Dosing help needed

OK, so we have recently started Levemir and overdosed at first with 1U. Since then, he has been in a bounce and we have been holding at .5U as other members have suggested. Today, we checked him at +11 and his back to back reads were 61 and 75. He is scheduled for a .5U shot in one hour. Should we continue on with that? I'm so new to Levemir and the idea of depot insulin that I don't know what to do. Thanks!
 
You don't have a lot of data yet. I would test about 10 minutes or so before his shot is due and post that number and ask for advice. Do not feed him, so you can see what his numbers are doing without the influence of food.
 
I like Dyana's suggestion of seeing where he's at when his shot is due.

We have 2 dosing methods on this group. Not sure if you've had a chance to look at them yet - the Start Low Go Slow method and the Tight Regulation Protocol. If you want to follow the Start Low Go Slow then you would reduce the dose because you got a test less than 90. If you're following Tight Regulation, you would hold the dose until he goes below 50.

The main difference between the 2 is that SLGS can be used with a cat eating dry food (Does he eat any dry) and the Tight Reg cannot be used with cats eating dry. Also the number of tests is going to be more with Tight Reg. You are already testing enough to follow the TR protocol if you want to.

If you have a few minutes between now and the next shot time to read a little on those, it will help us help you move forward with the appropriate advice.

Okay, I would feed him, and test again in 2 hours. Copy?
 
are you able to monitor tonight? Do you have a gravy medium or high carb food?

If you are comfortable... with that number....

It would have been better to not feed as that is now a food influenced number..... you probably missed that advise earlier in the condo.....
 
Yes, I can monitor. He has FF classics and some Iams for more high carb. I didn't understand that you meant to not feed him at all. I thought it was just until the next reading. We didn't give him his shot as scheduled. We will continue to monitor. Is this normal?
 
I would say that's about an average increase in bg from food on board....
so are you comfortable shooting a 131.....

i'll stick around.... there will be others later I'm sure....
 
I'm sorry. I saw the 0.50 in the units column of your spreadsheet and assumed you shot the dose :oops:

you might want to read the links given above about the go slow and tight regulation options.....

another option you have is to give a token shot.... half the dose.... just so you don't drain the depot completely.....
 
Hi Erin! I think it was a mix-up in communication earlier tonight.

It's fine that you went ahead and gave him his shot now. For tomorrow morning, though, you're going to want to skip and just get back on schedule tomorrow night. If you want to shoot early tomorrow night you can. It's important to have 12 hrs in between the shots, so you won't shoot tomorrow morning at your usual time. Just skip it and don't worry about it. He'll get on track soon enough and it's safer than shooting again 6 hrs after tonight's shot.

Does that make sense?
 
Yikes. He was reading high 200s this morning before feeding, so we gave him .25U. I hope that wasn't a bad thing. His name is Boops, and he is my only child. I've had this sweet boy since he was a few weeks old in April of 2000. He is my animal soulmate for sure.
 
Would you note that on your spreadsheet by either stacking the #s all in the preshot column like this:

+12 = 131
+13 = 174
+15 = 378

then nothing in the rest of the pm cycle column and a note in the comments saying "Shot 0.25u @ +15 and 0.25u at +6"

Or writing a description of what happened in the comments column.

Looks like he took it all fine, but just make a note to not shoot at less than 12 hour intervals if something like this happens again. The reason is that if shots are given too close together, you could overlapping action between both shots and that could cause lower numbers than you would otherwise expect.

For now, though, all is well.

You probably want to go back to 0.5u that you meant to be at now, and hold that dose for a few days unless he goes low enough to have his dose reduced, depending on which dosing method you want to follow. Lantus works best when the dose isn't changed too often.

You are blessed to have an animal soulmate! If you were already close, dealing with his diabetes will bring you closer than you can ever imagine. It's a crappy disease, but it does have benefits in how closely bonded you get with your cat. It's pretty amazing how in tune one becomes with our diabetic cats. :D
