I was out of town and my pet sitter stayed at my house while I was away. Keiko has been on 10 units 2 times a day and he is not doing as well as when he was on 10.5 and 11 when he hit some normal numbers.
I'm doing an Alphatrak curve today. So far, numbers are not in normal range. The good news is he is so far not in the red zone. Since I returned he has been grooming himself more, he's more active and alert.

My goal in treating him is to get him to normal numbers. I spoke to a different vet office near a neighborhood where I am considering a move and they use Vetsulin. I wonder if this might work better for Keiko.

Vetsulin was not formulated for cats. It was formulated for dogs. It's very harsh. You might want to mention it in your subject line and see what others have to say. I can't think of anyone who's switched from Lantus or Levemir to Vetsulin (also called Caninsulin

Most people with high does cats move to the longest lasting insulin they can get. In theory, lantus and Levemir have similar duration lengths but in practice it kind of seems like most high dose cats seem to get better duration from Lev. I would not switch to a shorter acting insulin. With high-dose cats especially it's hard enough to control their blood sugar without having it not last long enough. It looks like Keiko was doing better when you were around...I think it was January 19th or so.

Hopefully you can get some mid cycle test soon so you can see how low this dose is taking them. Most kitties need regular changes in their dosing and that will let you know which direction to go.

Vyktor was started on caninsulin, it was horrible stuff. Big dive to start with from the harsh action and then straight back up as it wore off too quickly. He was spending the majority of his cycle in high numbers on that. His vet had never used lantus but they'd never had someone home testing either so when I showed him the way the caninsulin was working they agreed to change. Vyktor was not high dose, I expect the results would be even worse on a high dose cat.

Neko started out with Caninsulin/Vetsulin - I would never go back there. It didn't last nearly long enough in Neko, petered out after around 8 hours, leaving her in high numbers for at least 4 hours each cycle or 8 hours a day. Some people get around this by shooting 3 times a day, but that's awfully hard on the caregivers. If you want to try a different insulin, I'd think about Levemir as Julie already suggested. The thing to keep in mind is that the Levemir onset and nadir is later, so the getting mid cycle tests usually means around +8.
 
Thanks for the information. Seems like this vet is not as up to date as my current one.

I'm not sure K was doing better before I left. I left Feb. 10. He actually seemed better in terms of behavior and his coat when I got back! Maybe we both needed a break from the sticks and injections (although he continued getting injections!). Maybe I needed one!
The sitter I have is great with both cats. She can give the shots and is comfortable doing so. I felt better with her staying at my place too. I know the cats quickly took a liking to her which they don't do as a rule with all people. She plays classical music for them.
