Gucci change of insulin

Good afternoon Mrs Russo,

Please see below the email from Davies. I have ordered the insulin in, this should be with us on Wednesday. Once it is in stock, either myself or Emma will call you and discuss further about exactly how to administer it and the monitoring required. In the meantime, I would be really grateful if you could order the pen that Sophie has recommended to facilitate in giving the insulin to Gucci

"Thank you for sending through Gucci's latest curves. How is he doing clinically? He seems to be fairly sensitive to Prozinc, even at 1IU but the onset is quick and short-lasting, resulting in some steep peaks/troughs in his glucose curves although we can't see from these at what times his owners have been giving his insulin. Based off of the data from his most recent curves, he would be a good candidate to change over to glargine insulin which has a longer, more basal onset of action and will hopefully stop the rapid fluctuations in his glucose with the aim to achieve better glycaemic control. We would recommend starting on 1IU BID of glargine insulin and with this we suggest using an insulin pen (e.g. the JuniorSTAR pen) to allow for better accuracy of dosing, as glargine insulin comes in 100IU/ml formulations (Prozinc is 40IU/ml). He will need to be monitored closely in the few days following transition over to glargine and therefore it is recommended that he have a FreeStyle Libre monitor placed in order for his owners to be able to keep a close eye on him at home.
