? Theodore Insulin Transition 12/14

Hello,

I switched Theodore to Lantus as I was finding Levemir a little tricky and it seemed it was bouncy plus I understand it was getting discontinued in Canada. Part of me is also frustrated as times I will check his glucose and it will show something like 4 (72) then I recheck with the same blood and shows up as 8 (144)...I thought 5% variance is normal but this far beyond exceeds that.

In any case I switched to see if we would have better luck. I was told to start with half so anywhere 2-2.5 since my vet office doesn't do quarters. I was curious about this as he was previously on much higher doses on Lantus and also DKA previously.

His numbers at 2 were horrible and he was acting not himself. I upp'd it quicker then I should have given his state and even 3 units he was having a poor appetite. He is acting like himself again on 4 units of Lantus but his numbers are very strange and his nader appears to be +3 but even last night I gave him insulin and at +1 he went from HI to 8.8 (158). Does this appear normal? Is he bouncing? He drops so quickly it's concerning but then his numbers are so high towards the end of the cycle. I know it's an adjustment with levemir having nadirs at the end of the cycle but I thought Lantus was supposed to eventually close the range and not be all over the place like this

Could you put a blank line in your spreadsheet after the last day of Levemir and label it "restart Lantus", so I know what part of the spreadsheet to look at? You need to do that for the US tab manually like you have done for the World tab.

Are you still following SLGS? You didn't wait long enough at each dose before increasing and missed testing several nights (8-10th), which was too fast even for someone following TR. It also looks like you got some +11's but not preshot tests. Safety first. How he did on Lantus ages ago doesn't matter now, as insulin needs can change over time. That 88 means the 4.0 units is too much if you are following SLGS.

If tests look wonky, like last night, you should retest.

Thank you @Wendy&Neko . I did 3.5 units tonight as he was already low (184) and drop really quick on Lantus it seems. Should I be lowering it further in the morning.

Yes I am following SLGS just had some confusion now that I switched. I panicked early on when switching as he wasn't acting himself and lethargic.
 
That +2 tonight looks oddly out of place. I would have retested it. I'd hold the 3.5 units, unless he goes too low on it. It might be that his better dose is 3.75 units, but give 3.5 units time, then decide.
