Caninsulin - possible remission? 5.9 mmol

    Hello, please help me. My cat is 10 years old. Few months ago we went for a regular checkup, and the vet said the cat has diabetes, gave us caninsulin 2 units in the morning and 3 in the evening. I started the injections and it was very difficult, but I managed. The cat gain weight, she's bulky now. However, she was very inactive after I started the insuline. One night she suddenly paralyzed, could barely moved her back legs. Then she went blind! I had no idea this is because of diabetes, but it was super scary. Luckily I gave her food, she ate A LOT. Few hours later in the night she was better and in the morning she was not blind anymore and she could walk, but she is somehow limping since then.
    We took the cat to vet, asked about hypoglycemia, he said the dosage is correct and he's the doctor, and if this happens again, to give her some sweet gel. I did again the insulin, cat started to be again super weak. After two days I decided I will lower the dosage to 2 units and 1 unit night. A curve was supposed to be done at vet, but due to covid they took only emergencies. So after a while I lowered the dosage to 1.5 and 1. All this took few months, since April. We finally went for a curve, and we told him that I give the lower dosage, not his 5 units. At the end of the day he said continue like this.

    Now I don't trust anymore the vet, as if I would have continued with the 5 units probably the cat would have died. I don't trust because my cat is super scared of car ride, and I thought maybe her initial glucose level was due to stress, but he said it's not possible.

    At one point I stopped the insulin for few days, but then I got scared. Anyway now I have an alpha track 2 second hand, and strips free style. I took the cat glucose in the evening, before the shot, and she's at 5.9 mmol. It's not too low? I am afraid to do the shot.

    My question is: can I stop the caninsulin for few days and take the cat's glucose each 2 hours and see if it's normal and maybe she had a remission and she doesn't need insuline anymore? Or I should continue to give the shots and monitor and there is a way to see that she that she's in remission?

    I don't see her drinking too much water, she eats normally, but she's not active, she mostly sleeps and she gained weight, like she's heavy compared to few months ago. What do I have to do? Take her to another vet and do a fructaminose test? She is stressed at the vet and I am afraid a new curve there will not be accurate due to stress. Please help me if you have an idea. How do I know that she is in remission? I am scared to give her insuline and also to stop it, and super scared of another hypo with blindness and all that. Thank you!!!     I am not familiar with mmol/L leading, so I had a quick look and 5.9 is just over 100. That is pretty low and 1ui will be definitely too much.
    My cat has been on the threshold of becoming independent for several months, but he still needs the minimum boost (0.1 - 0.15) twice daily, well, mostly - occasionally I skip the injection when he gives me under 100 before the shot (roughly 11:30hrs after the last one).
    But even then, his number tends to crawl back up if I skip for the full cycle.
    So I wouldn't jump on the conclusion about remission quite at this moment.
    You might want to decrease the dosage even further though, which is better done with a different type of syringe, U100, it helps you to be more precise to measure tiny amounts.
    Another thing I can suggest is to perform the test before each shot - it is particularly crucial when her numbers are becoming lower.
    I am so glad she pulled through the initial crisis, and you won't want to have that happen again for sure!

    Welcome, I’m so glad you are home testing, you’ve probably saved your kitty’s life!!! 5.9 is within the normal range with an alpha track meter, but it is too soon to know if your cat is in remission. We recommend that you do not shoot if bg is below 11 at least in the beginning. Have you fed her yet? If not, then get another test and see if her glucose is heading up. I’m not experienced with caninsulin, so others will be along to help you determine a good dose for her.

    Thank you, Oliver! I had no idea about these syringes, I have a box from caninsuline with U-40. So you're saying U-100 would be useful if I will go under 1 unit, right? You don't think 5 units per day was too much at the beginning? Unfortunately I feed her, I read now that I should have not feed her and repeat the test in 1 hour :(

    I fed her :( So now there is no point at doing another test today? It was very difficult with the injections, I HOPE she will let me take the blood samples from her ears.

    Here is the initial tip I got from @Elizabeth and Bertie who introduced me to U100 syringe - she explained better than I can, so I just copy her text.

