Monty having pain when eating

Monty has been in remission for a few years now, consistently healthy BG numbers with monthly checks.

He's got a few chronic health issues: hypertension managed by Amodip that's stabilised, stage 2 CKD managed with diet and Ipakatine (SDMA 17, then 25 8 months later and then 21 6 months after that). He gets very occasional (once a year?) bouts of pancreatitis.

2 weeks ago he presented with pain when eating and poor appetite, he was very inactive and sleeping most of the day. We took him to the vet and blood test showed elevated lipase consistent with pancreatitis. He was referred for an ultrasound and given Buprenorphin to manage his pain. The ultrasound found a 2.6cm mass near or on (unclear on images) his pancreas. The vet has referred us to a specialist for a follow up scan to learn more about it.

The vet also noted his gums were inflamed and that the mass could be new or it could be 10 years old and more diagnostics are needed. After a week on Buprenorphin his condition improved. His appetite is back but he's still clearly in pain when eating. Both I and the vet now think pancreatitis has passed but dental issues could be his primary concern now. He's booked in to have his dental issues resolved under general anaesthetic in 6 days. He's still grinding teeth when eating but clearly shows an interest in eating but gives up quickly.

I've asked the vet if we can bring forward his dental surgery or refer me somewhere else with availability, he's currently only eating 25%-75% of his regular intake and I need to give him Thrive kibble to get him to eat at all (~10% carb dry food) that I wouldn't normally feed because the phosphorus levels are not good for his CKD. But eating the wrong food is better than not eating.

My primary concern right now in priority order is:

1. Keep him strong enough to survive the dental surgery
2. Gain weight back after surgery
3. Investigate mass near pancreas
 
Just doing some Googling for some nice smelly fish-based food that's soft or that I can put in a food processor and add water to encourage him to eat over the next few days until surgery.

Do you think he could be nauseated?
Are you still giving him the bupe? That could help with the pain if it is caused by the teeth.

Thanks for sharing the link! He ran out of Bupe last night but vet is prescribing more that I'll pick up later today and continue 3 or 2 doses per day. He's also prescribing some mitirzipan. In the meantime I've bought a few different flavours of Sheba food for him to try that he wouldn't normally have. I don't think it's nausia. He's behaving much better after being on Bupe for a week. He's jumping up on the sofa again (~3 feet), scatcing my furniture and being his usual affectionate self - it's only when it comes to eating that he's grinding teeth and eating slowly.

If all else fails I'll just have to feed him the dry kibble sprinkled over of his regular food so he at least gets some of what he normally eats (KatKin steamed meat).
 
If it’s his teeth it will hurt to eat but hopefully the bupe will help. I would try not to give any high carb dry food if you can as you don’t want him falling out of remission. But of course he has to eat! That’s the most important thing. let us know how it all goes

The dry food I have is 9.6% carb so it's borderline, it's not a big deal if I feed it dulited by some of his regular wet food.

Prescription came through and I'm going to pick up his drugs in about 3.5 hours.

That’s good the dry is not high carb. And I’m glad you can get the drugs today.
 
I've got Buprenorphin for his pain to be given every 8 hours. I've got Mirtazapan for appetite stimulation. We just tried to give him that in a treat and he wouldn't have it. We tried pilling him but he kept holding it in his cheek and spitting it out after we were sure he'd swallowed it. I think through our attempts he ended up eating probably a half dose but the other half was wasted. Will try again 48 hours.

He's also been given Zodon for his mouth to be given twice a day (https://www.vetuk.co.uk/pet-meds-pr...mg-ml-oral-solution-for-cats-and-dogs-p-20881).

He's now very annoyed at me.
 
He did well yesterday evening and ate a decent amount of food, even managed to get him to play with his laser pen.

I gave Zodon this morning but unfortunately it made him retch afterwards and then vomit a small amount (including his Amodip). I've emailed the vet but likely won't continue with Zodon if it's just going to make him vomit.
 
My most recent diabetic, RK, absolutely couldn’t tolerate clindamycin (drug that’s in Zodon) and would violently wretch and vomit after getting it. She had significant dental issues so her reaction was unfortunate, since clindamycin a great drug for dental issues. But it can be harsh. I had to stop giving it.
