Hi all! First post here as my cat has been newly-diagnosed with Diabetes. I’ll pop a bit of a short history below, as Ted seems to check some boxes of diabetes but not others so I’m hoping to talk to others who have experience with diabetes!

Initial vet visit in November last year. He had gingival stomatitis (or something along those lines) and essentially needed all his teeth removed. Vet drew blood before the op & found high BG (in the early 20’s if I remember) but the Fructosamine test came back high end of normal. Tested urine and there was glucose present but no ketones, no protein and the specific gravity/concentration was normal.

Vet suspected diabetes so surgery didn’t go ahead and he opted for a few weeks of monitoring (bringing cat in for glucometer test & urinalysis). In this time Ted went downhill with his teeth as he was having a bad reaction to the gabapentin prescribed to keep him comfortable. One week into monitoring I took him in and explained he wasn’t a happy cat on gabapentin. They took his urine & blood. BG was lower (18-19 I believe), fructosamine came back lower, glucose still present in urine but again no ketones and SG was normal. He had put weight on. The vets then believed that maybe Ted had stress-induced hyperglycaemia from the pain in his mouth and for his welfare did the surgery 2 weeks later (earliest appt). In the mean time he was put on some opiate painkillers and he thrived!

Surgery was 9th Dec. Again blood draw before surgery said high BG (around 20 I believe).

I took him to the vet on 3rd Jan as it looked like a small infection was starting in a canine and he seemed to have lost condition. Vet repeated glucometer test and it came back at 30!! He had also lost over 400g (he weighed 4.7kg previously and was down to 4.3kg). His urine had lots of glucose but no ketones, no proteins, normal specific gravity.

The extreme BG caused the vet to prescribe Senvelgo. We started on Monday (when the order arrived to the practice). I took a urine sample today for Ted (morning wee, will have had 2 doses of senvelgo) and took it into the vets. Apparently there are trace amounts of ketones now where previously there was zero. Still lots of sugar in his urine and the SG is still within normal parameters.

I’ve ordered some urinalysis dip-sticks so I can keep a close eye on Ted at home going forward (alongside having a vet on board!).

Is there anything I’m missing here or doing wrong? I just want to see my beautiful boy well! Are ketones 2 days after being on Senvelgo a bad sign, even if it’s a small amount?

Welcome to FDF, I will have someone more knowledgeable on the Senvelgo but the glucose is checked with blood, I have not ever heard about urine, it is tested with blood by using an insulin pen or like a ReliON Premier human monitor and strips, example, if your Vet did not suggested home testing, you should, in order to know how is your cats glucose is before you give insulin is a must to avoid hypoglycemia, I will tag several members that can guide you with Senvelgo insulin and dosing, please ask your Vet if he is familiar with Feline Diabetes, many Vets are not and they treat FD like dog diabetes, and is not the same. Please continue to as we are here for you no concern is a small one, you will be contacted soon, to update you, and do not worry you have not, most of us at first when we hear Diabetes is overwhelming and we learn to deal with it eventually, you are in the right place for guidance, with your help we can make take furr baby back to being himself again:bighug::cat::cat:

If I were you I would stop the Senvelgo NOW. I don’t think your kitty should have been prescribed that with the other issues he is currently experiencing with his teeth. And the appearance of ketones is a very worrying sign. Ketones is one of the complications that can appear with this drug and can rapidly develop into diabetic ketoacidosis. I don’t want to frighten you but you need to act immediately. Don’t let the vet talk you into continuing with this drug. Your kitty may need insulin.

Thank you so much for your advice. I will be phoning the vet in a few hours when they open and stopping the sengelvo, as I felt the same way (that ketones are a bad sign showing up so shortly after commencing use). I’ll update after I’ve spoken with them!
 
If your kitty needs insulin I would ask for Lantus or Prozinc. I prefer Lantus they also have a generic form of Lantus. Then the members here can help you out with more information

I am so glad you have seen this post. I have been worried about your kitty. Please give your kitty lots of food to eat starting now, to combat the ketones and if he will let you add a teaspoon of warm water to the food, do that too.
