Good evening,

the vet visit was quite concerning. I understood that the vet thinks, that Binie is maybe fighting with multi-resistent germs in her kidneys and bladder. We already found multi-resistent germs in her bladder in january, when she was diagnosed with fd, treatet Binie with antibiotics and I thought all is well.

Hopefully we have results from the lab tomorrow and know more, what is going on and if my vets theory is right. The vet said that the problem might be, that we don't have antibiotics to treat the germs and there are antibiotics Binie can't take because she had severe side-effects in the past. If this is the case, we don't have much to treat her and this could mean, that we reach a point very soon, where we have to talk about euthanasia.

The vet was concerned, that Binie gets a sepsis before we can start an antibiotic treatment. Therefore we discussed steroids. Because Binie is still eating well and had no temperature, she decided we can take the risk to wait what the labs say tomorrow. She gave Binie fluids and novalgin for pain. I think the novalgin really helped. Binie stopped hiding in strange places and wanted to go out on the terrace again.

I feel quite numb right now. I cancelled all my appointments tomorrow and on the weekend to spend time with Binie.

Hopefully tomorrow I will know more and I will give an update.

Oh (((Heike))) I am so very sorry to hear this update. Of course you are feeling numb. It's so concerning that they don't feel they have an antibiotic Binie can take to fight the infection. I'm glad she seems to feel better from the fluids and pain medication.

I am so sorry to hear this. Was the culture sent to a lab? I’m been told in the past steroids should not be used with an infection so do you know what your vet hopes to accomplish? What happened with past antibiotics and which ones? Have you seen another vet for a second opinion? Sending prayers.

Thank you so much for your reply Elise! Yes, the culture was sent to IDEXX. In january they found E-coli and Enterococcus faecalis. She had Efex for 10 days.

In the past Binie took clavaseptine because of a bite wound and she was constantly vomiting from it and very unwell. She also took Cleorobe for treating an infected wound and her skin started itching badly. So these two antibiotcs are already out.

The vet thought about steroids because Binie was very unwell the last three days and she saw the danger of a sepsis. She explained to me, that it controls the inflammation while we wait to find a treatment option. After she saw Binie and I told her that she eats well, she said we can risk to wait and not to give steroids.

No, I haven't seen another vet so far. I didn't expected things to go in this direction. My vet has a cats-only practice and is quite experienced. She had already cases like that in the past. I will wait for the results and see if her suggestions for treatment convince me. If not, I will get a second opinion.

Steroids cause a cat to be more immunocompromised, meaning even more likely to get infections. Not sure it's a good idea. If Binie has a kidney infection, she will need a longer course of antibiotics than for a bladder infection.

I agree. Steroids lower resistance and can cause an existing infection to worsen or leave you vulnerable to infection. What is your vet thinking?

I would ask your vet if a consultation with an internal medicine or infectious disease vet would be a possibility. Your vet has options like some of the veterinary professional message boards where he could pose a question regarding how to approach treatment for your kitty.