    "Caninsulin is a U40 insulin, that means it has 40 units of insulin per ml. But human insulins are usually U100 and have 100 units of insulin per ml. So, the U100 insulin is 2.5 times as 'strong' (for want of a better word) than the U40. That means, for example, that one unit of U40 Caninsulin would measure to the 2.5 unit mark on a U100 syringe. And if you wanted to give 0.2 of a unit of Caninsulin that would measure to the 0.5 mark on a U100 syringe.
    U100 syringes are pretty widely available because they're made for human use. It is important to get the ones with half unit markings."

    and yes indeed, they are for humans so widely available just about at any pharmacy.
    And in order to be precise, it is better not to test within 2 hours of feeding.

    Something is not right. I took it again and now it's 4.5. I wanted to test my other cat (which is also sick, but not diabetes) and she almost bit me. I wanted to see if the AT2 is broken, but I cannot test her.

    How long since she ate? With no insulin, that is a great number. I just saw that you are using freestyle test strips in your Alphatrac? Are they new? I’ve never used an Alphatrac, I’ll tag @JanetNJ

    At 6:30 PM she had 5.9, I fed her at 7, and at 9 she was 4.5. Alphatrack I bought it second hand, from someone who had a dog. Then realized the control solution they had is expired, ordered one express, did the control test today, and that's what I got... the strips are not alphatrack. I should have bought a new kit instead, already spent $150 and now I'm not trusting the device and the strips. The other cat definitely doesn't let me test her. The freestyle package was new, I opened it today.

    Made a mistake, the strips are petsure, not freestyle. And I read some reviews saying they show less than the actual value. So basically I don’t even know what’s my cat glucose value. :-( Two days ago I asked several vets and no one has to sell alphatrack strips.

    Be sure to test this AM before you feed. I think the Alphatrak has a code that needs to be set, but I’m not sure what it is or how to do it. @JanetNJ uses an alphatrak. You can always get a human meter from a pharmacy too

    I might be wrong, but my understanding has been that strips need to be from the same brand and they are not compatible with other glucometres from other manufacturers??? Or is there a certain brand which are universal?
    Someone can educate me on this??? I am curious to know!!

    This was last night at 10 PM: Now I got a 6.9 by changing the code on alphatrak with petsure strips. I saw a video where they said to put the code for alpha track turbo, not purple, that is on the petsure box, because it will be higher and closer to reality.

    Thank you, Janet! In tested using code 22, which is on the box for AT2, then I saw their video saying that it's better code 4, as code 22 will give lower results. I switched to code 4 since last night.

    So yesterday evening I got 5.9, 4 (then switched to code that is more accurate) 6.9. I did do insulin
    This morning she had 5.6, one hour later 6.3. I fed her now, but I don't know if I should give insulin or not.

    She became very angry at the second try today, I don't know if this will be sustainable :( I tried to set the spreadsheet, but got error with google. Will try again.

    I cannot save your template, I tried many times, I get error from google. So I created myself a file, and I will just add manually the colors. I know how to do formulas, but I still hope I will be able to save a copy of your doc at one point.
    Please let me know if you can access the spreadsheet under my signature. She is under the blue section, I don't know... I just gave her 0.5 units caninsulin, but honestly I have no idea what I am doing.

    Try to take your kitty to the designated testing spot multiple times a day and just rub her ear, giver her a low carb treat, and let her go. This will help to desensitize her to the testing. Be sure you are relaxed and have everything ready to go before you start. My cat wasn’t thrilled with testing when we began, but after awhile he’d come running when I shook the test strips container. He loved the treats!
    Be sure to get a test 2-3 hours after the insulin. If you can, a Curve would be good to get (testing every 2 hours)

    My cat used to eat Orijen and Acana as dried food, and different wet foods. Since diabetes she eats Wellness pate, Little Big Paw (UK company), as dry food Instinct Raw Boost, and I give her raw organic chicken breast (from Costco), sometimes tuna and even raw pork she likes. She doesn't like to eat everything in one shot, that's why I leave her dry food too, as the wet food doesn't last for hours at room temperature.
    The other cat only likes Royal Canin (which I don't) so I am trying to feed her in separate room, but Mitzy sometimes steals some royal canin (pouch for digestion and dry food). I don't like Royal Canin but the cat has mouth issues and she is skinny, I am happy whenever she eats something.

