1/25 Logan AMPS 295 - at Vet's office

Logan's eyes got worse overnight. Both are now hazed over. We took him to the ER this morning and they're not sure what it is. They're trying to get him in with their opthamologist. Two possibilities are edema (fluid build up) and an infection of some kind. They did say they don't think his vision is impaired. They're concerned that he's not putting up a fight, at all. He has quite the bad rep at their office. He seems to have given up and that really scares me.

He did eat around 3 am and this morning. This morning, he ate about 3/8 of a can of FF before his tube feeding. I just feel like we can't win. I'm pretty depressed and angry right now.

Lots of feel better vines and good energy for Logan to start fighting. Tell that kitty how much you love him and you don't believe it's his time. He's eating, which is a good sign that he hasn't given up. He may be a little overwhelmed by it all but that's what you're there for.
 
Logan has been through a lot and he is probably very confused about what is going on....
I am not surprised that he isn't putting up a fight...he has been so good with you and all that you have had to do for
him over these long weeks. He is in a weakened state....just send strength to him and your love...

Oh Kelly. I'm glad you took Logan to your vet. I hope they figure out what is wrong and help Logan feel better. All fingers and toes crossed that it is something straightforward that is easily treatable. (((Kelly and Logan)))

Yes Logan is probably tired from all of the fuss and "procedures" that he has been going through.
Snuggle him up, talk to him and tell him how proud you are that he is being a good boy.
Maybe he just knows that you are totally devoted to him and doing everything you can to make him better by being good at the vet, maybe he is thanking you and is eager to do his part.
Try to focus on the positive.
We are thinking of you. Please update us with any news you might have.

Latte and I are thinking extra special good thoughts for you and Logan today.

Please let us know as soon as you hear anything from the vet.
Maybe you could ask them how much water is recommended to put through the tube on a daily basis for good hydration with the pred, because it can cause fluid retention. hopefully that is all it is...and they can give him something to reduce the fluids.

I just talked to the ER vet. The good news is that Logan's ketones seemed to have resolved - his electrolytes were normal. She said that if he was going into DKA, they would be off. Since we got the neg/trace reading this morning, I didn't think they were the issue. His glucose was 296. He saw the eye specialist and he said it's something caused by his high lipemic index. Logan is eating a high calorie, high fat food and since his diabetes is uncontrolled, his body cannot utilize the fat deposits like it should and the excess is spilling over into the fluid around Logan's eyes. They said it's pretty benign, but may keep occuring until we get him under control - IBD, diabetes, diet, etc. Right now, it's more important the he gets the high calories every day to reverse the liver damage. I have to put drops in his eyes for a while and the haziness should clear up in a couple of days. No idea how we'll manage that one!

She did say he finally growled at her when she put him in his cage so that made her feel a little better. We can bring home anytime today.

I just feel so awful that anytime we see a bit of progress, we take several steps back. I just hope he knows how much I love him and that I'm trying to everything that I can. I just hope and pray that it's enough and my little baby will fight through all of this. He's been through way too much to not even be 6 years old yet. :(

eating on his own, and growling, are both good signs.
is he on any liver supp like SAM-e?
 
hi kelly. i saw the word vet in your post title so i stopped by. i do not know history of all that is going on here (post DKA or just flirting with ketones, now HL cat with feeding tube?) but after reading first post i was going to tell you not to worry about the cloudy eye thing, but see you went to vet and got answer already. yeah that happens sometimes with HL cats, it resolves later, looks far worse than it really is.

question~ you are measuring ketones via urine test strips? i suggest if you possibly can to get a precision xtra meter and their blood ketone measuring test strips. the ketones are i blood long before urine (kinda like comparing testing glucose in urine compared to a BG) and you can get a far more accurate reading.
another question~ i see on SS there was surgery on 7th, that was just feeding tube placement right?
and last question, have you had an fPLI test?

don't be discouraged, HL just takes time to clear up. the liver is one of the few organs that can heal 100% which is good news, but it is a slow process. you have a feeding tube and that is the most important tool. food is medicine to the sick liver. he is young and is fighting.

