Help needed - not 911 but New Wenchie issue and question

@Deb & Wink So, did two retests this morning after administering the 1 drop protocol. Although she's not low, she's slowly moving lower. At +6 she was 335. I'm very happy doing this slowly and highly managing the dosage. Also tested for ketones and all negative.

Question - how long do I keep up with 1 drop dose? What should I look for to increase to a 2 drop dose?

If you are doing TR (tight regulation) you try to hold the same dose for 3 days or 6 cycles. Then reevaluate. Unless Wenchie has other ideas and drops really low before those 6 cycles are over (She walks to a "different drummer", is doing the "sugardance to her own tune", so I fully expect her to walk to her own beat.) ECID Every Cat is Different and Wenchie certainly proves that maxim.

If you are doing SLGS (Start Low, Go Slow), you try to hold the same dose for 1 week. Then reevaluate.

Right now, Wenchie has only been on this 1 drop dose for 2 cycles. We need to let the lantus insulin "depot" refill and see where that takes her BG levels. In any case, weather using the TR or SLGS protocol, the dose should be held for 4 more cycles.

IF and it's a very big IF, Wenchie does not drop too drastically low like she has in the past.

You have not selected one of these Lantus dosing protocols and you need to.

The complication is, since Wenchie is on such a low dose of insulin, drop dosing, the normal guidelines of how much to raise the dose by for a particular BG range do not apply. So instead of using the more common increase/decrease strategy of raising/lowering the dose by 0.25U increments or 0.50U increments, we'll have you raise and lower by drops.

As far as when to raise and lower, that will depend on Wenchie and if you choose the lantus TR or SLGS protocol.

p.s. Did you figure out yet how many drops you get from the 0.1U measurement?

OK - great info once again. So, I think I prefer the SLGS protocol. I'd rather go really slow and raise her up at the appropriate intervals, as long as she doesn't drop or spike suddenly. I will indicate SLGS on my SS.

I tested out the .1u measurement this morning and it's 3-4 drops. I will test it out again. Such a great idea! She seems tired but her little body has been through a lot in a short amount of time. She's still eating, drinking, purring but definitely lying around and not up for anything else. That's OK. She is an old lady (and I feel like one since this started!).

The drop dosing has not been too difficult. I have a pretty steady hand and don't mind wasting such a tiny amount of insulin should I miscalculate. I'm pretty confident I have this down but I check and double check by having my husband check after me. I will hold this dosing for a week.

We're finally going to get out of the house for a couple of hours. I tested her at +6 (12:00 noon) and she wasn't ridiculously high but a wee bit lower than +2 so I'm confident that she'll be ok for a short time. I need some fresh air!

We did - we actually went to the Lee Outlet Mall in Mass - what a beautiful day to walk around an outdoor mall!

Happy to be home, though. I'm always scared that I'll find Wenchie passed away. I wake up constantly in the middle of the night to check on her. I guess that's normal under the circumstances but it's heartbreaking.

Wenchie just threw up - she tends to throw up during that 2 hour period when we pick up her food prior to her AMPS and PMPS. Do you think Cerenia (injection) would help her? She's so painfully thin she cannot afford to toss up any of her food.
 
You can get Cerenia in pill form to have at home if you need it but with her habit of puking just before shot times makes me wonder if the problem is acid.

Do you have some 10mg Pepcid (famotidine)? Might be worth trying to see if it settles her tummy down. 1/4 of a 10mg pill is the usual starting dose.
 
Is she gorging on food? Eating too quickly because she is so hungry?
Any signs of hair mixed in with the food?
It's been shedding season at my home for a couple of months.
Wenchie is longer haired too. At least her ruff looks long.

Is the vomiting only with one of the food flavors? Maybe the beef does not agree with her.

Cerenia may help.

If you absolutely have to, don't take away the food before the pre-shot tests. we'll simply have to work around that.
It's going to be tougher, especially with her on such a low dose of insulin.

I had a late lunch out on my screened in front porch. Got my cat Dancer to come out with me. All those great smells from the chipmunks getting through the gap in the screens and the wrens that had snuck inside a couple of times this winter were keeping her sniffing all over the place.

Wenchie's a grazer. Eats very slowly, walks away, gets some water, relaxes for a bit, then goes back for more. She is a Maine Coon so that's possible. But this is too much of a coincidence.

I've been keeping track of her foods. She's all over the map as to the flavors so I'm thinking probably not. Chris suggested Pepcid so I will get some tomorrow when I go to Walgreen's. She's so thin - it breaks my heart.

We have our screened in porch closed down for the winter or else I would have been in there! The cats miss it - we need to live in a warmer climate!
 
Great - I ran out and bought some Pepcid AC. Getting my pill cutter out. She's eating now, had her 1 drop and seems content as long as there is food in front of her. Will give her the Pepcid before bedtime
 
We had great luck with the Pepcid AC here. Charlie absolutely HAD to have it and our university vet (whom I go way back with) said it was pretty innocuous. It had no impact on BGs.

Just make sure it’s Pepcid AC and not Pepcid Complete (I see you got the correct thing; just noting that for other readers), and the 10mg dosage. There are cheaper off-brands but I always had better luck splitting the Pepcid tablets. Some of the off brands were more likely to crumble into pieces or powder.

Pepcid AC (drug name famotidine) has fallen out of favor a bit in recent years in lieu of ondansetron and cerenia and others. However it has been my go-to for years and has worked well ... so I stick with it. Good luck!

I bought the generic Pepcid AC, not the Complete, and just cut one into 1/4 and gave it to Wenchie. I'm hoping it's just acid and that's why she's vomiting. Time will tell.

How often can I give her a 1/4 pill? I'm off to bed now. Thank you as always for all your help - you, @Chris & China (GA) , and @Deb & Wink have been my lifelines. I don't know how this is going to end but we are giving it everything we have.

Don't remember how often you can give the 1/4 pill dose. I seem to remember that Wink got that twice a day, roughly 12 hours apart.

I actually kept a separate monthly food and medicines chart for Wink. Still have them on my computer hard drive.
Back in March 2013, he was getting the Pepcid AC 1/4 tablet 2 times a day. He only needed it for about 8 days.
 
Twice a day it is - thank you everyone. She got 1/4 tablet at 8pm and I'll give her one at 8am.

She had a good night, no vomiting, so I'm hoping the pill worked. Will update my SS once we get our morning started. This first hour is for me and my coffee (after doing litter boxes, etc.)
