? +6 508! Need advice

Little one is still living in his high numbers and I wonder if his body is freaking out at the 6units of Lantus despite the fact he seems to need it based off of old data.

I left a message for the vet about a week ago and just called again, but she doesn’t seem to know what to do and I haven’t had the option to speak with her yet. He has his specialist dental appt on Thursday, but I’m afraid they’ll turn him away with these numbers. He seems uncomfortable, but he’s still eating, using the litter box, etc. I look back at when he had blues and yellows and am worried it just went away. I guess it is possible he’s on just too high a dose and I haven’t been able to get an appt with the vet or get a call back for blood testing for cushings or acromegaly. I don’t know what to do with him and it’s really mentally wearing on me in an already super busy work schedule as of late. I’m wondering what next steps are if the dentist vet turns the surgery down? Is it even fair to him to get an FME? :(

SS has been very high since January. I have no educational advice for you- Maybe @Wendy&Neko, Wendy is available and could pipe in again- Little One SS she has experience with high numbers and high dosing.
 
If a FME is what he needs and he is in pain without it, then yes, it is fair. Kitties seem to fine, after some adjustment, without their teeth.

Other than the jump from 4 to 5 units, you have been increasing by small and safe amounts. Other than a few missing evenings, you have been testing enough to catch any low dives that would say this is too much insulin. Therefore, this is not too much insulin. You have however, been holding some doses a long time. The nature of resistance is that you need to keep on top of it with increases. I would test for IAA, insulin auto antibodies, as well as acromegaly. Both blood tests go to the same place, Michigan State University.

Are you testing regularly for ketones? With these sorts of numbers I would test daily, if not more frequently. And keep getting spot checks at night every night. He could be seeing low yellows or other number he isn’t used to at night, then bouncing hard. Neko’s SS got a lot of black in it when she finally starting seeing low blues and even greens.
 
I tested the ketone strips against his litter pad tonight and they showed up as very pale pink (corresponding to “a trace”). What should I do?
 
Trace is still ok, anything higher would be a concern. Just keep checking daily if you can, to make sure it never gets darker than pale pink.
 
Hi there! With stomatitis in the picture, there could be pain and inflammation which would be causing some of the higher numbers. I am glad you have an appointment on Thursday. After the dental and any necessary dental extractions that should help the higher BG come down. I hope that the dental specialist recognizes this and doesn't delay your dental visit, otherwise, you are in a catch 22. Inflammation = higher BG's. Good luck and be your kitty's squeaky wheel and insist for the upcoming dental appointment.

THANKS to all. I don’t know what I’d do without this online group. I’m not sure my vet knows what she’s doing, and I’m happy to be going to this specialist Thursday. I’ll be sure to advocate for the FME despite high numbers because it IS very much a catch 22, and I only think it can get worse. My question about the FME was to see if it’s worth him going through the surgery if it will cause suffering that may be unneeded if things are close to the end. I dont think they have to be, and I think he still has fight left in him, but I’ve felt very unsure of things lately, and never want him to suffer. He’s quite skinny, but is still eating, using the litter box, mobile, still purrs for me, etc. It’s just me and the two boys, who’ve been with me for 12 years since they were kittens. I want the best for Little One and wish I could know how he feels and what he wants!
 
Also my vet is the same one who is insisting on keeping the dose, so I’m trying to do a balance between what I’m hearing here and from her. She says three weeks, but with SLGS, can I move it once a week? Should he be at 6.5 now? Also keen to see what this doc says Thursday too.
 
Yes, holding the dose just one week is fine. That's plenty of time to build the depot and see what the dose is doing. Go ahead and increase.
 
I’ll wait til Thursday to do the increase now depending on how the surgery goes. I don’t want him to spike before tomorrow morning because he got a lower number finally this am. I have to give him half a dose with a bite of food before the consult tomorrow. Crossing fingers he gets okayed for surgery!
 
I would wait for the increase until Friday. Some types of anesthesia induction drugs can lower the BG. If he’s slow to eat after extractions, you don’t want to make it worse with an increase that night. Make sure you get a good supply of buprenorphine for the pain.
