Welcome back to Kona's Crazy Adventures... if we're not stranded on a highway at 26 degrees for 14 hours in one season, we're in the ER for 8 hours on the next episode.

Overall, Kona is stable at this time. She ate this morning and I gave her full shot.

In summary: since Thursday 2/8, Kona hasn't been eating all that well or not at all, has had lots of diarrhea, and very lethargic. She has not been vomiting!

If both come back insignificant, moving to ultrasound if Kona is still not eating on her own or continues to have diarrhea or any issues.

When/if you get a chance, any feedback or second pair of eyes on all this would be greatly appreciated. I'm just looking for anything jumping out that looks odd in all the test results that I should ask my vet when I bring in a fecal sample later today. Last night the ER vet basically said everything looked normal and sent me on my way, but I see a lot of lows and highs on these papers this morning. Normal vet this morning said if things don't go back to normal and this is NOT a transient thing, then she would like to do ultrasounds. If Kona eats fine tonight by being on Cerenia and maybe some of the other stuff they gave me, so I can give her shot, I'll feel a lot better...

I will go off and try to rest. I don't think Kona's going to drop BG too crazy, but I'll try to wake up here in an hour or so to test and keep an eye on her.

Hi Jason, soooo glad you are home and Kona is doing much better. Prayers for continued improvement for sweet Kona. You are super Dad!!

I'm noting the above meds they discharged you with to go home....I suggest have your regular vet call in a script to your local pharmacy. Both of these meds are super cheap and have them on hand going forward in decent quantities (I have 30 pills in each with refills - if vet will agree to that). But I need to use gabapentin for all vet visits for Ivy and sometimes I need to use it for one reason or another (I try not to though).

YES! Absolutely, I told my normal vet this morning that I need to have all these things in my emergency kit so I don't have to mess around and have to drive to differrent towns and wait 8-24 hours to get a simple pill or medicine that could have got me out of the crisis on my own. She is on-board, as far as quantities, who knows what's she's willing to write. I just want something to get me through 3 days minimum and the ability to refill when these things expire.

It's tough finding a delicate balance wanting to keep people informed, but also recognizing extra worry that might be influenced by me giving too much information that makes it all appear more dramatic than it is. I got the drugs and she ate this morning, so we're off to a great start!

Just wanted to ask about why the Entyce rather than continuing with Mirataz.
I don't want to be alarmist, but my cat who recently passed had a terrible reaction to it to put it mildly.

"Side effects include diarrhea, vomiting, increased drinking and urination, drooling, gas, nausea, abdominal pain, increased gastrointestinal sounds, head shaking, and lethargy. In cats, dehydration and infections have been reported. Bloodwork side effects include an increase in blood urea nitrogen (a kidney function indicator) and an increase in phosphorus levels."

He had all of this. Again, I don't want to stress you out, but if the Mirataz is working, why change?
 
From what I see a test for pancreatitis was not run. Nothing jumps out at me. None of those tests I see test for pancreatitis. Only 4 ondansetron is crazy. I’ve never used Entyce. I’ve heard cats drool when given so must taste awful. Mirataz or cyproheptadine is what I use. How’s Kona’s appetite today? Do you have cerenia ? For nausea I like ondansetron as I mentioned but you don’t have enough. You would need it for several days.
 
We've seen a few cats here try Entyce, with mixed success. I would never use it personally, but that's because it causes excess growth hormone to be produced and is contra indicated for kitties with acromegaly. Note, I don't think Kona has acro. The excess growth hormone is what could cause BG to go up.

I think Elise suggested 20 ondansetron in a previous message. I typically get 20 at a time. I would definitely get a prescription for onda that is good for "as needed" for a year that you can fill at any pharmacy. The 4 pills might tide you over until you get more, but depends if the pharmacy has it in stock.

As for labs, some of the values are out of range but could be because of inflammation or infection or stress or diarrhea. Basically telling you what you know. I'd ask the vet for her/his interpretation.

Hey, Lara, Mirataz did work on Friday late afternoon when this all started. That first dose worked into the next day. Yesterday when I attempted to use it again, it didn't do anything for the entire day.

Thanks so much for the warning about Entyce. The vet called it that one of the side effects was that it could raise BG as well.

I was told to only use it if I absolutely have to.

It appears the Cerina is the key here and is what is getting her to eat. Maybe also the pain med injection... it was supposed to last for 24 hours.

I'm going into the vet now, I'll ask if I should try the Mirataz again tonight should she not eat. The bummer with Mirataz is it takes about 2 hours to kick in. I might ask if I should do it preemptively.

Both vets, really want me to limit the use of any medicines given unless needed, they really want to see if she can go without this stuff on her own.

They did run the test for Pancreatitis. They used the Vcheck fPL.

The results was 2.3 ng/mL. Reference Range is 3.5 - 5.4

Anything under 3.5 was deemed very unlikely it is pancreatitis related.

I think I'm going hold off on using Entyce unless I absolutely need to in an emergency, but I will probably try the Miracatz again to see if it will work.

Yes, the vet to say those values out of range are probably influenced by all the factors around this event. I'm bringing in my test results reports to my main vet now to have them look it over as well.
 
UPDATE:

BG has been steadily going down nice and slow and we are now in the yellow zone. This is great to get out of the pinks!
I gave her a small snack with the test and she ate it, so another great sign.

I'm off to the vet to drop off a fecal sample. I believe it is not Kona's, but her sister who also has diarrhea.

I'm guessing Kona hasn't pooped yet because maybe all the probiotics are working and the stool is getting more solid and forming. I'll try to catch her poop and run her fecal as well.

