Suspected Possible Diabetes

    Hi, Tiger he's 17 for about a week now has been drinking more water and acting a little lazy, not lethargic. Haven't taken him to the vet yet. I checked his glucose at home(I have a sugarcat named Jazz Man) with Reli on meter. It read 450 about 15mins ago. I'm going to give him his dinner in about an hour. Should I just go ahead and give him 1 unit of Prozinc while he is eating? He doesn't have a problem eating ff pate. We know he's high right now. Or should I wait a half hour after he eats and retest. Then if still high give him 1 unit or .5 to start with? And recheck in a couple hours?

    I wouldn't start insulin until he has been checked out by the vet. An infection could raise bg. Just keep feeding low carb and hope his bg can be controlled with diet alone. I think I would start with only .5u. It's easier to find just the right dose if you start low and make small increases until you reach b the right dose. You will be less likely to have hypos that way.

    Thanks yeah I gave him .5 about an hour ago. If he has an infection would his bg stay high even with the shot?

    Checked his temp no fever. You wouldn't even know his bg was high, he doesn't act like Jazz Man did when he was first diagnosed. The only red flag was he is drinking more.

    It's been 5yrs since I had to start at the beginning. Jazz Man is regulated.

    I know what you are going through. My Callie's twin brother ,Patches, was showing symptoms of having diabetes. I took him to vet and she found he had a UTI.I had been testing his BG and they were running between 200 to 410. Vet didn't want to start him on insulin until she had all his blood tests back. We are hoping it can be controlled by diet.He had blood drown last October and all was good then. Of course anything can happen with a 15 year old cat.I should know tomorrow what vet recommends.
    That's why I suggested waiting on the insulin. I will start on Lantus when I have vet's ok.

    He may come down some, but still be above normal numbers. 450 is pretty high and 0.5 is a low dose. Since you are experienced and can test, it was fine to shoot, but I would still get him checked by the vet. And I would also check his urine for ketones.

    Maybe create a spreadsheet for Tigger, and we can follow the blood glucose readings and help you out.

    Great directions on how to do that are here.
    No waiting after eating to inject insulin when you are using Prozinc.
    For Prozinc the order is test, feed, shoot in a short window, 10-15 minutes if you can get the routine that short.
    You only want to /need to do that "waiting after eating" with the faster acting insulins like the NPH and Vetsulin/Caninsulin insulins. That wait is so the food starts to be digested, processed and can get into the bloodstream to intersect the onset of those faster acting insulins and slow down the steep drop.

    Today Pre- 520, shot .5 and getting ready to do a recheck.
    I also started him on Amoxi just in case it is a infection.

    Ok. Please continue to post here in the Feline Health (Welcome & Main) forum for now. PZI ISG (insulin support group) is lightly viewed, not many people peeking in over there.

    Certainly ok to move over there eventually.
    Would really like to see a SS (spreadsheet) and signature from you first.

    Just tell people that enough is enough. Give you time to digest everything coming your way.

    Remind folks this is not "your first rodeo" and you have dealt with feline diabetes before.

    • Freakouts and nervousness and fear are SOP (standard operating procedure) here.
    • Mindfulness breathing, yoga, tai chi, active sports, are all ways various members deal with the stress.
    • Ok to vent too. No religion, no politics, try to keep the language 'clean', no "vet bashing" are all good ideas.
    • What else do you want to know?

    Just got some pee from his stream. Used One Step Vet-10 strips Ketones +15 and Glucose ++500 all the other tests were normal range

    +5 is 5 hours after the insulin shot
    + hours are always counted from the time of the insulin shot.

    AMPS is morning or AM pre-shot test. To make sure the BG is high enough to give the insulin.
    PMPS is evening or PM pre-shot test.

    We suggest a BG threshold of >200 for new or returning members using a human meter, with Prozinc, AT THE beginning.

    Higher BG at pre-shot test if using a pet meter to test with.
    Until you learn how YOUR cat does on this insulin, on this dose.

