Switching insulin

This post is not for me, so disregard Mauer's spreadsheet and such.
This post is for @Marlene82 who's Karls mom.

Karl was diagnosed this past december, so he's still a new diabetic. Marlene hasn't had good results with prozinc the last two months, so she got a prescription for levemir today.

Karl is fed raw and canned food, runs outside and cannot be kept inside. We suspected he ran out and found some food, but Marlene has kept him inside for a week, without any results to Karl's BG.

At what dose should she start Karl on? Marlene can periodically test more, but not regularly. So I believe start slow go slow is the way for Karl?

Marlene will participate as much as her language skills allows her.
 
When I switched from prozinc to lantus, the suggestion was to go a little lower than 1:1. However, I see that Marlene just increased the dose and does not have enough data with 1 U, so perhaps 0.5 would be a safer place to start?

Just an opinion.... I am sure people with more experience will pipe in...
 
With a switch to prozinc you can give the same dose or maybe a tad less. Based upon the recent increase .75 would be a good dose.
 
Sometimes with a change in insulins they can have a strong reaction, so she may want to make sure that on the day she makes the switch she can monitor reasonably closely. And probably keep him just in case.
If she is not able to monitor him fairly closely for at least the first couple of cycles I would possible shoot 0.5


I have a cat that gets stressed out when he can't go out, I have found that if I play hunting games with him throwing balls getting him to play with his toys, that usually wears him out and settles down.

Correct, the 70% is when switching from Lantus to Levemir, or the other way if anyone should do it. When switching from one of the other insulins to one of the L's, we take the dose of the existing insulin into consideration. So if the existing dose was getting greens, we might back off, but otherwise not. The problem is we don't have enough data to see what the previous dose was doing, so I agree with either 0.75 if she can monitor closely, 0.5 if not.

Well, you've all said what we was thinking. From one nervous wreck to the other! Marlene is going to start on 0.5 considering her testing, that Karl's prozinc might be too old and lazy, and that Karl previously had great numbers on a low dose.

Marlene is envious and freaked out by Mauer's new color range, so fingers crossed Karl will behave nicely and spread some green happiness around :D:D:joyful:
 
Sasha, just a thought, If Marlene creates a thread of her own, then you can still do the explaining for her but at least she can do a search and see the info pertaining to Karl.:)
And we all know it's another Kitty and CG rather than thinking it's a second post from you, when we are glancing through the lists of new posts

Perhaps this is a good idea.
This have worked before, but the prozinc forum isn't as busy as this one. So it's easier to find again.
I'll definitely talk to her about it :D
