I don't have any experience with budesonide, but I am sure some will weigh in. What are Rex's symptoms?

Mainly nausea and vomiting. Its not a constant but the vomiting episodes have been happening once a week now. The vet says we can keep doing cerenia, but she'd rather treat what's happening vs just the symptom... which I agree. So she said if this med works, then great! If not, an ultrasound will be the next step to see if we can find a cause

Did they do a Thyroid screen also?

See if any of it resonates with you. Does she go a long period of time without food? If not then then maybe a thyroid screening might be a good place to start and if nothing shows, then an U/S might be a good idea. Hopefully, others will weigh in.

When do the vomits happen? After a long fast?

My Neko was on budesonide, it is a steroid that acts more locally in the GI system than does pred. Her heart couldn’t handle pred.

It’s IBD, not IBS that you typically see in cats. Diagnosing starts with an ultrasound, but needs an endoscopy or surgical biopsy to diagnose and differentiate between it and small cell lymphoma. Neko could not have the anesthesia required for a biopsy, so we had to treat without really knowing what it was. Ideally you want to know what you are dealing with so you can medicate appropriately. Small cell means chemo and you would rather not give that if you don’t need to. Note that a biopsy cannot be done if a steroid (budesonide or pred) is being given as it masks what is really happening. You need to off the steroid for several weeks first.

If IBD, you typically treat with novel protein diet and probiotics (I give Visbiome which is specifically for IBD). You might also want a GI panel, the Texas A&M is one such, that tells you if the B12 or folate is low, as can happen with these cats. In which case you might need supplementation. You might need a steroid for a short term only if you can manage with diet,
