? Triton will be on Lantus this Friday!

Hello Everyone!

Triton has been on vetsulin for about 1.5 months and we are switching over to Lantus!!

I have been reading the a lot of good things about Lantus and I hope we can get Triton regulated fast.

So since I will be starting Friday morning on Lantus, I would like some advice on what dosing method I should start him at? Should I start him at .25unit or .50? My vet recommended 1 unit.

He is about 7.8 pounds and might be getting thinner which makes me worry. He has a vet appointment to make sure there is nothing else other then diabetes.

Well hello there Triton's Mom (what's your name?)! Welcome to L&L Land! My buddy Lou also started on Vetsulin, and we switched over to Basaglar (similar to Lantus), then Levemir. First things first, it's a big change. You have to forget everything you've learned about dosing Vetsulin after you switch (most importantly, you dose Lantus based on how low the BGs go, not on the preshot BG). Thankfully you haven't been on Vetsulin that long, so hopefully your transition will be easier than mine was (we switched after almost a year on Vetsulin).

Keep in mind you choose the dosing method that works best for you. Start Low Go Slow (SLGS) is more conservative with a higher reduction point and performing a curve once a week. Generally folks on SLGS do not test as frequently (outside preshots). Tight Regulation (TR) is the more aggressive method. Your reduction points are lower and you test much more frequently, so no need to perform full curves weekly.

As far as dosing, we take your current dose into consideration when switching to Lantus. You don't need to start over and slowly increase. Looking at your spreadsheet, you are currently at 2.25u, so perhaps you can start at 2.0u if you are able to test frequently. I don't think I would go lower than 1.5u to start. We'll let some more folks with more Lantus experience chime in about dosing.

I think since you just went to 2.25 today, it may be better to start at 1.5 or 1.75

The 2U dose was getting him into blue numbers.

Do you have U100 syringes for the Lantus?

Yes, I will have U syringes with half units for the Lantus.

How often should I test? I know @AMPS & @PMPS definitely! I normally test any time I'm at home. Definitely at +2 and +6 after AM shot. +2 and +4 at PM shot
 
You're doing great with the testing. We like to get 2+, 4+ 6+ and of course AM and PM pre-shot levels. At any time that things seem to be going a bit too fast, you can always test more - look around at some of our spreadsheets and you'll see there are a lot of people, myself included who test hourly when our kids are surfing in the lagoon. Luci got away from me today, I should have tested her at 2+and I didn't...she broke her foot recently and hasn't been doing much - but apparently she's getting better from that episode and decided to run off and visit the sharks (too low of a number)! Silly girl!

Hello and welcome Giselle. I too started on Caninsulin (Vetsulin is just US rebranding) and then switched to Lantus. I think you and Triton will like it better. My girl did. And not only that the syringes were smaller needle gauge.

You would probably be fine starting at 2 units. I would have said less if you were seeing green. It it’s OK to down to 1.75 if you want an extra margin of safety.

Note that Lantus is a depot insulin, so it may be a few cycles before you start seeing much action.
 
I think the others have covered a good deal of what you need to consider at this point in time. There's some new stuff to learn but you've got the basics and lots of people to support you and help you learn the ropes. I'm sure you and Triton will be happy with the switch.

The only thing I can think of is an explanation of the depot. Vetsulin is an in/out insulin so you give the shot and it works for a given amount of time (usually about 8 to 10 hours) and then it's done. With Lantus, there is a small amount of insulin "stored" as a depot every time you shoot so it takes a few days to get the depot filled and that depot creates some overlap between shots so the ups and downs in BG are not as abrupt as they are with Vetsulin.

I'll be watching to see how you and Triton are getting along with the change. :)