    One Step Vet-10 strips Ketones +15 and Glucose ++500 all the other tests were normal range. I'm in a fb group and they are freaking out. Take him to the vet take him to the vet..I'm like..I've been down this road. I've had cats with fiv, felv, chf, uti's and urinary crystals, uri's and have subq'd a zillion times. I told them I want to give the antibiotic time to work and that .5 is nothing for Tiger since his bg is so high. My Vet is fine with this. Talked to her today.

    The only Facebook group associated with this site is the Feline Diabetes Group, not the Feline Diabetes Support group. It may be easier for you to choose one so you don't get conflicting information.

    I just wish everyone would quit telling me to take him to a vet in those other groups. He is one of those cats that get REAL STRESSED OUT, screams bloody murder and pants on the way in. It takes about half an hour to get him to my Vet.

    Repeating what's been suggested in the FD group:

    Giving 50mls of fluids (she has the supplies) to help flush any ketones. Tiger is eating well and acting fine, so the presence of ketones on the strip doesn't really point to going into DKA and with her problems getting him to a vet, thinking giving the fluids and keeping a close eye on him is a reasonable choice.

    Repeat ketone test ASAP.

    Jacque...on most strips, when you compare to the guide on the side of the bottle, it will say "None, Trace, Small, Moderate, Large".....is that what yours have?

    We'll try not to "freak out" too much with you here on the FDMB message board.
    Some of the folks on those fb groups are not as experienced and more prone to "yelling to get your attention" shall we say. Not calm in many situations. Not aware that you are experienced in treating a diabetic cat.

    Chris is more "level headed." As are some of the moderators and admins over on our sister FB page
    the Feline Diabetes Group. Plus a few people like BJ Mattson and Cat Dickerson I would trust what they say.
    SS will be really helpful. Hope Chris has gotten over her "stomach bug" and can help you set it up.

    I'm sure someone has already asked this question. How long does it take for cats to get used to Prozinc? Ya know...until the insulin is built up. Like a week?

    Prozinc doesn't "build up" in the body like the depot insulins (Lantus, Basaglar, or Levemir) do. You should see some sort of curve fairly quickly.

    Going to check Tiger's glucose in half an hour. If high should I up the dose to 1 unit? Or wait one more day? Been 2 days now on the .5 2x a day.

    I'm not a dosing expert, but I saw your question so here I am. You're still getting levels 500+. You haven't had any levels in the green mid-cycle. I agree with Deb to up the dose to 1 unit. It's possible Deb thought you had already given morning dose. I certainly don't think upping to 1 unit this morning would be an issue. Will you be home to keep an eye on your baby during the day? It's unlikely that Tiger will have any hypo issues, but as you know, it's always best to be cautious with a dose increase.

    Ok, not sure when your last insulin dose was.
    What dosing cycle are you in? AM or PM?

    Do not give any insulin more often than every 12 hours.
    Not even if you think you gave a fur shot.
    Not even if you think the dose did not all go in.
    Not even if your cat is running at high blood glucose levels.
    Test for ketones if high BG's
    Prozinc can have a bit of an overlap effect in SOME cats.
    Prozinc can have a duration of between 10 -14 hours.

    ECID Every Cat is Different.
    You need to test and find the onset, nadir and duration for your cat.
    BG testing is the best way we know how to find that out.

    Very risky to give more insulin in the same dosing cycle.

    AMPS is morning or AM Pre-shot test
    PMPS is evening or PM pre-shot test.
    +1 is 1 hour after giving insulin
    +2 is 2 hours after giving insulin.

    SS is the best way for us to see what you are doing with the dosing.

    When you get your SS setup, put notes on there.
    So we know when your AM cycle starts.
    So we know when your PM cycle starts.

    You may have to read back in the post a bit.

