I have a call into UGA and have talked with someone at Blue Pearl. They advise using a specialist as well because of the complicated surgery, diabetes and his age. The consult is $163. The ultrasound $500-600.
Surgery $3000-5000

I know one thing, I don't want my vet doing this surgery if that's what's needed. Who knows anyway if what she saw was even accurate. Annie at Blue Pearl said even if they do a mobile ultrasound they would have to redo it. Why pay for both. The mobile is $405. and they are experienced but not board certified. Elise, you were correct. The vet was deceiving about the qualifications of the mobile unit. I'm going to wait to hear from UGA but I'm still seeing the Internal Medicine is not taking new patients right now due to a lack of personnel in that area.

It's always better to ask, Patty. The worst they can say is 'no' and, as Elise comments, they may be much more responsive for urgent, more complex cases. (Praying for that...) If you don't hear back quickly, maybe chase them up? Squeaky wheels...

I finally reached the animal medical center (my vet) and they have a call into this company for the mobile ultrasound.
Are they still angling to do any surgery required, Patty?

Also, have you said to them that you're looking for a possible referral to a specialist hospital?

@tiffmaxee - When you get a moment, Elise, can you have a gander at the above link and give Patty some feedback please? (I don't have US-specific knowledge so don't know what to look for.)

I have only spoken to the receptionist. She said they are waiting on the mobile ultrasound company to call them to schedule. I indicated to her that I was concerned the diagnosis might require a specialist to
perform the surgery. If that happens they would redo the ultrasound and if have to pay twice.
Just talked with vet. Going to my laptop. BB

She agrees with me since Blue has a complication (diabetes) that I should probably just start with the specialist since they'd have to redo the diagnostic anyway and I'd have to pay twice. She said they do perform that surgery there but he would not have the round the clock care he might need. She advises UGA over Blue Pearl and said price would be better as well. They will call to see if they can take him at UGA.
His stomach is down a lot today. That medicine must have helped. He's feeling good. Amazing!
 
I just heard back and UGA is definitely not taking Internal Medicine patients. I asked about ER and she said they couldn't do it there because they don't have the personnel. Athens has been hit hard by Covid because of all the college students so I'm sure that has something to do with it. I'm disappointed but there's Blue Pearl. At least they can do the diagnostics so we can see what is going on. I trust them more than the mobile unit.
 
This from the mobile vet service website just threw up a MASSIVE red flag for me:

How does working with Atlanta Mobile Veterinary Specialists differ from traditional referral?

Our service differs from referring to an outside specialty hospital in that all communication will be done with the primary veterinarian. We do not offer direct consultations or follow up with clients.


They make a point of saying it twice:

Please note that we do not meet directly with the client. All communication will be with the referring veterinarian, who remains the primary contact on the case.

[Emphasis mine]

If I'm reading this right (and I believe that I am), this means that you don't get to have ANY dialogue with their specialists whatsoever; you would be reliant solely on what your own vet tells you about what they said. Also, in terms of any potential diagnosis the external service might come up with, because they refuse to deal with clients then their diagnosis will only be based on what the referring vet tells them about Blue's history. All the valuable information that could be gleaned from your intimate knowledge of Blue wouldn't be taken into consideration at all.

In situations like this, you need to have direct access to the specialist who will have responsibility for Blue's care. You need to be given full and comprehensive details of the diagnosis and have all your questions answered so that you'll be able to make fully informed decisions.

That service only provides diagnostics, not specialist surgery. This reinforces my worry that your vets are not looking to refer Blue to a specialist for any surgery that may be required. Blue Pearl have already confirmed that they would have to perform their own diagnostics and if your vets are seriously planning to refer Blue to a specialist facility they should know that and should not be recommending additional diagnostics that are of no value to a referral hospital (and which would be an unnecessary additional cost).

I'm really worried for you. You've already told us that they don't have overnight nursing facilities (and it doesn't sound like they can provide much in terms of intensive care). Would they be looking to shuttle Blue back and forth to an ER facility for night time care???

I think the practice should be proactively recommending urgent referral to a specialist hospital, not faffing around with an external diagnsotics-only service whose USP seems to be a promise not to poach their clients.


(@tiffmaxee - As soon as you can, Elise, I'd be grateful if you could have a look at this and tell me whether or not my interpretations are reasonable and whether or not my concerns are valid.)

I still think part of this is the metronadozole plugging him up.

I'm back. Just had to take a break. I'm depressed. Praying for the best but thinking of the worst.

Too bad I can't get in sooner. Only gave me 3 pain pills for him so I guess I'll have to go back. :rolleyes:

Another 45 minutes until I test pmps and I'll post as soon as I do that and we can discuss.
 
Thanks guys. It's been a hard day. Even though I am scheduled to take Blue on Tuesday I worry about him until then.

His BG is still going down. It's 116 pmps.
 
Patty, I just saw the pmps. I’m concerned about you. You won’t be good for Blue if you can’t think clearly. I think you need a night off and should skip. What do you think?

I think the suggestion to skip is a good one. Blue's in good numbers and you desperately need rest. You need to look after you in order to look after Blue.

Thank you both for your help and support. Blue is waiting on food with the rest of my gang.
I think I'll give him a drop of LC along with the others since they're used to a snack at shot time.
