Triaditis Admin please advise

It’s been a long while since I’ve been on here. So I’m not up to date with Calcifur’s insulin chart. However, his glucose is very well managed.

That being said. He recently was diagnosed with Triaditis and I need advice on diet and any help on how to move through this process with ease.

Why do you need to change his diet? Does he have a food allergy? Been diagnosed with IBD? What symptoms is he currently seeing?

According to blood test and ultrasound, an IBD diagnosis was given. He also has pancreatitis and inflammation in his liver and gallbladder with sludge buildup.
I was hoping to get advice about possible healthier foods. Fancy Feast, his primary food, seems to be rather oily and contains lots of ingredients. I'm not sure if this negatively impacts his digestive system.

I'm also just overwhelmed with everything my baby has going on. I was looking for reassurance that we can successfully get him through yet another diagnosis.
I started Marboflaxocin yesterday. It made him nauseated, so I gave him Cerenia. And he starts Ursodiol today.

Are there any natural remedies that can help support him through this?

@Lane&Biddy — Biddy has triaditis and must take Prednisolone yet is still doing very well. Lane, I thought you might have some support to give.

I have a cat with verified IBD (surgical biopsy) who is on Prednisolone although he is not diabetic. I would focus on trying to find a food that works for Calcifur. If he does have to take a steroid then you will just have to adjust insulin. What about pain meds? Is he taking pain medication? Does he seem to have pain? Have you tried Cerenia? It can be helpful where inflammation is involved in the gut since can act as an anti-inflammatory there. Some cats really do well on a daily dose of it. @fpa

That's not enough to diagnose IBD, you also need a biopsy, either from endoscopy or surgical.

Regardless of what is wrong with his GI system, you might want to consider probiotics. Visbiome is a great one for IBD or other GI conditions, and is specifically made for IBD. It's what my IBD kitty takes.

Regarding diet, you might want to try a novel protein diet. By that, I mean a protein he's never had before, like lamb, duck, rabbit, venison, kangaroo. he has advanced cardiomyopathy and takes two blood thinners and many heart meds. I don’t believe he can have a biopsy. He can’t have anesthesia. He started visbiome last week. just got him some rabbit pate from Raw’s.

Calcifur typically eats small meals throughout the day. I was told he should eat only twice a day when dealing with pancreatitis. Is this correct?

His vet said he may need to be on antibiotics 6 plus weeks to clear up the bacterial infection. Does this sound accurate?

For Biddy’s Triaditis we tried novel proteins but didn’t see any improvement. We switched to all hydrolyzed protein, Hill’s z/d canned. Unfortunately, it is a higher carb food so that with 10mg pred twice a day it’s safe to say Biddy probably isn’t going into remission but I’ve come to terms with that. He has responded well in the past to Visbiome probiotics but saw the most significant improvement with Proviable KP. Triaditis I quite tricky I am so sorry you are dealing with it as well.

Are you giving any B12 supplements? Injections work better than oral supplements but Cobalequin is a good oral product. It comes in a spinkle capsule and chewable tablet.

Thanks for the further details on his heart condition, you are correct that no surgical or endoscopy biopsy without anaesthesia. One possible method to diagnose - if his lymph nodes are inflamed, a sample can be taken by ultrasound guided biopsy, that only needs sedation. That biopsy would then be sent off for PARR (cloning) analysis. The only reason I'm going on about biopsy is that if it is indeed small cell lymphoma instead of IBD, a novel protein diet won't do much. If a novel protein diet does help then that is an indicator he has IBD, though cats can have both. As I have experience with.

I will see if we can get that biopsy on his next visit in a month. His ultrasound sound did show a possible swollen lymph node and thickening of the SI.

If I don’t feed Biddy every 6 hours he vomits. I have an automatic feeder but he is a food bully to our other cat so I wake up in the night to feed them and I go home for lunch to feed them. I am very lucky to have that flexibility. As long as I keep the routine he does not vomit at all. But if I am late feeding him it is guaranteed he will vomit.

Oh wow. My cats never have trouble with Zenequin (Marbofloxacin) and I like the once a day dosing. I am sorry it made him vomit, and I hope that was a one time thing.

We see that a lot here — many vets are stuck in the old way of thinking regarding feeding. Obviously they’ve never had to manage the day to day and hour to hour insulin cycles of a diabetic cat. This may work all right for dogs, but is not appropriate for cats. Diabetic cats do much better with multiple small meals during their cycle (generally before their typical nadir hour.)

so the multiple small meals won’t put too much stress on his pancreas?
have you used pancreatic enzymes to support through pancreatic episodes?

Pancreatic enzymes are for cats with EPI (Exocrine pancreatic insufficiency), and should not be used in cats without that condition. You could try digestive enzymes, which are different.
