Changing from Prozinc to Lantus

So Oscar saw the internal medicine group this morning because I was not happy with regular vet and diabetic cat. This vet wants to change from Prozinc (2.5U) to Lantus. They did a frutosamine test today and will report results when received. Will also let me know what dose we will start with. She said we will start from the beginning I think.

Anyway since Lantus works differently from Prozinc can someone tell me their experience changing to Lantus so I know what to expect in the beginning?
 
It would be a mistake to start from the beginning. We usually suggest the same dose or a little less. Looking at your spreadsheet 2.0 would be a good starting dose. Lantus is a depot insulin. It will take 5-7 days for the depot to build but occasionally a cat will react to tge first or second dose. After 5-7 days you will know what the starting dose is doing.

Cleo was switched from Prozinc to Lantus in May this year. Cleo was on 3 units of Prozinc and the Vet wanted me to start at the beginning on Lantus. @tiffmaxee Elise is a thousand percent correct about the starting dose! After reading the stickies, perusing this forum for posts of others that have switched and advice of experienced members, it would have been a huge mistake to start at the beginning. Especially since Lantus needs time for the depot to build.
When will you start? Do you have the U100 syringes needed for Lantus? Like Elise said, please read the stickies and dosing methods. That helps in preparing for Lantus in kitty :). Good luck!
 
Hi Josette, Benny was switched from Prozinc to Lantus (using the generic Glargine) early on in after his diabetes diagnosis. My then-new vet said that it was the insulin most associated with cat remission, so I went with it. It did not show progress right at the beginning, but neither had Prozinc. Benny's diabetes was just that bad. However, in time, he slowly became more controlled, and I'm convinced that the Lantus was one of the two reasons why.

In technical terms, please do read the stickies, because they will prepare you for using Lantus. It is not the same as Prozinc in terms of dosing, syringes, etc. Keep posting on this forum, because you'll find a lot of great help. Listen to the experts who will give you their feedback and answers. Welcome!
 
Switching from PZ to Lantus was the best decision we made. We really started to see improvement after making the switch. Sunny stopped having such extreme peaks and valleys, and his numbers mellowed out, and that's when I noticed he began feeling better. I wish I had known from the beginning that Lantus and the TR method were the most likely to get a cat to remission.

Thank you. Going to switch my regular vet also! But will continue to see this internal medicine group for the diabetes. We felt sooo much better after our visit since I was so stressed and Oscar even more so.

Yes I wanted lantus from the beginning but my vet if 40 years said Prozinc was the only one approved. When we started questioning his dosing he got offended and offered a referral to internal medicine group. Then he didn’t want to see me about the diabetes since we had an appointment with IM ( which was over two months away). So looking for new regular vet is next on the list.

I just ordered the new U100 syringes from Chewy along with one lantus pen (20%discount on first order) . She also gave me a prescription for one pen and another for one vial but I want the prescription for 5 pens so I can order from Marks Marine in Canada. In the meantime I had to get something as it may take 3 weeks from Canada according to their site.
And I don’t know what dose she will prescribe. She might have said 1 or 1-1/2 but I can’t remember. She will send me an email with her notes from his chart.
 
you should be able to use the script you have to get the 5 pen pack from Mark's, that is what I did. In my experience they don't care if you have a script for 1 pen, 1 vial, or what -- as long as it is for Lantus/glargine they seem ok with it

Oh okay. I had Chewy call her for the one pen and syringes so I can send the original prescription to marks . Thanks. Of course she wants me to use virals instead. Says she doesn’t like the silicone stopper on the pens. Likes the one on the vials better ?????? Also doesn’t like the people meters but I will keep what I have for niwy
 
Got the results from urinalysis. No bacteria or infections. The frutosomine test results said 385 which she said was very good on Prozinc according to the chart! So the dose on Lantus will start at 1U twice a day. Chewy pharmacy called her and she signed off on the lantus and the syringes.

I am to do a curve after 7 days and send in all the info.

anyone notice a difference in their cat after switching to Lantus. Do I need to watch him like a hawk….which I do already…..’are you okay, are you breathing’.

I started on Lantus so can't speak to switching from PZ. But Lantus is a very gentle long-lasting insulin.

Please double check that the U100 syringes are the ones with the half unit markings if you haven't done so already. Dose changes are in 1/4 increments if you'll be following dosing advice from here.

I second what Holly said 100%. I tried Vetsulin, then Prozinc, then Lantus and Lantus has by far been the best. Wish I had started it right away. Good luck with the switch!

Yes I did order the syringes with 1/2 markings. Didn’t want to take a wild guess where they were.
