello and happy new year, FDMBers!

    I hope you all had a lovely holiday season and that Santa Paws was good to all your kitties!

    I'd like a spreadsheet review and dose advice, please. Acro-cat Snuggie was doing well at 8u in early December, but his blues were high and he spent quite some time in the yellows. I decided to increase his dose to get into lower blues and see fewer yellows. (NOTE: I'm not striving to get him into lots of greens. The fear of hypo was too much for me, so my plan is to make sure he spends lots of time in blues and low yellows).

    Once i increased his dose, his numbers got worse. We are now up to 12 units. I have seen a few blues in the first few cycles of a dose increase, but I'm seeing far too many pinks and reds.

    He was Dx with a UTI in mid Nov and was on Orbax for 6 weeks. The vet saw the bacteria in the microscope (urine was drawn w sterile needle/cysto). We sent it out for C&S but the lab wasn't able to grow anything. His BGs started to decline shortly after we started the antibiotics, but then they went up again. I will take him back to the vet once the antibiotics are out of his system, but I'm wondering if maybe his dose is too high? I know that's usually not the case with acro cats. I asked for advice in the Lantus/Levemir board but wanted to cross post here because I know the acromegaly complicates all of this, and there's tons of experience on this board.

    He's still on Cabergoline, right? It's possible that his tumor is pulsing lately. Any chance of a bad tooth or some other infection causing higher numbers (I am doubting this, but just asking.)

    I know Wendy will come along to help you out and give her excellent perspective. If it were my kitty, looking at that spreadsheet, I would keep increasing every six cycles unless I saw green or low blues. I'm glad you will be taking him back to the vet for a recheck. I wonder if Orbax is the right antibiotic and has cleared the infection. It's very perplexing. I would be very frustrated! He's going in the wrong direction! How does he seem otherwise? Does he seem better now? Is he constipated at all? I saw where he has had in the past some problems with constipation? Any diet change (other than the HC the other night :) )

    I’ve been thinking about you and Snuggie. I just checked his spreadsheet and see some lovely blue on this 13 units! That’s good. How does he seem to be feeling?

    Sorry for the late reply! He has had a rough week, went back to the vet Wednesday to recheck for a UTI, but they wanted him to come in the next morning so the sample would be fresh for the lab. Thurs we went back and he needed another enema. Thurs night he was in and out of the box, not eating, vomiting, and panting. He wouldn’t let me test him, he was more growl-y than he’s ever been. Called the vet and he wanted him back the next day. Turns out he has a digestive tract infection! He got a shot of convenia and cerenia, and his BG has been plummeting - into the 50s today! So we skipped this am’s shot and are reducing dose. He already seems perkier and is eating again. Yay!

    Wow! How did they go about diagnosing the digestive tract infection? Hope the Convenia works — not too sure about that cephalosporin on digestive tract infections - but I think you are not able to pill Snuggie as I recall. And something good is happening since he is feeling so much better! That makes me REALLY happy!
