New to diabetes, lots and lots of questions + DKA?

Hello, this may get very long so I'm so sorry! I have been reading up and will comtinue to research but I have so many questions. My cats have never had any health problems and have never needed anything other than well visits, so this has thrown me a bit. My kitty boy is 9, I noticed I had to fill up the big water bowl every night and that there was so much pee my cat was basically flooding the litter box. So much it just sits on top and is a nightmare to scoop!

Went to the vet, blood glucose was 350 so it all made sense. His numbers were low that day actually, as he tends to hover around 400-500. Perscribed 1 unit twice a day of Novulin N. 12 hours apart. I know it's not really recommended anymore, but right now its within our budget- I *am* trying to get the lantus savings card pens figured out and get him on that. I do think the novulin is kind of working a bit but it's only been about a week. So we're around 400-500 to start most days, and around 4 hours later we end up around 200-250. I have not been able to complete a curve yet and will try this Sunday. Right now we are testing before each shot and when able a few hours after. Also if we think something is up.

I'm concerned about DKA. My boy had high levels of ketones in his urine a qeek ago (via dipstick at home) and when I asked the vet she said it's because he's been unregulated, his body is burning fat for fuel. That's true. He has lost 4 pounds (I have no idea how quick that happened but I noticed he was feeling lighter.)

Starting last night he's thrown up maybe 3 or 4 times. Yellow (bile?) Also diarrhea today too. No appetite. A lot less water intake but i cant tell if its just "normal cat amount" as hes been drinking soooo much in the last months. Seems like none was getting drunken and not really any pee in the main litter box the last few days. Well today there was a large amount sitting on top of the litter. As far as food, he was eating now he's not. We've tried the recommended lower cost low carb foods, and all the recommended flavors of fancy feast and friskies pate. He hates pate. Tried making it like a gravy and warming it up (he LOVES gravy) but all of a sudden hes a picky lad lol! Meow mix wet food, I forget the name but its low in carbs. He was liking that! Finally after trying so many he would eat it. Well, seemingly not anymore or it's just his low appetite. Seems tired. Tested ketone and it was trace amount.

Does that indicate DKA at all? Anything I should worry about immediately? I left a voicemail for the vet. I also don't know what I should do about insulin because I can't get him to eat. Not give it? I gave friskies meaty bits and he only ate about 1 oz. Any guidance is so so appreciated. I'm worried about my boy, all of this has been overwhelming but I want him to be comfortable and feel well.

Thank you sugar parents!

IF YOUR VET CAN PRESCRIBE LANTUS
Which I hope the vet does

You can definitely get 5 pens of Lantus or the generic Glargine

You can Check
CVS, Rite Aid, Costco, Walgreen's, to get the price for 5 pens either Lantus or the generic Glargine , plenty of members use the generic
Just call around and see who has the cheapest price , it's really not that expensive
With the pens just keep them in the fridge middle shelf , they all have the expiration date on them .

Also
The 5 pens will last about a year, we use the pens just like a vial, you would just insert the syringe it the gray rubber stopper on the pen and draw out your insulin
Its generic lantus

If you use the Good Rx
GoodRX has a 1-800 number to assist you in using their services to get your pets prescriptions filled if the pharmacy gives you any issues.

I think I found it for you

finally got the pharmacy to honor the 5 pens for $35 coupon. I had my vet write the prescription as 8 to 25 units twice a day and that seemed to do the trick.

I think you need to get your cat into see the vet. I'm really not liking that he's not eating well and is lethargic. Most uncontrolled diabetic cats are constantly hungry. Lethargy, at least for me, is a big red flag for DKA. DKA occurs if there's an underlying infection or inflammation + not enough calories + not enough insulin. If you're seeing more than trace ketones on a ketone urine test strip, your kitty needs to see the vet.
