? Shorshe: Anaesthesia for CT and no reduction in BG with LC dry food

I have consulted the small animal hospital at one of the universities in Bangkok. I have an appointment with an endocrinologist about the Diabetes. If she okays the BG values, Shorshe will have CT scan and extraction of a sample of the growth in her ear. They will decide after that if she needs surgery.

Meanwhile I have switched Shorshe to Dr Elsey's clean protein. However, her BG has not drastically reduced. It holds steady at 200s. Occasionally dipping below 100. Does anyone have any explanation for this?

Additionally she was throwing up frequently, so I did her folate and cobalamin and full blood panel. While other parameters are fine, her cobalamin levels were below normal. I have given her mecobalamin injection on the weekend and plan to supplement orally as well. I have updated the folate and cobalamin levels on the labs part of her chart but not the rest yet.

Probably because of the low levels of b12 she has been reluctant to eat, and I am having to give her mirtaz every 4-5 days (even at 1/16th of 15 mg tablet, she gets pretty focussed on food). |

Any dosing advice? Generally, vets here are very stubborn about prescription diet etc. Any articles anyone can share that I can show them about LC food, esp wet food? I know that RVC london has some articles on the topic but I am unable to download any of them (paywall). Any help, advice, dosing and food related help much appreciated.

We had similar numbers late last year when we were on Dr. Elsey’s. I forget if it’s the carb percentage of the Elsey’s or just the nature of dry food that was the problem. We saw a nice shift down in numbers by going to lower carb wet.

From what I can see the website gives dm carb % of Dr Elsey’s at 1.97 and 5.36% depending on the food. I am also trying her on young again mature (i saw the brand recommended on Dr Lisa’s site as you try to shift to a wet diet) but she doesn’t seem to like it as much and eats a lot less.

Unfortunately complete wet food available locally are all above 10% carbs. And fancy feast and other imported wet food are between 8 and 12 usd per 85gm can which is exorbitant especially when you compare to locally availble commercial wet food which about1.2 USD per pack.

while shorshe will eat some of the complementary tuna wet food (I found one called regalos here which is about 9% dm carb), she won’t eat more than 1.5 packs a day which is less than 90Kcal and her RER is about 200KCal.

Could you point me to the row/column in the Labs tab where that appears? I can't find it.

Maybe the endocrinologist can help with the vomiting/low cobalamin too. They might want to do an ultrasound to check for IBD or something else. If Shorshe has an allergy to a protein, chicken is a common allergy. If that is what is in the Dr. Elsey's, it might explain higher BG values. We've seen more than one cat get a food bump from Dr. Elsey's, but not all so sounds like it's something that impacts just some cats. But with no recent spreadsheet data, we can't help you with dosing.

Folate (Normal levels: ng/ml 10.4-20.7) 17.2
Cobalamin
(Normal levels pg/ml 290-1500) 144

Her USG report is also on the lab's tab on the spreadsheet (here is the PDF: https://drive.google.com/file/d/1UaiZU5OwMis7vPocvmbROOmfiwiczrV7/view?usp=drive_link), and everything seems okay except the slight enlargement of the portal vein (though the portal blood pressure is normal).

Also, the spreadsheet is updated as of today.
 
If Shorshe has an allergy to a protein, chicken is a common allergy. If that is what is in the Dr. Elsey's, it might explain higher BG values. We've seen more than one cat get a food bump from Dr. Elsey's, but not all so sounds like it's something that impacts just some cats.

Dr elsey’s has a salmon variant that seems to have hydrolised protein and no checken and under 2% dm carb. I will try it next. She is currently having the rabbit and chicken.
 
Has the vet talked about B12 shots? She definitely needs supplementation, and hope that helps her appetite.

See if the vet has Mirataz, it's a gel version of Mirtazapine that you put in their ears, and very easy to modify the dose.
 
I gave her a b12 (mecobalamin injection 250mcg) shot a week back and vitamin bcomplex shot 3 days ago and she is also receiving oral b12 everyday. Te gel version is not available here but I give her 1/16th-ish of the mirtaz tablet (mirtazapine 15mg). Any higher and she is very shouty and eats like a maniac. This dose on alternate days works best for her.
