12/26 Kloey AMPS 398,

New to this forum, have been on the general forum.
Kloey had been on Vetsulin, went DKA with large ketones, hospitalized 2/21, home 2/23. Was changed to lantus, the hospital placed a freestyle libre.
Don't know what happened last night with the high numbers. I thought maybe a fur shot but I feel like it went in fine.
We are trying to move her schedule up in increments, the hospital got her on a 12/12 schedule.

Looks like she is bouncing from the greens in the morning of 2/23. A bounce can take up to 6 cycles to clear.

Btw, the Glycobalance wet and dry are too high in carbs. Is she still eating that or only FF pate?

Getting back on a 12/12 shot schedule - suggestions for newer caregivers
Glycobalance wet is what they fed her in hospital and she wouldn't eat FF when she came home. So I'm trying to work her back on it. She is not on dry.
She did eat some seafood FF this morning but all the others she just tries to cover. But with the ketones she has to eat. We are going to start looking at different foods cause she is not a huge fan of FF. We did feed her Sheba in the beginning and loved it, but she will not touch it now.
So maybe she is bouncing, I was just sick when I saw that pink last night. Thought she was off the bounce with all those yellows. Thanks for the link, I will read through it.

Yes, with DKA and ketones in the picture, it's important that she eats so you want to feed her what she will eat. Glycobalance canned is 14% carbs. Whenever you make the switch to low carb, just watch for her numbers and insulin requirements going down.

She is due to clear the bounce today. Hopefully she will give you blues and greens again today.

Btw, have you read through our dosing protocols? Since she is on the Libre and 100% canned food, you might want to consider following TR - especially with ketones involved - it will help you get to a good dose faster and keep her at a dose where she can spend more time in normal numbers.

Dosing Methods: Start Low, Go Slow (SLGS) & Tight Regulation (TR)

I wasnt sure I could do TR because I work 1- 2 days a week and I'm gone for right at 10-11 hours. So could not do testing during the day.

I was out to grocery store. She went down to 84 libre, husband intervened with 1 tsp glycobalance, kept going lower to 78 libre. Husband gave 1 tbsp 20 %carb. Finally up to 110. Now going lower again. Just checked BG (not libre)she's 93.

Don’t feed any more unless she drops under 50. She should start to come up on her own. If that doesn’t happen by pmps, stall, DO NOT FEED, and post for dosing advice. If you feed we won’t know if the pmps is food influenced.

If you can test as close to 2 hours after insulin and get an auto feeder you might be able to do TR. WITH SLGS a reduction is earned with a bg under 90. So the dose tonight would be 1.75. Since it hasn’t even been a week on lantus I would take the reduction.

I don't think we could do it. I leave at 515 and don't get home till 6. Husband leaves at 7 and doesn't get home till 630. Couldn't get the evening shot in on time.

Ok, PMPS is 107. Fed 1 tsp glycobalance 1.25 hrs ago. So do I not give or do I stall until she is above what number?

Give her another half hour Don’t feed and let’s see the bg. We don’t want to skip with her having recently been DKA.

Lantus is working for her better than Vetsulin. Let’s see if she starts to come up more. I would like to see that. You will definitely be reducing the dose.

I also am trying to get her back on a much earlier schedule. Er hospital got her on 12 and 12 which does not work for us. If we stall long. I won't be able to get her back before I go back to work.

What time is it now for you? It would be better to not skip because of her so recently DKA. Make sure she eats plenty if you skip. Maybe shoot at 5 or 5:30 a then shoot 30 or 60 minutes late to get to your desired time.

How about checking her at 12 and then decide? She should bounce soon. Or get up and shoot extra early. It’s easier to get back on track when delaying shot time.

I will check at 12. What would be extra early? I can get up whatever time I need to. So it's better to delay a shot time than move up a shot time to get back on track?

Today was her 1st cycle off a bounce from the hospital low. So she will probably bounce again from this low?

It depends on the bg. With being new to lantus and lower than you are used to shooting early can act like a slight increase and you actually want to reduce her dose. Her new dose is 1.75. If she were bouncing and how it would be better to shoot early. Being low shooting early is not a good idea.

She’s on the way up at last but not as high as I would expect. If your shoot you would need to test her at +2-3 and if she drops again it could be a long night and not much sleep. So it’s that or yiu can skip and shoot at 5 or 6 I guess.

I think I will skip and give at 5. It'll keep her safe and my sanity and help get back on track. Thank you for your help. Kloey is going to be the death of me. LOL
So it should be 1.75u at 5am?

I am out of glycobalance and will have to go get some today. Should I just switch fully to FF or get the glycobalance. Yesterday was a mix of both glycobalance and FF. She ate a half can of FF @5am and another half at +6.

I would not switch cold turkey. Is the glycobalance the canned? If so it’s so it’s not high carb like the dry. I believe it’s mc.

Yes it is the canned. Is it ok to give some glycobalance and some FF? Maybe FF @PS and glyco at +2 & +4-5.