    My problem is that maybe the cat doesn't have diabetes anymore (if ever had... honestly the diagnostic was based on a single test at vet, with a super stressed cat), she gain weight, she clearly has issues at back legs since the hypoglycemic horrible episode, and I decreased the dose based on my "feelings", because the doctor insisted on 5 units per day. After the curve they did, he said around 2 units is fine. I don't know what's worse, to give her too much insuline, or too litte?

    If I will do a fructosamine test at vet, it will be relevant if the cat is still diabetic or not? If I give her 1 - 1.5 units per day, is there any test that will show me that is too much? If I want to do a blood glucose, when I feed the cat during the 12h cycle? whenever she wants she can eat? I am sorry I have so many questions.

    Too much insulin is worse than too little. A hypo can kill quickly. If you can get some tests in we will be able to see how she’s doing and if insulin is needed. The dry food is very high in Carbs and getting rid of that may do the trick. I know it’s hard with multiple cats, I had the same problem. Yes one test at the vets isn’t a good indicator since stress can raise BG significantly, that’s why home testing is so important. Keep asking questions, no apologies needed!!!

    That's why I looked for a dry food that is lower in carbs and has no grains etc. But yes, I might try to eliminate it completely. For wet I also chose grain free and low carb. I think the Welness pate and Little Big Paw are good options. I hope raw chicken is good, as she really likes it. I always thought raw must be good.

    My frustration is that I know people with old cats, they only bought the cheapest possible dry food on the market, and these cats are doing fine. While I pay a fortune for all kind of high quality wet food (and throw away half of it), I had constantly sick cats and I kinda don't afford anymore the vet. Plus, I don't trust the vet anymore. 2 years ago my cat had blood in the pee, so immediately I took her to vet. In few days of home treatment with antibiotics she was fine, but I paid $400 for an echo, and they said she has a huge tumor on the bladder, asked several specialists and they said they won't operate. We did nothing, therefore. Now 2 years later, I don't even know if she has that, or if she actually had diabetes. Before the insulin she had no problem with the legs, now she's limping. It was a huge struggle for the injections, my cat is very wild. She never likes it. Now with the ears, I don't know if she will let me do it. She is crying, wants to bite me and I see the ears damaged.

    I am thinking about fructosamin test in case I cannot home testing, I understand it tells an average and not the highs and lows, but... also, I hope I will have money for a dental clean, as that might help too.

    Please tell me if I will try tomorrow the curve, during the 12 hours she can eat whenever she wants?

So if I aim for a 2h test, I can feed her each 2 hours after the test? Because she eats small qualities during the day.

    It’s ok to feed her during the curve, just try to keep the bulk of the food before her nadir, which is the lowest point of the cycle. Have you had a chance to check out the link I posted above, the Beginners Guide? I think it has some feeding guidelines.

    I always fed my cat a small amount after each test, so that should be fine. This first curve is a baseline. The more tests you get over time, the more we will know if she needs insulin and if so, how much. You’re doing a great job helping sweet Mitzy!

    0.5 per units per 1kg is Caninsulin general rule?

    She was about 4,3 kg and now 5,5 after she gain weight from insulin. At least the vet said this. Well, the insuline doesnt depends also on the glucose values? I feel like i damaged the cat with these 5 units every day from 19 feb to 19 march, when she did the hypo episode. Vet insisted to continue with 5 units but I lowered the dose. On June 1st he did glucose curve and he said yeah keep going with 1,5 morning and 1 evening. After a while I lowered to 1 and 1. Now I don’t know, I am highly stressed that I am killing the cat with my own hands.