Hey JoJo, thanks for stopping by. Logan had exploratory surgery almost 3 weeks ago. He hasn't wanted to eat well for quite some time and I finally was able to get referred to an internist. She did an ultra sound and found that his liver, spleen and intestines looked inflamed - this was early Dec. She aspirated some cells from the liver and spleen, but they came back negative for lymphoma. So, she put him on Metronidazole/Pepcid/Vitamin E for a couple of weeks, but he didn't improve. His inappetance got worse even with a stimulant. She did a full GI panel through Texas A&M and everything came back normal except the folate test. She said he had inflammation in the small intestines but they weren't sure if it was IBD or lymhoma. They got biopsies of his liver, stomach, pancreas and small intestines. Everything was normal except the small intestines biopsy - showed moderate IBD but no lymphoma anywhere.

So, we started him on prednsiolone last Wed (1/20). He's been very insulin resistant since going back on insulin in Sept so I monitor his ketones throught he urine. He was negative on Tuesday. After starting the pred on Wed, his BG actually came down a little into the mid to upper 200s and he seemed to feel better. On Thursday night, the strip was bright pink the second I pulled it out of the urine stream. We increased his insulin and started pushing through extra water. Since we had the feeding tube, they thought he was ok to be at home. The ketones did go down over the weekend and he seemd to be feeling better.

I did buy the precision meter, but couldn't find the strips! I finally found them on-line - they should be here today.

He won't eat much of anything on his own. He won't touch the Wellness or Merrick anymore. He's eating the hill's a/d right now through the tube and a little FF on his own. They said that he most likely won't ever eat the food that I push through the tube.

i see you tried some wellness core last night, that was dry food? did he go for it?
kinda of a catch-22, need him to eat, yet at same time raw only is what would be best for IBD. have you tried any raw at all (i have known cats that eat raw with gusto and are fussy and only pic at any canned food so i have to ask).
but this is what i'm thinking, have pred on board, flirting with ketones, numbers are high, possibly are going to have to add dry food to diet to keep him eating...this looks like a job for R. *in flies a bottle of R with red cape flowing*
what do you think...you game?
 
If you are going to consider raw food and/or dry food is more appealing to Logan, then maybe you could try freeze dried raw food like Stella & Chewy's. Quincy goes crazy over it. You could break it into little pieces and feed Logan dry chunks, or add water to it and rehydrate it a little.

It was Wellness dry...he ate a couple handfuls and seemed to like it. I was just trying it to see if it peaked his interest, at all. Since his immune system is suppressed, does that increase the chances of him getting an infection from the bacteria in raw? I haven't ever tried it, I'm not sure he'd go for it or not. It's worth a shot. I'd need a script for the R, right? I'm not sure the internist would give it to me, but I can ask! Would that just bring him down a bit so the lantus could have a chance to do its thing?

No script needed for R :) But because it can be so potent in bringing down BGs, you'll need guidance from Jojo or some of the high-dose folks on how/when to use it. R isn't something to take lightly, but is a great tool when used correctly.

Logan was so happy to see us that he actually purred at the vet's office...and in the presence of one of the techs! He hasn't purred at a vet's office since he was hospitalized when he was one (when he first diagnosed with diabetes). Then, he would purr as soon as he heard my voice. So, I think he was happy to be getting out of that place. He ate a little when he came home. He's been sleeping since then. We have to do eye drops twice a day for two weeks. That's going to be fun.

He had trace ketones this afternoon when he got home. So, I'll keep flushing his tube with some extra water.

I'll have to learn more about the R. It sounds a little scary! But, worth considering.

He might rest after he cools off...he's pretty p*ssed about the eye drops! Only 26 more times to go...well, 52 since it's one in each eye. This is going to be fun!

He was a bit more active tonight, so maybe he's feeling a little better. He didn't eat anything on his own tonight, but I'm sure he'll wake me in a few hours. I don't mind, at all, though! I'm just happy to see him eat, even if it means getting up in the middle of the night.