The bupe is good for pain relief. Did the vet give you instructions on how to give it? You don't squirt it in the mouth like any other liquid med. You stick the tip of the syringe into the cheek pouch and squirt. The bupe will be absorbed by the mucous membranes. It has no effect if swallowed.

Feel better, Kona! You look better in blue and green, not pink and yellow.
 
Jason, absolutely a minimalist approach to medication is great. Overmedication brings all sorts of problems.

With the Mirataz, yes, you give it proactively, in anticipation of when you want Lady K to feel hungry, so if you have about a 2 hour envelope before it's working for Koha, then rub some on then, and it'll kick in when anticipated. And the Cerenia is great for this as well. It needs about an hour to take effect. You want to make sure she has Cerenia, so you're not making her feel hungry when she is nauseous. Cerenia is primarily a drug for nausea, but it has some nifty analgesic properties as well.

I'm not trying to play vet here - I feel so pompous - but I just went through this a couple times so far, and the last time very recently.

You can watch her video here just in case you might have tried one of these foods.

On the first set of labs, she has high neutrophils which is indicative of inflammation or infection.

Here is great info on the different tests:
low calcium
low sodium
likely the diarrhea she’s had
low PCO2
high PO2 -- I can’t find much on this in cats but believe it is related to total oxygen in blood dissolved in plasma
BEceF is Base Excess of the Extra Cellular Fluid; a lower value of this indicates metabolic acidosis; metabolic acidosis is more commonly seen in CKD cats but nothing about her labs indicates CKD.
high sO2: i’m not sure what this is; SO2 is sulfur dioxide and I’m not sure of it’s significance here and if that is what they are testing.

If it were me, I’d have someone sit down and explain what each of these parameters is and the significance of them being high or low. I wouldn’t just accept “everything’s ok”. Obviously there are acceptable ranges for these parameters for a reason.

You might check out the various probiotics and digestive enzymes at Adored Beast. I use Healthy Gut and Gut Soothe. They are exceptional and what my vet (holistic) always recommends. She has worked extensively with the owner and said she really knows “her stuff”. I also find Visbiome Vet to be an excellent probiotic.

Many, many vines for your sweet girl. I hope you can find someone who can help you with this.

Yes, they did, thanks so much for the callout. It's pretty much the same how I give her the 1-TDC dental capsules. Note: get it up in the cheek pouch/gums, in an effort to not have her just swallow it.

I'm holding off with Mirataz and will continue Cerenia after both Vets give me confidence they didn't find anything in fecal or urine cultures. Other than the ultrasound, we are ruling out everything we can. If we find nothing, I guess we will never know, it was just a GI issue of some sorts that was a very scary and expensive upset stomach.

This is great to know! I rarely give the Royal Canin Glycobalance when I want to boost carbs. It's a good 14% option for her, only for snacks to boost up BG. If I find another suitable 14%, I'll work on phasing it out.

Here is great info on the different tests:
low calcium
low sodium
likely the diarrhea she’s had
low PCO2
high PO2 -- I can’t find much on this in cats but believe it is related to total oxygen in blood dissolved in plasma
BEceF is Base Excess of the Extra Cellular Fluid; a lower value of this indicates metabolic acidosis; metabolic acidosis is more commonly seen in CKD cats but nothing about her labs indicates CKD.
high sO2: i’m not sure what this is; SO2 is sulfur dioxide and I’m not sure of it’s significance here and if that is what they are testing.

If it were me, I’d have someone sit down and explain what each of these parameters is and the significance of them being high or low. I wouldn’t just accept “everything’s ok”. Obviously there are acceptable ranges for these parameters for a reason.

I'm going to give her a chance to look at them, but I will for sure ask specific questions on these and look for a more comprehensive explanation. I paid big money for it after all... It drives me crazy that we have to ASK for the results. :banghead:

You might check out the various probiotics and digestive enzymes at Adored Beast. I use Healthy Gut and Gut Soothe. They are exceptional and what my vet (holistic) always recommends. She has worked extensively with the owner and said she really knows “her stuff”. I also find Visbiome Vet to be an excellent probiotic.

This is great, I will check these products out. And yes, visbiome has been on my list for awhile now, as soon as I use up the current products, I'll switch over.

It's frustrating not to know exactly what may be going on. Hope this is a acute something that Kona is in a process of recovery.

Yes, I totally hope it is something acute. This has also been a lesson learned for me that I need to pick up dirty food plates the second they finish and I need to date cans of food in the fridge and just throw them out after 2 days max. I also want to clean water fountains more regularly as well.

I've been wondering the last few days if this is just bad hygiene on my part.

Yah, nothing like sitting in the ER and hearing other caretakers sobbing and coming in with all sorts of critical cases and I was just sweating thinking of the worst for Kona. So far, if it is a transient issue, I think we got lucky and I'm blessed.

UPDATE: Kona ate her dinner and I was able to give her the full shot. She pooped just now and it is starting to firm up. I collected it and put it in the fridge to take to the vet tomorrow so they can check it out. After the main poop, she has been going back and forth, back and forth between litterboxes trying to poop more. She's not making any noises and it doesn't appear she is there long or that it is uncomfortable, but going into it many times.... just trying to get a last bit out?

She is starting to be more active and less lethargic. All the best signs I can ask for! We love Cerenia over here!!! :cat:

This is great to know! I rarely give the Royal Canin Glycobalance when I want to boost carbs. It's a good 14% option for her, only for snacks to boost up BG. If I find another suitable 14%, I'll work on phasing it out.
