Hi and welcome to the forum.
It is great you are hometesting and taking a test before every insulin shot. To know if the insulin is working though, you will need to be getting a test in sometime between +4 and +7 ( that is 4 to 7 hours after the dose is given).
By the time you are taking the next test after 12 hours, the insulin has worn off and the blood glucose will have gone back up higher.
Lantus dosing is based on how low the dose takes your kitty, not the preshot.
Insulin is a hormone and it doesn’t work like an antibiotic, where it fixes the problem straight away. It takes times for the cat to get used to the hormone and for things to start working. Some cats get the hang of it quickly but most cats can take months or longer. So patience is the biggest thing that is needed with feline diabetes.
Here is what I would suggest.
I am going to give you a link to our spreadsheet. If you set that up and test before every shot and then again during the cycles, we can help you with dosing. And if you would set up your signature (vey easy) so that we can see all about Buddy , we will be very happy to help you. We need all that information to be able to do so
Are you feeding a low carb wet diet?
Are you given snacks during the cycles as well as the 2 main meals?
Do you have a hypo kit set up in case of low numbers? There is information about that in the below link as well.
We have 2 dosing methods for Lantus and you can choose one to follow. More into about that later.
It’s not uncommon for diabetic cats to get pancreatitis. How is his appetite now?
Here is the link to the spreads and signature