    If you have not given the AM or morning dose ,then 1 unit sounds good.

    p.s. Can't wait until the SS is up and running and linked to your signature.
    Noon here.
    But when I dosed my cat he'd get his shot at 7 am my time.
    It's why we use the + hour system. So we know how long it's been since you gave insulin when we read your spreadsheet.
    + hour system limitation is we have no idea what times you give the shot.
    So some comment in your post to let us know the dosing cycle, AM or PM is needed.
    As well as some comment in your post as to how long it is before you give the shot.
    People using Prozinc often find that testing at +11 will let them know if the Pre-shot BG rises a lot between +11 and the next pre-shot test.

    Beginning of 3rd day. 11am his morning pre shot is 545 +12. He ate ff gave 1 unit.

    According to my notes the +11 is always high over 500, the +3(after morning shot) has been the only times that it was in the low 400s

    Let's see if the 1U of Prozinc can budge the blood glucose.
    I'd only wait 3 cycles, before bumping the dose up another 0.25U.
    Don't want you to bypass a good dose.
    Your choice, if you want to increase by more.

    When I clicked to insert the link is real long. Is the link supposed to be real long?

    You can shorten the name your put in your signature. Such as Tiger's SS.
    Then you highlight those letters and click on the little chain icon in the post header.
    Copy the URL that Chris gave you and you have the link in your signature.

    You may need to have a second window open to do this. Or save the Url to your clipboard or to another location.

    Instructions for pasting the SS link in your FDMB Signature block:

    1. Login to FDMB and click on your user name in the upper right corner.

    2. Select "signature”. The signature block is limited to three lines. If you have a lot of other information in your signature block, please be sure you place the spreadsheet link so it is easily located by other members (i.e. please don’t stick it at the very end of a long, detailed description of medications).

    3. Put your cursor in the text box, type in your kitty’s name with the word “Spreadsheet” (e.g. Furbaby’s Spreadsheet), highlight those words, and then click on the “hyperlink" icon above the text box:

    4. A text box will open that states "link"; paste your link into the box next to “URL” and click on “insert”.

    5. This should result in the highlighted text turning light blue which means it is now a hyperlink to the spreadsheet and anyone clicking on it can see the spreadsheet.

    6. Click "save changes" and you're done!! Great job!!!! If you need any help at all, please send me a private message by clicking on “Marje and Gracie” (to the left under Gracie’s photo) and then “Start a Conversation”. I am more than happy to help you or do the Spreadsheet for you.

    I believe the signature is now limited to 2 lines and 250 characters. So abbreviate other things we you can. Or leave them out.

    Never mind you have it now.

    That may be, but found your location, address, phone number, and all sorts of other personal data pretty quickly. Took me < 60 seconds.
    Imagine what a hacker could do with all your personal info.
    It's why we don't want too much personal info in your signature.

    If it's the same one you use in other places, it might be a good idea. Most of us go with our name and cat's name (like Chris & China, Deb & Wink, Wendy & Neko, etc. etc.)

    Just make sure it's right and you like it when you change it...you're only allowed to change it every so often (like every 30 or 90 days?)

    Mostly from your full name and your vague state location that you had in your signature. Before I alerted you to change it.
    Lots of internet sites let you find more info on someone with just a little bit of data.
    Like beenverified.

    Yes, user names can be changed.
    All history will be changed to reflect the new user name.
    All former posts are changed to reflect the latest and greatest info in the signature.

    There is software behind the scenes that links signature data to your post.
    It's not actually part of your post.

    Still high. With Prozinc being an in-and-out insulin, you could consider increasing the dose by 0.25U dose increments until you see better BG levels.

    Testing in both 12 hour dosing cycles is recommended. Many cats drop to lower BG levels in the nighttime, as they are more active then.

    I think Tiger is high enough you could attempt a dose increase almost every cycle. Be prepared to monitor closely with dose increases. Be prepared with your hypo kit and instructions. So you know what to do if the BG (blood glucose) tests/readings/levels are low.
    You might want to practice changing your thread title also. Upper right hand corner of the screen to edit and get the thread tools options.

    In a hurry..I did get 2 readings in the middle of the night(my time) PMPS 360 +3, 375 +6
    I'll check into the other stuff. Yes I have a kit..always lol
