Switching to Lantus

I will be switching Frank to Lantus very soon. Right now he is doing well on Vetsulin (2 units/2x day), but this will soon not be dispensed by the vet anymore.
So I am wondering about Lantus. The vet says we will start at 1 unit Lantus. I need to gather more info on Lantus before I start Frank on this. I would like to hear any thoughts you have about starting up on Lantus. I know that you don't shake it, and that the syringes will be different. If there is something that you think is important that I focus on, could you please let me know?

Welcome, Delma! Lantus is a different kind of insulin - it's also one of the best out there. Like was said, read, read, read...and then read some more. I am having good results on Lantus, far better than what I had with other insulins, and am very pleased with it overall.

Perhaps the biggest difference between the two types of insulin is how you approach dosing. Lantus is a long acting insulin. Dosing decisions are based on the nadir (low point/peak action) in the cycle rather than pre-shot (PS) values. In addition, in most cases, doses are held for a minimum of 3 days. The protocol that we use here is a modified version of the German Tight Regulation (Tilly) Protocol.

Hopefully, your vet was recommending 1 unit twice a day (BID) since Lantus needs to be dosed every 12 hours.

We are very data driving in this forum. We encourage getting PS values and at least one additional test each cycle, at the bare minimum. If you are not already home testing, learning to do so will be essential.

Hi Delma and welcome to Lantus Land! I don't know if you are currently home testing but that is one of the MOST important things about Lantus...well about any insulin really but Lantus especially. You will need to know when the nadir (lowest reading) is for your cat in order to help our dosing team help you. You will need to be testing before each shot as well as getting spot checks throughout the day and night so we can identify Frank's nadir and any other pattern of insulin use...for instance, my Morris is a double dipper...he will go up after a shot, then go back down and then go up again before the next shot....and his nadir is at +8 or +9 (8 or 9 hours after shot) while others may be +4 or +11...ECID.