    I too am having similar issues with my cat but your cat is 5.5KG they are goner need more then 1 and 1 units per day.My cat is 8KG and has been put on 3 and 3. At first Caninsulin was really messing her up but it appears they get more adjusted to the insulin over time and its best to work up to the dose required. she was on prozinc 3.5 and 3.5 a day and was doing very well on it but due to the pandemic they can’t source it atm.

    I highly doubt 1 and 1 is doing any harm - I had this last year when my cat was first diagnosed we worked up from 1 and 1 to her required dose.

    I was at my wits ending with worry but as long as there is food around she would hopefully go to it if she felt too low - It does get better and you will feel more confident as time goes on

    But you must get consistent with your doses then they can do a test in 2 weeks about how your cat has been, stay on low does for a week then get the test done and up as required in small amounts.

    On other places I find that a cat staying 4-8 mmol/l is considered safe, but on our spreadsheet the green ends at 5.5. This means whenever she's higher than 5.5 she needs insulin?

    Since you are just beginning to test and having some trouble doing it, I wouldn’t shoot if she’s below 11. I’ll tag a few others that know more about caninsulin and dosing. @Deb & Wink

    I did 0.5 units at 9:40 am. Usually I do it at 8. So the PM dose is due soon... I basically bought the alphatrak because I felt that the cat doesnt need insuline anymore, but now I am not sure the strips are ok or not. I did the control test (with the control substance) and it falls under what’s on the box. Maybe I’ll try one more sample and that’s it for today, poor cat, I poke her so many times.

    I don't feel comfortable suggesting a dose when the test strips being used are not the ones designed for that meter.

    We do NOT recommend using test strips in your meter that are not made for that meter. The knock off brands are not as accurate and you do NOT know if the code you are using for those other test strips are giving you any kind of accurate results.

    You'd be better off buying a human meter. All our protocols are written with using a human glucometer in mind. Most Canadian drugstores will have meters for a low price. It's the test strip cost that gets you.

    I asked 3 vet cabinets and no one had AT strips to sell me this week. At Petsdrug online store they are out of stock.
    Well I guess with what I have now the curve I tried today was useless.
    Human glucometer requires more blood quantity?

    A good meter that people in Canada buy is the Contour Next One human glucometer and the matching test strips. It has a really nice feature. It lets you add more blood to the test strip within 60 seconds if your first blood drop was not big enough. So you have a second chance for a good test with only 1 test strip.

    "Always give the cat the insluin dose unless they are sick"

    This seems to be a common theme across a lot of the literature online

    I would give her .5 and .5 everyday for a week and then go to vet and see how she has done.

    Hello, thank you! I could do that, but how exactly the vet will check how she was during the week?

    They take appointments after one month, due to corona it is very crowded, as before they took only emergencies.

    Surely they could see you if you explain your concerns?

    And if she is in remission her body would hopefully counter act such a small dose of insluin if she goes to low

    Or do a month of .5 and .5 but it would be better a week or two then vets

    And see how she is - her drinking, weeing, eating etc.

    I understand that I need the AT2 strips, but I cannot find them, only on Amazon and they deliver at the end of month. I’ll try to call more vet cabinets tomorrow.

    IF these would be AT values, I would inject or not? Can I please get someone’s opinion on an if scenario? The values are in the blue zone, but on different websites I find different values as being considered “normal”.

    we will go to vet when she has the appointment, but this was the whole idea, for me to test at home.

    i took the kit from someone that used it on their dog, along with the strips. I bought the control solution new, the strips were in a new box. I’m new to this, I didn’t had all the info.

    It’s not a problem for me to inject her insulin, I just feel I did it already more than she needed. Maybe I’m wrong.

    Strips from Amazon are coming at the end of month, when is also the vet appointment. Paid $120 for 50 strips, it’s a lot, but I do want to get accurate results. With human meter I won’t be OK.

    Meanwhile I really don’t know what to do meanwhile, if I will damage her kidneys if I don’t do insulin 2 weeks, or if I should blind inject 0.5. I am scared of the hypo and her weak back legs.

    I would keep testing with the strips you have, or get a human meter to use until the AT strips arrive. If her back legs are weak, some methyl B12 along with some folate can help. Most of us use Zobaline for diabetic cats, but I’m not sure if it’s available in Canada. I’m not comfortable giving dosing advice. I’ll tag @JanetNJ Her kitty used to be on caninsulin so she is more familiar with its action.

    If you can't test and are worried about hypo then I'd do 0.25. He came really close to the bottom of normal on that 0.5 u. Your cats numbers are in normal range most of the time.

    Can you add more information to your signature? Click on your name at the top right corner and choose signature. Add info such as your pets name, date diagnosed, insulin type, food you're feeding and any other health concerns.

    Start giving b12 methylcobalamin supplements to help with the neuropathy.

    I am testing for 3 days with AT2 and Petsure strips, but Deb&Wink said it's better to use the AT strips. Not only because she said it, but I also wanted to at least test the difference between them.
    I used the code 22 as it is on the PetSure bottle, but then saw a video and they said it's better to use code 4 (for AT turbo) because it's more accurate, as code 22 tends to show lower results.
    I did now the test and used two strips and each code, first time I got 4.4 and then 5.1. Same drop, I hit by mistake the big vein and she had more blood than usual. So it's true, code 4 is more accurate. I used this one since yesterday.

    I think I'll pass the injection, I will see in the morning how she is. Thank you so much, Janet! I almost finished the Petsure box in 3 days :D there is definitely no way I could use $120/50 strips forever, but I hope the results will be similar to Petsure.

    7.2 this morning, no injection. Should I be worried that she wasn't lower in the morning or it's fine? Thank you everyone for your help!!

    7.2 is a normal range number on an Alphatrak. It's a good number esp after not having a shot yesterday. Looks like your cat wants remission. :). Crossing fingers for you

    Hello, still no AT strips, waiting for amazon delivery. Continued to monitor with AT device and PetSure strips. Today we got from the pharmacy a human device One touch verio for free and we bought the strips.
    AT showed 5.6, while the human meter showed 3.9 from the same blood drop. There is no code on the human. Please if someone can help and tell me if that 3.9 makes any sense. Thank you!

    Hello, today I received the alpha track strips from amazon. The package was super damaged, all the strips were outside the box, pretty disappointed after I spent 120 CAD. Anyway, I measured from same drop blood with AT PetSure strip 5.6, and with AT AT strip 6.3. Updated the spreadsheet with measures I was able to took in the past week.

    Tomorrow morning my cat has an appointment to another vet, not the one that prescribed insulin. Should I ask fructosamine test, maybe, as she had no insulin since July 11? And general blood test? We'll ask pills for the weak back legs.

    Since I stopped the insulin the cat lost weight, basically returning to her weight before we started the insulin few months ago. During the insulin she was bulky, gaining weight.

    At this point I don't know if she was really diabetic and went into remission, or I gave her insulin for nothing all these months... who knows what organs I have damaged.

    If you could give me some ideas, please, what to request tomorrow from the vet that will be relevant. Thank you for your help!

    The vet was impressed that I was able to test at home. Thanks to this forum!
    He said it might be honeymoon phase and that I should continue to monitor her glucose levels. He didn’t like that we used caninsulin as other insulin would have been better. I agree, but at that time I didn’t know.

    They did a general blood test and later today the clinic said the results are really good.

    Vet didn’t recommended fructosamine test as he said it’s more suitable for cats that are not tested at home.

    I am happy with the results, of course, but I feel like we are missing something. Not only diabetes related, but in general. Maybe when I have funds available I will do an echo for the abdomen.

    The petsure strips I believe are quite reliable, but I’ll try to compare them more with AT strips. However it’s not easy to change the code in the same session.

    I’ll probably ask for a teeth cleaning, but maybe I’ll use a dentist vet, not the general one. I’m kind of afraid to stress the cat at this point.

    Sounds like a good report overall! If you have a copy of the bloodwork you can post it here and we’ll get someone to look it over and see if they notice anything the vet might have missed.
